Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The key findings are as follows: In 2009, the number of work-related burns in Michigan based on the multiple reporting sources was 1,461 which is 3.25 times greater than the official estimate of 450, which is based on reports from employers to the Bureau of Labor Statistics. Hospital/emergency department reports identified 1,248 work-related burns, Michigan Workers' Compensation Agency identified 306, Michigan Poison Control Center identified 106, and the Michigan Fatality Assessment and Control Evaluation program identified two deaths from work-related burns. Among the 1,662 reports received from the four reporting sources, 201 were reported by more than one source; thus the total number of work-related burns identified in 2009 was 1,461. Because eight individuals had two separate burn incidents, 1,453 individuals were burned at work in 2009. The most common type of medical encounter was an emergency department visit (75.6%). Sixty-one percent of all burns were in male workers and 85.3% in Caucasians. The most common part of the body burnt were wrists and hands (34.2%) and upper limbs (20.6%). Second degree (65.4%) and thermal (65.7%) burns were the most common types of work-related burn. Fifty percent of work-related burns occurred to workers in either the Accommodation and Food Services or Healthcare and Social Assistance industries. The truck operator exited the truck's cab and noticed that the truck was on top of the victim, he then moved the truck off of the victim. Emergency medical services (EMS) arrived within minutes of notification and transported the victim to a local hospital where he died later that same day. Contributing factors identified in this investigation included: the contractor lacked an Internal Traffic Control Plan and backing procedures, and the dump truck was not equipped with monitoring technologies to assist the truck operator monitor vehicle blind spots. In addition, the police department did not provide work zone training for police officers performing details for construction projects. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, contractors performing work on or around roadways should: 1. Develop, implement, and enforce an Internal Traffic Control Plan (ITCP) specific to each construction site to minimize vehicle backing and to help protect workers on foot AB - 2. Ensure backing protocols are in place and that designated individuals are assigned as signalers to direct backing vehicles on construction sites AB - and 3. Consider installing monitoring technology on construction vehicles and equipment to assist operators in detecting workers on foot within blind areas. Municipalities should: 1. Provide work zone safety training for all employees who perform work on or around roadways, including traffic details AB - and 2. He had been at this site for about a week. The other worker was a tractor operator/pesticide applicator who was operating a tracked tractor pulling a 1,600-gallon trailer-mounted pesticide tank to fumigate his employer's fields. The two workers started work at 1:30 p.m. and were scheduled to work to 11:30 that evening. At about 7:20, the victim exited his parked pickup truck on the edge of a field and walked toward the tractor, which was actively applying fumigant. He did not communicate with the operator. As it was dark and the victim was not wearing a high visibility vest, the tractor operator did not see the victim approaching the moving tractor until he was a short distance away. He began to slow the tractor from about 5.5 mph to 5 mph. The victim approached the tractor's left side and reached for the cab access ladder handrail and attempted to place his left foot on the ladder's bottom step. As he was attempting to mount the ladder of the moving tractor, his right leg was caught by the left rear track and he was pulled under and run over by the track. Investigators determined that the victim had previously ridden along with the operator, but he had entered the operator's cab only when the tractor was stationary after the victim had refilled the tractor's pesticide tank. The tractor operator's manual stated that the extra seat in the cab is to be used only for instructional and service diagnosing purposes. Then read the story. Our safety talk today is about a 42-year-old logger who was killed after being struck in the head by a falling sheave. The logger was standing in the landing zone when one guyline slipped off its anchor stump, and a second guyline slipped off when its anchor stump came out of the ground. This caused the tower of a yarder to collapse and strike the boom of the delimber. There was a cable pulley system attached to the delimber, and the impact of the delimber's collapsed tower caused a sheave to fall off the boom and strike the victim in the head. He died of acute head trauma. So here are some ways we can prevent something like this from happening where we work: Plan the landing site for yarding requirements beforehand to ensure that machines maintain sufficient safe distance. Make sure to check that anchor stumps are at the correct height to secure guylines, and the guylines will not interfere with decking the logs at the landing. Follow manufacturer's' recommendations for setting up guylines. Have a competent person check guyline anchors daily, before and during operation. Make sure that there is deflection in the skyline to reduce stress on the yarder tower. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Have we checked the guyline anchors today? Are the anchor stumps the correct height?" "Does anyone know the manufacturer's recommendations for setting up guylines?" "What do you all do to set-up the landing site for yarding?" Discuss a similar situation at your current site. The victim was attempting to fell one of the Douglas-fir trees at the time of the incident. He was later found by his cutting partner underneath the alder tree limb. The victim's chainsaw was used to cut the limb off of him. He was later pronounced dead at the scene. So here are some ways we can prevent something like this from happening where we work: Make sure to check for hung or snagged trees and limbs in your area and the area around you, and communicate with each other about hazards. Get help from a supervisor, a cutting partner, or a more experienced worker if you notice a hazard. Management or I need to make sure everyone is trained to safely deal with snagged or hung trees and other logging hazards. Management or I should provide an interpreter if needed, and extra training and supervision to promote safety on site. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. "Have you seen any snagged trees or limbs or other hazards that should be discussed with your supervisor or cutting partner?" The seven-man crew had finished setting up a construction zone on a two-lane highway to repair damaged guardrails along the westbound side of the roadway. The crew had placed alert signs in each direction of travel in preparation to stop all traffic prior to completely closing the westbound lane and had just dropped off the victim, who was designated as the flagger for the eastbound traffic. Traffic flow was to be coordinated with flaggers by two-way radios. Witnesses stated that the victim walked out into the eastbound lane with his back to oncoming traffic while facing the other flagger and the lane of traffic he stepped into had not yet been stopped. The victim was struck from behind by a passenger vehicle traveling approximately 55 mph. Police interviews with the driver stated that his vision was obscured due to the glare of the sun through his windshield and he did not see the victim. Recommendation No. 1: Flaggers should never enter the roadway before traffic has been stopped and never turn their backs towards oncoming traffic. Recommendation No. 2: Operators of motor vehicles should always adhere to roadway warning signage by slowing down and paying close attention while in a work zone area. Recommendation No. 3: Employers should train employees quarterly to keep flagging safety policies and procedures fresh in their minds while working in highway situations. This is the 12th annual Michigan Fatality Assessment and Control Evaluation (MIFACE) report on acute traumatic work-related (WR) deaths in Michigan. There were 135 WR deaths in 2012, representing 131 employers and 133 separate incidents. Key findings for 2012: The number of WR deaths (135) and the fatal injury rate (3.2 deaths/100,000 workers) were down compared to 2011 (141 WR fatalities, 3.3/100,000 workers, respectively). The largest number of WR deaths occurred in Construction (20, 14.8%), then Transportation & Warehousing (19, 14.1%), then Agriculture (18, 13.3%) and then Retail Trade (12, 8.8%). Manufacturing, Administrative & Support & Waste Management & Remediation Services and Other Services each had 11 WR deaths (8.1%). Agriculture had the highest risk of death (20.7 deaths/100,000 workers), followed by Transportation & Warehousing (18.6 deaths/100,000 workers), and Construction (15.7 deaths/100,000 workers). The most common cause of death was from a motor vehicle (36, 26.7%), followed by homicide (28, 20.9%), then falls (16, 11.9%), and then non-vehicular machines (14, 10.5%). The cause of death could not be determined for one individual. Individuals who died were most likely to be men (92.6%), white (79.2%), married (59.7%) and have at least a high school education (70.0%). The average age was 48.3 years old and ranged from 18 to 84 years of age. Thirty percent of work-related fatalities occurred to college-educated individuals, the highest percentage since this was first tracked in 2001. Illegal drugs, alcohol or side effects of prescribed medication was a potential factor in approximately 10% of the deaths. The largest number of work-related traumatic events occurred on a Wednesday (26) followed by Monday (23). March and June were the most common months (16), and 12:00p.m. to 3:59p.m. was the most common time (33). Forty (41, 49.4%) of Michigan's 83 counties had a fatal WR incident. Wayne County had the highest number of WR fatal incidents (38, 28.4%), followed by Oakland County (10, 7.5%). The county of incident was unknown for one fatal incident. Of the 135 WR fatalities, 27 (20.6%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. The number of WR homicides in 2012 nearly doubled compared to 2011 (28 homicides in 2012 compared to 15 homicides in 2011). A narrative summary of each WR fatality is in Appendix I. The decedent and his coworker were elevated approximately 15 feet in the scissor lift while they painted a building's eaves/fascia. To protect the ground from paint overspray, the workers laid opaque plastic on the asphalt pavement. The plastic covered a storm drain grate, which was recessed 4½ inches. The workers noticed an area that had been missed during their painting work, so the lift had to be moved back to the missed area on the fascia. Both workers knew the storm drain grate was nearby but thought it was several feet away. While the lift was elevated, the decedent was in charge of the operating the lift. While in the process of backing the lift to the missed area of fascia, the lift's left rear tire entered the 4½-inch storm drain depression, which was located approximately 20 feet away from the building causing the lift to tip to its side toward the building. The decedent's coworker jumped from the lift as it was falling and received non-fatal injuries. The decedent stayed in the lift basket and struck the ground, receiving head injuries. Emergency response was called and the decedent was transported to a local hospital where he died two days later from his head injuries. Factors which contributed to this fatal incident include: 1) Site hazard (drain and associated deteriorated surrounding pavement) was identified but corrective measures not undertaken AB - 2) Unmarked site hazard covered when pavement and drain were covered with plastic AB - 3) The lift was elevated when it entered the storm drain depression AB - 4) Spotter was not used while operating the elevated lift. RECOMMENDATIONS: 1. Assure work areas are inspected for hazards and take corrective actions to eliminate the identified hazards before and during operation of an aerial lift. 2. Employers should ensure that workers who operate self-propelled elevated lifts are adequately instructed in how to perform their duties and trained to safely operate the lift. Training should include, but not be limited to: a) how to perform a work area inspection to identify and minimize identified hazards AB - b) how to operate the lift in an elevated position AB - c) how to perform a pre-shift inspection that includes a visual and operational check AB - and d) report to management when the lift does not pass a visual or operational check. 3. Employers should maintain equipment in accordance with the manufacturer's recommendations. 4. Employers should develop formal, written and specific equipment instructions to be used as training tool to ensure employees receive consistent training. 5. Employers should, in addition to developing and implementing a health and safety program, develop mechanisms to ensure adherence to the health and safety program by both management and employees. One approach is ensuring the company's culture has safety as a core value. Kentucky Injury Prevention and Research Center AB - February 17, 2014, on a cloudy Monday, around 4:40 p.m., a 40 year-old male tanker truck driver (victim) was driving down a straight level 3-lane interstate headed north bound to a neighboring state going approximately 70 m.p.h., to deliver his 45,000 lbs. of liquid sodium sulfate when he came upon a slow moving vehicle in the right lane, the victim swerved to the left to avoid the vehicle and his 2001 Kenworth tractor trailer left the road. The victim then overcorrected and his semi flipped landing on its top in the right side of the roadway in a grassy area with a rock cut ditch. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Commercial motor vehicle drivers should avoid distracted driving to prevent resultant collisions and driver errors. Recommendation No. 2: Commercial motor vehicle companies should consider using Defensive Driving training for their employees on an annual basis. Recommendation No. 3: Commercial motor vehicle drivers should be trained to know the signs of fatigue and steps to take to eliminate driving while fatigued. The planks he was walking on gave way, and he fell 96 feet and struck a concrete barrier on the ground. He was 45-yr old. What Can Be Done to Prevent This from Happening Again: Conduct a job hazard analysis and put into place a safety and health plan where workers are assigned tasks. Provide fall protection equipment to all employees working at heights of 6 feet or greater. Use wooden planks or other structures for temporary work platforms that are made of materials that can support at least four times the maximum intended weight, as required by OSHA. Design and install individual planks so that shifting does not occur. Designate a person to actively survey the planks and promptly replace those that are damaged. Clearly mark surfaces that are not designed for worker access to prevent entry. The decedent had unsuccessfully attempted to remove a 12-foot section of the rail of a railroad on the south side of the facility with the tractor bucket. The facility's General Manager determined that another method should be used to remove the rail. The decedent was in the process of backing the tractor away so he could move it to another area. The tractor length, including the bucket was approximately 15 feet. The decedent backed the tractor approximately 20 feet. The ground was snow covered. The back wheels of the tractor slipped into an approximate nine-foot-deep ravine with an unmarked edge. As the tractor was sliding down the ditch, he attempted to drive the tractor forward, but the tractor continued to slide into the ravine. When the front wheels of the tractor reached the edge of the ravine, the tractor overturned to the rear. The decedent was pinned against the ravine bank by the tractor seat. A ROPS was an optional piece of equipment when the tractor was built in 1972. Factors that contributed to this incident include: 1) Tractor not equipped with a ROPS and seatbelt AB - 2) Edge of embankment not marked AB - 3) Frozen, snow-covered ground AB - 4) Two-wheel drive tractor was not equipped with chains for traction. RECOMMENDATIONS: 1. Retrofit older tractors with properly designed and manufactured ROPS and seat belt. If a ROPS is not available, consider replacing the older tractor with a tractor which can be equipped with or already has a ROPS. 2. To alert workers, mark edges of an embankment with visual cues, such as elevated, reflective stakes, orange fencing, or permanent barriers when in a traveled work area to alert workers. 3. Tractor operators should use extreme caution when using tractors on or near sloped terrain, ditches, or embankments. 4. Management should develop and implement a written safety and health program and train employees about the program. 5. Install tire chains on two-wheel drive tractors during winter to aid traction. After the pressure cooker was turned off and the outlet was opened, the victim was discovered dead inside. The CA/FACE investigator determined that, in order to prevent future incidents, food processing companies should ensure that: 1. Periodic safety audits and inspections are conducted and documented to identify all hazardous work areas. 2. Employees working in or around confined spaces are properly trained and the training is documented. Employees working at elevated heights wear fall protection when necessary. 2. Employees are properly supervised when working at elevated heights. 3. A site-specific fall protection plan is designed and implemented for work to be performed at elevated heights. The co-worker on the roof tried to grab the sliding plywood by jumping onto it, but could not stop it. The co-worker's life line prevented him from falling over the edge of the roof. As the plywood reached the edge of the roof the wind whipped it 30 feet from the building, striking the Hispanic laborer on the back of the head just below his hard hat. A crane operator witnessing the events unfolding immediately began blowing his horn trying to warn the laborers on the ground, but it was too late. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: When conducting overhead work, employers should take precautions to protect workers on the ground from falling objects, tools and debris. Recommendation No. 2: When possible, employers should move workers out of the hazard area when conducting overhead work. Recommendation No. 3: When employers hire foreign workers, they should have training materials printed in the native language and provide extensive safety training to ensure understanding. Recommendation No. 4: When the presence of high winds occurs, employers should consider postponing high overhead work to eliminate many incidents that could occur. Occupational and Environmental Medicine AB - In the winter of 2013, a male roofer in his 40s died when the cantilevered 2" rigid roof insulation he stepped on broke, causing him to fall approximately 26 feet to the frozen ground. The firm had a fall protection program including a safety monitor, warning line, and personal fall arrest system (PFAS). The foreman had dual responsibilities: installing insulation and acting as the warning line's safety monitor. The decedent was not wearing fall protection while working inside of the warning line attaching insulation. A coworker who was wearing a full body harness with retractable lanyard and tied off to an approved anchor point screwed to the metal roof deck was working at the roof edge cutting the insulation flush with the roof. While leaning over the edge, the coworker's cell phone fell out of his pocket to the ground. The foreman had left the roof to obtain more roofing material. When the coworker left to retrieve his cell phone, the decedent left his work area to finish the remaining four feet of insulation work at the roof edge. He did not put on his full body harness and attach to the anchor site. A coworker saw him, stand up and stretch, and then take a step backward onto the insulation extending over the roof edge. The overhanging insulation was unable to support his weight and broke, causing him to fall approximately 26 feet to the frozen ground below. Emergency response was called. The decedent was airlifted to a local hospital where he died several hours later. MIFACE investigators identified the following items as key contributing factors in this incident: 1. Personal fall arrest system not utilized. 2. Safety monitor had other duties and not present on roof at time of incident. 3. Cell phone on roof in violation of company policy. 4. Possibility of wind speed as a contributing factor. 5. Unusual roof top work (no parapet structure) for the company. RECOMMENDATIONS 1. Employers should ensure that workers are protected against falling while working at an elevation. This includes, but is not limited to, maintaining the integrity of an established warning line system and ensuring the use of personal fall arrest equipment. 2. Employers should, in addition to developing and implementing a health and safety program, develop mechanisms to ensure adherence to the health and safety program. One approach is to ensure the company's culture has safety as a core value. 3. The State of Michigan should distribute MIOSHA-required safety and health regulations when individuals apply for a Builders or Maintenance and Repair License and to all roofing companies. An alternative approach would be to require commercial roofing companies and roofing companies directly subcontracted by all building contractors to receive training analogous to a MIOSHA Construction 10-hour course. Department of Public Health AB - On June 19, 2012 a 63-year-old male municipal signal operator/electrician (victim) was fatally injured when he fell out of a vehicle-mounted aerial lift's raised bucket. The victim and one co-worker were at a four-way intersection replacing a traffic signal bulb. The victim was inside the raised bucket accessing the cantilevered traffic signal when a tractor-trailer driving through the intersection struck the raised bucket. The victim was ejected out of the bucket and fell approximately 17 feet to the roadway below. A call was placed for Emergency Medical Services (EMS) by the driver of the tractor-trailer. Within minutes EMS and the local police arrived at the incident location and the victim was transported to a local hospital where he was pronounced dead. Contributing factors identified in this investigation included: no fall protection used when working from a raised aerial lift bucket AB - positioning of the aerial lift truck that allowed vehicles to pass under the raised bucket AB - and improper work zone set up including inadequate quantity and positioning of temporary traffic control devices. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1. Provide and ensure that fall protection is used when working from an aerial lift truck's raised bucket/platform AB - 2. Ensure that when performing work in roadways that work zones are set up, at a minimum, in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part 6, developed by the U.S. Department of Transportation Federal Highway Administration AB - 3. Ensure that employees' exposure to moving traffic is minimized when working in and around roadways by developing temporary traffic control plans, which never allow traffic to pass underneath raised aerial lift buckets/platforms AB - 4. Provide and ensure that employees working along roadways wear the appropriate American National Standard Institute (ANSI) compliant high visibility safety apparel AB - 5. Provide work zone safety training for all employees who will be required to complete tasks while in proximity to roadways AB - 6. Ensure that each department develops, implements, and enforces a comprehensive health and safety program that includes hazard recognition and avoidance of unsafe conditions AB - and 7. Provide work environments that, at a minimum, meet all relevant Occupational Safety and Health Administration (OSHA) regulations and industry accepted standards of practice. In addition, manufacturers of aerial lifts and employers should: 8. Consider using contrasting colors and installing strobe lights along the booms of aerial lifts.
