Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Emergency services were called and the victim was in cardiac arrest when emergency personnel arrived. The victim died 20 minutes later in the hospital from injuries sustained from the fall. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Roofing industry employers must train their roofers in fall protection including how to wear and how to inspect their equipment for defects. Recommendation No. 2: Employers must ensure that worker safety is adopted by all employees from the top management down to laborers. Recommendation No.3: Kentucky OSHA enforcement regulations must be stricter for repeat offenders and willful citations. Recommendation No 4: Employers should provide written worker safety programs as well as ensure that all employees are annually trained on safety programs including the use and care for personal protection equipment. The scrap processor fit the wrench on the bolt and an excavator operator applied tension to the sling on the excavator. Once the bolt was loosened, the scrap processor signaled the excavator operator to lower the tension, after which the scrap processor moved the wrench to the next bolt. After successfully removing 40 bolts using this method, the scrap processor placed the wrench on the next bolt and signaled the excavator operator to apply tension. While tension was applied, the victim grabbed the sling and leaned over to observe the bolt. The wrench slipped off the bolt, shot out and struck the victim on the right side of his head causing him to fall 6 feet to the ground, striking several pieces of scrap metal. Emergency Medical Services were called at 11:40 am. They arrived approximately 15 minutes later and the victim was flown to a Level 4 trauma center. Physicians placed the victim into an induced coma. The victim succumbed to his injuries 13 days later and died. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should ensure employees use appropriate tools for the required job. Recommendation No. 2: Employers should ensure employees use appropriate personal protective equipment such as hard hats, and fall protection. Recommendation No 3: Employers should require guarding or ensure that fall protection is utilized when employees are working four feet or more above ground. Recommendation No 4: Employers should develop and implement a comprehensive safety and health program that includes an operating procedure for removing bolts on a locomotive flywheel. The decedent was in the process of checking the truck for an oil leak. The truck had been driven up a slight incline and parked with the front wheels on a cement apron surrounding a building. The front wheels were positioned several inches east of a 5-inch deep trench and the back wheels on dirt in a concave-shaped 3-inch trench. There was a consistent elevation decline from the building to the west to allow for water drainage. The decedent started the truck, placed it in neutral, and partially depressed the parking brake. No wheel chocks were used to prevent truck movement. At some point while lying on his back working under the truck, the truck moved backward, with the front wheels entering a 5-inch deep dirt trench. The truck's sway bar and link to the rear of the front axle fell on the decedent's chest, pinning him against the cement. A building employee had left approximately one hour earlier and had noted the truck running. When he returned, the truck was still running, so he looked under the truck. Seeing the decedent, he screamed for help. Emergency response was summoned. A jack was used to raise the truck. The decedent was transported to a local hospital and declared dead. Factors which contributed to this fatal incident include: 1. Vehicle not blocked from inadvertent movement 2. Unsafe work practices. Recommendation: Persons performing vehicle maintenance should ensure that vehicles are adequately supported while maintenance is performed underneath and that vehicles are protected against inadvertent movement. The decedent started his generator in the back of his pickup that was parked approximately six feet from a two-track road to power his Craftsman 16-inch, 3.5 HP electric chainsaw. Police pictures show that the tree fell directly toward another tree. As it was falling, the top portion of the tree broke away, presumably when the top of the tree struck the tree in its path, and came back toward him and struck his head. The actions of the decedent just prior to being struck by the top of the tree were unknown. After placing a deer camera, a hunt club member who had spoken with the decedent approximately 10 minutes earlier, came back down the two-track but did not see him. He went to look for him and saw the decedent lying on his back near the stump of the tree he had just felled. He called for emergency response. The Medical Examiner arrived and declared him dead. MIFACE identified the following causal factors contributing to this incident: 1. Insufficient felling and site assessment techniques. 2. Lack of personal protective equipment. 3. Working alone. RECOMMENDATIONS: 1. Fellers should adhere to proper felling techniques, including conducting both a tree and a site assessment to determine and mitigate identified hazards, use proper notching techniques, appropriate personal protective equipment, and never turn their back on a falling tree. 2. Fellers should work in teams while felling trees, or alternatively, have a reporting procedure in place if working alone while felling trees. 3. Hunt clubs should develop a safety policy for members that includes at a minimum, working alone policies, emergency phone numbers, location of nearest emergency medical facility, etc.) and post them at the facility/circulate them among the membership. The decedent constructed the drag using two 4-inch by 6-inch by 14-foot treated wood posts attached to four 8-foot channel-style steel beams and one shorter middle channel-style steel beam. The drag was attached to the lift arms of the tractor's three-point hitch. At the second steel beam, the decedent had attached a screw-in bolt in the wood, attached the chain to the bolt, and the chain to each lift arm. At some point during the leveling of the soil, the drag caught on something on the ground and "flipped" up, coming to rest upright against the back of the tractor. One of the drag's steel beams struck the decedent's head. Police indicate it appeared the tractor stopped immediately. When the decedent was found, the tractor was still running. Emergency response was called and he was declared dead at the scene. Factors contributing to this incident: 1. Rutted, clumpy, uneven soil in area he was leveling. 2. Construction of the home made drag. 3. Chains vs hard attachment to tractor. 4. No roll over protective structure (ROPS) on tractor. RECOMMENDATIONS: 1. Evaluate site conditions and select proper tool for the task to be performed. 2. Older tractors should be retrofitted with a roll over protective structure (ROPS). The victim parked the forklift at the end of a warehouse aisle, elevated the platform to approximately 10 feet to help reposition metal racks, walked out onto the platform, and fell off the open end and landed on the cement floor. The victim was wearing a body harness AB - however, his lanyard was not connected to the anchor point on the forklift. The victim sustained trauma to the head, eye, shoulder, and wrist, and died eight days later from these injuries. Contributing Factors: 1. Unsecured edge of lift platform 2. Lanyard not connected to anchor NJ FACE investigators recommend that these safety guidelines be followed to prevent similar incidents: 1. When operating an elevating platform forklift, a personal fall arrest system should be worn at all times, which consists of an anchorage, connectors, body harness and retractable lanyard connected to appropriate anchor point. 2. Prior to elevating the forklift, all workers should be tied into the anchor point on the cab. 3. Forklift manufacturers should consider designing a barrier on the unsecured edge of platform, and a lock-out tag-out system where the machine will not lift unless the anchor is in use. 4. A safety and health plan based on a job hazard analysis should be developed by the employer and followed where workers are assigned tasks. Shortly thereafter, about ½ mile from the landing, another truck driver noticed tire tracks that trailed off the main haul road into a steep canyon. He then saw the wrecked truck and trailer below. The driver was found next to the rear axle of the truck, approximately 150 yards below the road. There were no skid marks or steering corrections indicated by the tire tracks suggesting that the driver inadvertently drove off the road after encountering the dense fog condition. RECOMMENDATIONS 1. Employers should train truck drivers to recognize unsafe driving conditions and to stop operations when conditions are unsafe 2. Logging truck fleet owners that require operation under fog conditions should install front fog lamps in trucks and ensure that lights are working and windshields and cab windows are clean. 3. Drivers should use seat belts when operating a logging truck and employers should enforce existing seat belt use policies. 4. Drivers should clean their windshields as often as needed and conduct regular vehicle inspections to ensure that brakes are working correctly. Then read the story. Our safety talk today is about a 37-year-old field mechanic who died when he tried to stoke a warming fire with gasoline. The victim and a coworker were at a logging site to work on a log-loading machine. The weather was cold and rainy. The coworker started a warming fire with diesel fuel and chainsaw gasoline. He left for about 15 minutes and came back to find the victim lying 35 feet from the warming fire engulfed in flames. There was another fire about 20 feet from the warming fire that was later found to be a burning 5-gallon plastic gasoline container. The victim was pronounced dead at the scene from severe thermal injuries. So here are some ways we can prevent something like this from happening where we work. Never use gasoline or saw fuel near an open flame. Make sure to use the correct materials to safely start and stoke a fire. For example, use a drip-torch with a mixture of diesel fuel and gasoline (ratio 3:1 or 4:1). Management and I must make sure that employees understand that when pouring gasoline on an existing fire, a flame can travel up the stream into the container and explode. Management and I must train employees on the hazards and safe procedures for starting and stoking fires. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "What other precautions do we need to take when handling gasoline?" "What would you do if you are outdoors and it gets cold and rainy?" Discuss a similar situation and what employees can do. Express your commitment to providing materials needed for the work environment. Commit to follow-up at the next safety talk. The incident happened at a hospital parking lot where his employer was a subcontractor hired to install a storm water drainage system. His employer was a site preparation contractor. The victim was an experienced laborer and a member of the Laborers' International Union of North America. His job duties for this project included digging trenches and laying and connecting storm drain pipe. On the day of the incident, the victim and two other employees were digging trenches and installing storm drains. At the location they were working there was a buried duct bank which was in the way and conflicted with the plans for installing the storm drain. This duct bank contained three lines of PVC electrical conduit piping encased in concrete. Each line of conduit piping contained four power line cables carrying 7,200 volts each. In order to install the storm drain pipes to the necessary grade, the employees were using an excavator, breaker bar, and a rivet-buster-type jackhammer to chip away at the duct bank concrete. The victim was in the trench chipping the duct bank's concrete when his rivet buster punctured the conduit and contacted the power line cable. He was electrocuted and died at the scene. This was the victim's first season working at his employer's hop farm. He had been employed there for six months. His job duties included hauling bales of hops from the field by operating a tractor with a trailer. On the day of the incident, he was operating a utility tractor to tow a 25-foot empty flatbed trailer from the farm's shop to the equipment storage area. As he was moving through the level dirt yard, the trailer separated from the tractor and bounced up and overtook the tractor. The front of the trailer landed on the tractor. The victim was crushed between the trailer's frame and the tractor's steering wheel. Investigators determined that the trailer was incorrectly attached to the tractor. Instead of using a hitch pin to connect the trailer's tongue to the tractor's hitch, a bolt was used. There was no physical evidence found at the scene to indicate that the bolt was secured at its bottom to prevent it coming loose. The tractor was found in its highest gear with the hand throttle in the lowest position (indicating that the victim was using the foot throttle to control the tractor's speed). These two facts suggest that the tractor may have been travelling at a high rate of speed. Additionally, there was no rollover protective structure (ROPS) on the tractor. A ROPS might have provided protection to the operator when the trailer landed on the tractor. He was operating a tractor pulling a trailer when the trailer detached and came forward and landed on top of the tractor and crushed him. This was the victim's first season working at his employer's hop farm. He had been employed there for six months. His job duties included hauling bales of hops from the field by operating a tractor with a trailer. On the day of the incident, he was operating a utility tractor to tow a 25-foot empty flatbed trailer from the farm's shop to the equipment storage area. As he was moving through the level dirt yard, the trailer separated from the tractor and bounced up and overtook the tractor. The front of the trailer landed on the tractor. The victim was crushed between the trailer's frame and the tractor's steering wheel. Investigators determined that the trailer was incorrectly attached to the tractor. Instead of using a hitch pin to connect the trailer's tongue to the tractor's hitch, a bolt was used. There was no physical evidence found at the scene to indicate that the bolt was secured at its bottom to prevent it coming loose. The tractor was found in its highest gear with the hand throttle in the lowest position (indicating that the victim was using the foot throttle to control the tractor's speed). These two facts suggest that the tractor may have been travelling at a high rate of speed. Additionally, there was no rollover protective structure (ROPS) on the tractor. A ROPS might have provided protection to the operator when the trailer landed on the tractor. They had finished work on the chimney and were in the process of swinging the bucket around to bring it back to the ground, when the bucket came in contact with overhead power lines running behind the house. The bucket hit a 7,200 volt line and the two masons were electrocuted immediately. They were not discovered until the local electric company was notified of a power outage and sent a crew to discern the cause of the outage. More than one hour elapsed between the time of the incident and arrival of the utility workers. The utility workers arrived at approximately 11:50 am and discovered the two masons still in the elevated bucket of the aerial lift, part of which was on fire, and immediately called the fire department. The two masons were declared dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employees should conduct a jobsite survey to identify potential hazards and develop and implement appropriate control measures for the hazards. Recommendation No. 2: Employees should follow existing OSHA regulations and safe work practices concerning the operation of equipment in close proximity to overhead power lines, and take steps necessary to de-energize or insulate power lines before work begins. Recommendation No.3: Employees should ensure that when working near a high voltage overhead power line where visibility could be obstructed or clearances difficult to determine, an observer is used to help the operator maintain the required clearance. Recommendation No 4: Employees should be trained in hazard recognition and the avoidance of unsafe conditions. Recommendation No. 5: Employers should ensure that workers who operate aerial lifts are properly trained in the safe use of the equipment. After freeing the vein feeder, the foreman started the mixer section to remove the dried concrete. This created a cloud of dust, and the foreman instructed the two employees to leave the area to avoid breathing the dust. As the foreman started to leave the area, he stepped on one of the four doors covering the mixer section. Unknown to the foreman, the door had broken hinges and missing interlocks. Both the foreman and the defective door fell into the running auger mixer section. By the time one of the employees hit the emergency stop button to shut off the mixer, the foreman was chest high in the auger mixer and had died from his injuries. Emergency medical services (EMS) were contacted, and the victim was declared dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation 1: Employers should develop and implement written procedures for maintaining the Rapidmix 400 C1 continuous mixing plant to include pre-inspection, cleaning the mixing plant inside and out immediately after each use, making prompt repairs to broken parts such as the broken door hinges, and promptly replacing missing and /or damaged parts such as interlocks for the doors covering the mixer section. Recommendation 2: Employers should train employees on hazard recognition for all aspects of the job involving such equipment as the Rapidmix 400 C continuous mixing plant including instructions not to walk on or stand on the doors covering the auger section of the mixing plant while the plant is in operation. Recommendation 3: Employers should provide documented refresher worker safety training annually or prior to the mixing plant's first use each year. Recommendation 4: Manufacturers should consider installing a switch so that the equipment doors could not be open during operation. The 380-foot tall tower was supported by 21 guy wires at seven elevations, three at each height. The lowest three guy wires were at 43 feet. The victim and his coworkers were replacing the guy wires at the time of the incident. The workers first released the lowest three guy wires from the ground anchors. The next set of wires was at 93 feet. The tower immediately bent in response to the removal of the tension in the lowest guy wires. Tension gauge readings indicated that the remaining guy wires were either overtensioned or tensioned unevenly. The workers adjusted the wires so that the tower was not bent. The victim, who was wearing a safety harness, started climbing the tower to remove the wires at the 43 feet. As he was climbing, the workers on the ground observed the tower to be moving "like a wet noodle." The victim dropped the three wires and quickly climbed down. He then climbed to 43 feet to install the new guy wires. He successfully attached two wires before switching to the other side of the tower to attach the third wire. Meanwhile, two workers on the ground picked up one of the wires that was just attached and pulled it hand tight. They took enough slack out of the wire so that it was elevated and not touching the ground. At this moment, the tower start to collapse. The tower reportedly buckled or bent at 43 feet where the victim was. The lower section of the tower (from 43 feet down) fell northwest, while the section above 43 feet fell to southeast (180 degrees from the direction where the lower section fell). The whole tower collapsed. The victim, whose harness was still hooked to the tower, suffered fatal crushing and fall injuries. One worker called 911 and the rest ran to attend the victim. The emergency response staff arrived within minutes. The victim was pronounced dead at the scene. CONTRIBUTING FACTORS: 1. Temporary guy wires were not installed to support the tower. 2. Removal of the guy wires at the 43 feet resulted in an unsupported lower section of 93 feet. 3. The new guy wire was pulled hand tight without touching the ground. The weight of the wire added a lateral force to the tower. 4. The guy wires were overtensioned. 5. The weight and movement of the climber might have further destabilized the tower. KEY RECOMMENDATIONS: 1. Communication tower owners should have a structural analysis completed and the tower structural integrity confirmed before sending contractors to work on a tower. 2. Communication tower contractors should use temporary guy wires to maintain tower structural integrity when replacing guy wires. 3. Communication tower contractors should conduct a job hazard analysis (JHA) and develop standard operating procedure prior to conducting any tower maintenance work. 4. Communication tower contractors should provide employee training on hazard recognition and prevention. 5. Owners of communication towers should maintain tower manuals, and tower construction and maintenance records. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers requiring employees to access the top of tankers must provide adequate fall protection. A crane operator was lifting a precast panel from an A-frame located at the rear of the trailer facing the driver's side. A single looped tag line approximately 45 feet in length was in use. The following sequence of events is postulated: As the precast panel from the rear of the trailer was lifted up and over toward the truck cab, the looped tag line caught the edge of the unsecured panel at the front passenger side A-frame. The decedent was standing near the passenger door and noticed the looped tag line catch on the corner of the incident panel. As he was approaching to move the tag line away from the panel, the tag line ran out of slack, tightened, and exerted enough force to cause the panel to fall from the A-frame, strike his right shoulder and head, and then pin him to the ground. After the panel fell, the crane operator lowered the precast panel he was lifting. Coworkers unhooked this panel and then hooked the crane to the panel pinning the decedent. The panel was lifted from the decedent and emergency response was called. The decedent was transported to a local hospital and declared dead. MIFACE identified the following factors to be contributory to this incident: 1. Unsecured concrete panel. 2. Looped tag line. 3. Unloading sequence of trailer. 4. Truck driver entered swing radius of crane and load. RECOMMENDATIONS: 1. Transport devices, such as shoulder bolts, should be utilized to secure individual concrete sound panels to the A-frames during transport and unloading unstrapped loads. 2. Standard Operating Procedures (SOPs) should include work practices that address load securement during the unloading process. When load configuration permits, truck drivers should wait to release the load securement tie down assemblies until after the material to be unloaded is secured with the unloading line or other unloading device. 3. Truck driver training, in addition to training regarding SOPs, should emphasize safe driver positioning during unloading and to never enter the unloading zone without confirmation from the individual unloading the trailer. 4. Additionally, crane operators and their employers should ensure employees follow appropriate crane operation procedures. There were two co-workers on site at the time of the incident. They both heard a popping noise and noticed that the victim was being electrocuted. They went to the work platform and could feel the electric current so they got the work truck and used it to push the work platform away from the overhead power lines. At the same time, two different home owners placed calls for emergency medical services (EMS). EMS, the fire department and the local police arrived at the incident location within minutes. The work platform was lowered and the victim was transported to a med-flight helicopter and brought to a large regional trauma center where he died of his injuries later that day. Contributing factors identified in this investigation included employees working within 10 feet of and coming in contact with energized overhead power lines, and that the company did not have an Injury and Illness Prevention Program (I2P2) and did not provide health and safety training. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure employees remain at least 10 feet away from any energized power lines located on worksites and provide training about the hazards of power lines and appropriate personal protective equipment AB - 2. Ensure that only "line-clearance tree trimmers" and power companies trim, access and maintain trees that are within 10 feet of energized overhead power lines AB - and 3. Develop, implement, and enforce an Injury and Illness Prevention Program (I2P2) that addresses hazard recognition and avoidance of unsafe conditions. In addition, home owners and property owners should contact power companies and/or "line-clearance tree trimmers" to maintain trees that are within 10 feet of energized power lines. The victim attempted to make another cut on the inside left fork, when the pole saw he was using became stuck. When the victim and a co-worker pulled on the pole saw, their hard hats fell off, and they did not put them back on. The victim used a Stihl MS 170 chain saw retrieved from the truck and sawed through the main trunk of the tree that caused the left fork section to drop and strike the victim in the face and head. The co-worker did not witness the victim being struck, but found him lying with a 9 foot section of the tree limb on him, bleeding from his nose and mouth. The co-worker called 911 and emergency crews responded to the scene. The victim was airlifted to the nearest trauma center. On Sunday, September 8, 2013, the victim succumbed to his injuries, expiring at 5:37 a.m. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation 1: Employers should ensure employees are using the correct equipment for the job. Recommendation 2: Employers shall provide applicable appropriate worker safety training specific to the employee's job responsibilities. Recommendation 3: Employers shall ensure that personal protective equipment (PPE) and special tools needed for work are provided, worn and used that are specific to their job responsibilities. Recommendation 4: Employers should implement and enforce a drug-free workplace program to ensure their workers are not working impaired. On the day of the incident, there were five employees, including the victim, on the job site. They were framing rafters for a covered patio. The victim was the lead framer. The victim and two other employees were taking measurements before making saw cuts in order to install the next set of rafters. The victim had been working from both a 12-foot extension ladder and a 6- foot stepladder. The investigators determined that it was likely that he fell from the stepladder. When he fell from the ladder he struck his head on the concrete patio. He died at the scene. The investigators found that both ladders were set up properly and that they had no defects.
