Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

TASK Using a reach forklift to place 1300lb steel trusses. HAZARDS Workers were on foot in the fall zone while the operator picked and moved the trusses. The employer used new slings and a new beam clamp but did not realize that this method needed to be approved by the manufacturer. TASK Using a rough terrain reach forklift to place 3000lb steel purlins. HAZARDS The chain wrapped around the carriage, outside of the 24" load center, created a tip-over hazard. The operator's manual warned, "Do not suspend loads using slings or chains directly from the forks or fork carriage". TASK Using a reach forklift to place 850lb wood wall sections. HAZARDS Workers were directly under the sections as they were being placed. The winch attachment slid onto the forks. The manufacturer and rating of the attachment were unknown. TASK Using a reach forklift boom to hoist a 1900lb sculpture. HAZARDS With the carriage removed, they used a chain wrapped around the knuckle pin, a come-along, and a sling to rig the sculpture. Plastic zip ties were used to keep the chain on the hook while workers positioned the sculpture. REQUIREMENTS: You must have written approval from the PIT manufacturer before making any modifications to the PIT. You must protect workers around PITs. The CA/FACE investigator determined that, in order to prevent similar incidents, employers with workers who operate water trucks should: 1. Ensure that the machinery and equipment provided to transfer high-pressure water can safely withstand all reasonably anticipated loads. 2. Implement policies and procedures as part of a safety program to ensure that unsafe conditions are identified, and safe work practices are followed. These are the key findings: Work-related crushing injuries were identified through multiple reporting sources 1) In 2016, there were 1,066 work-related crushing injuries, including four deaths in1,066 individuals. 2) In 2017, there were 1,079 work-related crushing injuries, including seven deaths in 1,076 individuals. 3) In 2018, there were 1,274 work-related crushing injuries, including five deaths, in 1,272 individuals. 4) Over the three years combined, there were 3,419 work-related crushing injuries in 3,414 individuals; 2 individuals each sustained 2 unique crushing injuries in the same calendar year and 3 individuals had 2 unique crushing injuries in two different calendar years. For 2016 through 2018, the Federal tracking system that relies on employer reporting, estimated only 1,030 work-related crushing injuries in Michigan or 30.1% of the total of 3,419 crushing injuries we identified in the three years (48.8% of our Michigan multi-source total in 2016, 20.4% of our total for 2017 and 22.8% of our total for 2018). The U.S. Bureau of Labor Statistics' estimated rate was 16 per 100,000 full-time equivalent (FTE) workers in 2016, 6 per 100,000 FTEs in 2017 and 9 per 100,000 FTEs in 2018, which was only 69.6%, 26.1% and 33.3% of the rate of 23, 23 and 27 per 100,000 workers of work-related crushing injuries identified in Michigan's multi-source reporting system. The most common type of medical encounter was an emergency room visit (2,556; 75.0%). Eighty percent of all work-related crushing injuries were among men and 81.6% were among Caucasians. The most common part of the body injured was an upper limb (2,387; 69.8%) followed by a lower limb (758; 22.2%). Two NORA Sector Groups - Manufacturing and Services (except Public Safety) accounted for over a half (57.2%) of all work-related crushing injuries. Mining (except Oil and Gas Extraction) Sector Group had the highest rate of crushing injuries with 147.4/100,000 workers, followed by Oil and Gas Extraction Sector Group with 123.5/100,000 workers. "Pinched between" and "Struck by falling object" were the two main causes of work-related crushing injuries with 37.0% and 21.4%, respectively. Workers' Compensation was the expected payer for only 71.1% of the 3,071 crushing injuries that were identified in the hospital/ED records and for which the payer type was specified. For 2016 through 2018, the Michigan OSHA program completed inspections at 50 worksites identified by the surveillance system as having had a crushing injury. MIOSHA issued 97 violations and assessed $287,250 in fines. In 42 of these 50 inspections the employer had not addressed the circumstances causing the crushing injury (e.g., no guard on the machine where the crushing injury occurred) even though the MIOSHA inspection was performed months after the occurrence of the injury. The laborer had worked in the construction industry for 12 years. He had received trenching training from his employer, a construction contractor, and had experience working in trenches. The laborer and a coworker, who was also the site competent person, were working at a residential site to install a new side sewer line. To access the line, they used excavators to dig a 25-foot-long by four-foot-wide trench. The trench depth varied from five feet to six and a half feet. They did not use a trench protective system, which is required at depths of four feet or more in Washington State. The laborer had been hand digging in the unprotected trench for two hours to expose a water main. As he was doing this, a section of the trench sidewall collapsed. He was buried in soil from his waist down. The coworker entered the trench and attempted to rescue him, but was unable to. He then used an excavator to dig out soil from around the laborer. The excavator's bucket struck and ruptured a water main, causing the trench to start filling. As the trench rapidly filled with water, bystanders responded. From the side of the trench, they held the laborer's head above the rising water while the coworker used the excavator to dig a side trench to drain the water. First responders arrived and after 30 minutes freed the laborer. He suffered pelvic fractures and internal injuries. Investigators found: 1) The trench was not protected from cave-ins by an adequate protective system. 2) Spoil piles three feet high of type C soil were placed within two feet of the trench edge. 3) Spoil piles were positioned on the uphill side of the trench creating a surcharge towards the trench opening. 4) It is possible that the excavators created vibration in the ground and caused the trench side to sluff. The victim entered the grain bin in an attempt to dislodge the product when the bridged corn collapsed resulting in the employee falling and becoming engulfed. CONTRIBUTING FACTORS Key contributing factors identified in this investigation include: 1. Lack of hazard recognition 2. Failure to utilize fall protection 3. No emergency action plan. RECOMMENDATIONS Kentucky FACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Employers should implement a job hazard analysis process. 2. Employers should implement an emergency action plan. 3. Employers should require employees to utilize adequate fall protection when entering a grain bin is necessary. 4. Employers should train employees on the known hazards associated with grain bin entry. This 42.3-foot steel hulled vessel experienced mechanical problems approximately 1 mile away from the harbor. It was raining and the seas were rough, with reported 6 to 8-foot swells on the north jetty and 10 to 12-foot swells on the south jetty and winds about 5 mph. As the Captain attempted to maneuver back to the bay, he heard a loud noise, and lost propulsion which meant maneuverability was also lost. He ordered the crewmembers to don immersion suits and called the US Coast Guard (USCG) to request immediate help. The Captain donned a PFD while in the cabin. While waiting for the USCG at the entrance to the bay, also known as the bar, several large waves hit the vessel. An estimated 16-foot-high wave/breaker struck the vessel, rolling it onto its side. The two crewmembers were thrown overboard as it rolled. The Captain exited through a broken window, entering the sea from the bow. The vessel flooded in part from the open aft cabin door, turned into the waves and quickly sank. The USCG arrived 20 minutes after the mayday call was received and found and retrieved the Captain and one crewmember. The second crewmember was found unresponsive and face down in the water approximately 10 minutes later. Resuscitation efforts were started and he was transported to the hospital by boat, then helicopter. At 10 p.m. he was pronounced dead due to trauma and drowning. OR-FACE received the initial notification of the incident from its ongoing surveillance of programmed Google alerts. This investigation report is based on review of the US Coast Guard Marine Information for Safety and law Enforcement (MISLE) Incident Investigation Report (2016) and NIOSH commercial fishing safety research and recommendations. A phone discussion was also completed 10/30/19 with Mr. Greg Merten, USCG Civilian Search And Rescue (SAR) Planner, Columbia River Division. RECOMMENDATIONS: 1) Confirm major vessel damage is thoroughly inspected and repaired prior to sailing, especially prior to sailing in rough seas and inclement weather. 2) Establish and follow maintenance and inspection intervals for critical components 3) Before deciding to fish and venture to sea, Captains must consider weather conditions and vessel suitability. 4) If the decision is made to cross the bar in rough seas the Captain should request and obtain a USCG escort. 5) Secure all potential water in filtration points in rough seas, including doors, windows, and hatchcovers. 6) Captains should work together to request regional safety training and encourage crew members to participate in available safety training. The framer had worked in the construction industry for fifteen years. This was his second day on the job with his employer, a new single-family housing construction contractor. On the day of the incident the framer, a carpenter, and the crew leader were working at a two-story house construction site. They were using wall jacks, 16-foot long 2x4 poles, and self-supporting ladders to raise a sheeted wood frame exterior wall on the upper floor of the house. The wall was approximately 43 feet long by 13 feet high. The workers did not determine the weight of the wall prior to raising it, but after the incident the wall was estimated to weigh at least 1,000 lbs. As they were raising the wall, the framer was working on one end of the wall, the crew leader on the other end, and the carpenter was in the middle. When they had raised it about eight feet, they stopped to place brace poles. Their plan was to secure the wall with braces and then when other workers arrived on site they would push up the wall the rest of the way by hand. As the framer was securing a brace pole under the wall, the pole broke. The wall then fell in sections, with one section striking and crushing him underneath. He suffered multiple fractures and internal injuries. The other workers were not injured. When work started again several days later, seven workers lifted the wall. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: 1. No crosswalk signage AB - 2. Ineffective crosswalk roadway markings that were in poor condition AB - and 3. Assigning crossing guards to unsignalized intersections without the appropriate traffic control measures. Develop, implement, and enforce a policy that prevents employees from working alone in certain situations. 4. Ensure that a job hazard analysis is performed prior to the start of each project and updated if there is a major change in the scope of the project. 5. Develop and implement a comprehensive safety and health program that addresses hazard recognition, avoidance of unsafe conditions, and proper use of equipment. In addition, equipment manufacturers should: 1. Adopt and implement the concept of Prevention through Design (PtD) to identify potential hazards associated with equipment and then eliminate these hazards through design changes. 2. Develop a pictograph of the overhead crushing hazard and set a minimum height clearance for operating the excavator. TY - Report A2 - Michigan State University AB - The Division of Occupational and Environmental Medicine at Michigan State University (MSU) and the Michigan Department of Health and Human Services (MDHHS) operate a surveillance system for monitoring work-related burns in Michigan. This report describes these injuries for the years 2014 through 2016. Key results include: Work-related burns were identified through medical records submitted by hospitals, poison control center reports, Workers' Compensation claims, and the Michigan Fatality Assessment and Control Evaluation (MIFACE) program. There were 5,069 work-related burns including two deaths among Michigan residents. There were 19 individuals who had two or more separate burn incidents. The Bureau of Labor Statistics Survey of Occupational Injuries and Illnesses (BLS SOII) estimated only 2,070 work-related burns occurred from 2014 through 2016. This estimate was 59.1 percent lower than the Michigan-based surveillance system. Almost two out of three burns (64.1 percent) were among male workers and the rate of work-related burns among males was 60 percent higher compared to females. The most common areas of the body affected were wrists and hands (36.4 percent of cases in 2014 and 34.7 percent of all burn injuries in 2015-2016). Roughly half of cases (51.0 percent) were diagnosed with a second-degree burn, with 22.4 percent experiencing a first-degree burn, 4.4 percent experiencing a third-degree burn, and 22.3 percent of cases with no specified degree of burn. Most (65.8 percent) burns were caused by a thermal exposure. Slightly more than one in five burns (21.0 percent) were caused by a chemical exposure. The remaining 13.1 percent of burns were caused by electrical, radiation, other, or multiple exposures. The accommodation and food services industry accounted for the highest percentage (27.6 percent) of burns, however the mining, quarrying, and oil and gas extraction industry had the highest rate of work-related burns (251.9 burns per 100,000 workers). Workers' Compensation was the expected payer for medical care among 2,674 cases (56.6 percent of cases identified from medical records). The payer was unknown for 18.4 percent of cases identified by medical records. The Michigan Occupational Safety and Health Administration (MIOSHA) program completed inspections at 61 worksites identified by the surveillance system. MIOSHA issued 121 violations and assessed $219,880 in fines related to occupational burns from 2014 through 2016. The carpenter set up a scaffold using a 10 feet long by 12 inches wide manufactured aluminum extension plank placed between two step ladders. This allowed the carpenter to access the old siding boards that were to be removed and replace them with new ones. While working on the plank approximately five-and-a half-feet up the ladder, the carpenter pulled off a piece of wood siding that came off easier than anticipated. The extra force exerted while pulling the board caused the carpenter to lose balance and stumble backwards and fall off the plank, landing on the ground below. The carpenter sustained soft tissue injuries to the neck and back. After the incident, the employer used an aerial lift for the remainder of the job. The valve stem was not removed from the tube within the tire to release the air pressure. The CA/FACE investigator determined that, in order to prevent similar future incidents, employers with workers who change multi-piece tires should: 1. Implement policies and procedures as part of a safety program to ensure that tires are deflated prior to wheel disassembly. 2. Remove all pneumatic tires and replace with solid rubber tires. 3. Consider providing employees with certified training in commercial tire service (CTS), or hire companies with CTS technicians. Department of Labor and Industries AB - A 31-year-old laborer was severely injured when he fell 24 feet from a roof deck, landing on concrete. He was using a personal fall arrest system, but he had not adjusted the rope grab and there was too much slack in his lifeline. The laborer was employed by a framing contractor. He had worked for the employer for two years. On the day of the incident, the laborer was working on the construction of a new school gymnasium. Shortly after 7 a.m. on a cold winter morning, he went on to the partially sheathed plywood roof deck to prepare it for more sheathing installation. The roof deck was covered with frost. His first task was to use a propane burner torch to melt the frost and dry the deck so that workers could safely work without slipping on the 8/12 pitch roof. He was wearing a full-body harness with a lanyard and a rope grab attached to a lifeline that was tied-off to a roof anchor point. Using the torch, he worked his way up the roof. As he did this, he slipped on frost and slid about 20 feet down the roof. He attempted to arrest his fall by grabbing a roof truss but was unable to. Believing the rope grab on his lifeline would arrest his fall, he did not attempt to hold on to the rope. He slid over the roof's leading edge and fell 24 feet, landing on a concrete floor. He suffered fractures to his thigh, heel, and wrists. Investigators found that the laborer had not adjusted his rope grab short enough on the lifeline to arrest his fall. After the incident, the rope grab resting on the floor had two feet of slack between it and the vertical section of lifeline. Department of Public Health AB - On November 3, 2017, a 19-year-old "gig economy" bicycle food delivery worker died when he was struck by a dump truck. At the time of the incident, he was logged onto a food delivery, application-based platform via his cell phone, which was in his pocket. He was riding his bicycle along a roadway when he was struck. Once the dump truck operator realized his truck had struck a person, a call was placed for emergency medical services (EMS). EMS arrived quickly and the victim was pronounced at the incident location. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. No side guards on the truck; 2. Vehicle blind spots; 3. No company provided safety and health protections for the victim, including no personal protective equipment (PPE), because working as an independent contractor; and, 4. No dedicated bicycle lanes. The Massachusetts FACE Program concluded that, to help prevent similar occurrences, employers and other owners of motor vehicles with gross vehicle weight ratings over 10,000 pounds should: 1. Consider adding side guards to these vehicles. Employers, including employers who utilize workers with nontraditional work arrangements should: 1. Ensure that all workers who will use bicycles along roadways to perform tasks are wearing the appropriate personal protective equipment (PPE) for the task and should consider providing daytime running lights for the bicycles; and, 2. Ensure that all workers who will be in proximity to roadways to perform tasks are trained about the concept of vehicle blind spots. Employers who utilize independent contractors should: 1. Comply with the Massachusetts Independent Contractor Law to ensure employees are not misclassified as independent contractors; and, 2. Municipalities should consider adopting a complete streets approach to ensure the safety of pedestrians and bicyclists.
