Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

According to the U.S. Bureau of Labor Statistics, Census of Fatal Occupational Injuries, nationally in 2018, "unintentional overdoses due to nonmedical use of drugs or alcohol while at work increased 12 percent from 272 to 305." As a result, the driver of the dump truck lost control of the vehicle, collided with a second passenger car and subsequently rolled into the median, spilling the load of gravel onto the interstate in both the east and westbound lanes. The driver of the dump truck was extricated by emergency responders and transported by ambulance to a local hospital. After arriving at the local hospital, the driver's condition deteriorated, and he was flown to a major university hospital located out of state. The victim succumbed to his injuries at 10:30 PM later that same night. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1) Failure to perform road test. 2) No formal driver training program. 3) No defensive driver training. RECOMMENDATIONS - Kentucky FACE investigator concluded that, to help prevent similar occurrences, employers should: 1) Commercial carriers should require drivers to complete defensive driver training, prior to driving company-owned vehicles. 2) Commercial carriers should implement a road-testing program for all new incoming commercial drivers to verify their driving skillset. 3) Commercial carriers should implement formalized driver training program for all new, incoming drivers that includes a performance evaluation period. The ironworker and his journeyman coworker, assisted by an apprentice, were constructing a mechanical enclosure windwall structure to house a rooftop HVAC unit. Inside the windwall was a wooden form for a concrete stem wall to support an HVAC unit. They were installing windwall sections. The ironworker was welding angle brackets to support these sections while working from an extension ladder leaning against the inside of the windwall. His coworker asked him to get a clamp to support a bracket prior to it being welded. The ironworker then stepped sideways from the ladder onto the top of the wooden form and then jumped down from the 32-inch-high form onto a sheet of plywood covering an HVAC ducting hole. He landed with both feet on the cover, which had the word "HOLE" painted on it. The cover failed and he fell through the hole, landing on concrete 42 feet below. Investigators determined that the general contractor had installed the cover. The 32-inch by 62-inch hole was covered by a 5/8-inch thick piece of 10-year-old worn plywood. The plywood was 48 inches by 60 inches and was secured on one side with two nails to the stem wall sill. This placement left a gap of one inch on each end and only two sides of the plywood supported. When the ironworker landed on the plywood, it buckled in the center. REQUIREMENTS: 1. Hole covers must be capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. 2. Holes must be guarded by one of the following fall restraint systems: 1) a standard guardrail or equivalent, 2) a cover, 3) a warning line system. 3. It is the responsibility of management to establish, supervise, and enforce, in a manner which is effective in practice: Training programs to improve the skill and competency of employees. In this instance: a. Recognize the hazard of falling into or through holes. b. How to select, place, and secure hole covers. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: 1. Subcontractors should check hole covers to verify that the covers are adequate in strength and placement and report potential problems to the general contractor. 2. Employers on multi-employer sites should use contract language that clearly defines the safety responsibilities of each contractor prior to initiation of work. The laborer's employer was a general engineering contractor that was working on a light rail project. On the day of the incident, the laborer was the lead worker. He and two other workers were disassembling steel tower frames of a temporary shoring system for forms that had been used to construct concrete pillars to support an elevated light rail line. As the crew was working, they experienced a problem that prevented easy disassembly of two coupled steel frames. The two frames could not be separated due to the binding of steel pins used to connect the shore tower sections. A member of the crew had been using a forklift to hold the frames upright. The laborer instructed him to move the forklift away from the frames. The laborer then attempted to free the pins by using a sledgehammer to knock the pins out. After he hammered out the last pin, the two coupled 690-pound frame sections fell on him as he stepped into their path. He was hospitalized with head injuries. He died four months later. Investigators found that although the laborer had been trained by his employer, was provided with the shore tower manufacturer's instructions, and had access to company engineers and other knowledgeable employees for consultation on how to proceed safely, he chose to use an unsafe method to take apart the tower sections. Safe methods of disassembling the shore tower parts would include the use of a forklift or crane to stabilize the shore tower parts while a worker removes the pins connecting the frames. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: Workers must follow manufacturer's instructions for disassembly of shoring systems, and any supplemental instructions provided by the employer; such as a hazard assessment or on-the-job instructions, when specially prepared by the employer's mechanical and safety engineers. This report describes these injuries for 2017. Key results include: Work-related burns were identified through medical records submitted by hospitals, poison control center reports, Workers' Compensation claims, and the Michigan Fatality Assessment and Control Evaluation (MIFACE) program. There were 1,831 work-related burns, including one death, among workers in Michigan, representing a 2.1 percent increase from the previous year. The Bureau of Labor Statistics Survey of Occupational Injuries and Illnesses (BLS SOII) estimated that only 630 work-related burns occurred in 2017. The BLS estimate was 65.6 percent lower than the Michigan-based surveillance system. Almost two out of three burns (64.6 percent) were among male workers and the rate of work-related burns among males was 59.2 percent higher than the rate among females. The most common areas of the body affected were wrists and hands (33.9 percent of all burn injuries in 2017). Most (71.4 percent) burns were caused by a thermal exposure. Slightly more than one in five burns (23.5 percent) was caused by a chemical exposure. The remaining 5.1 percent of burns were caused by electrical, radiation, other, or multiple exposures. The accommodation and food services industry accounted for the highest percentage (31.2 percent) of work-related burns and the highest rate of work-related burns (117.3 burns per 100,000 workers). For work-related burns identified through medical records, Workers' Compensation was the expected payer for medical care in 938 cases. There were an additional 28 work-related burns that received Workers' Compensation wage replacement for lost worktime, but which had another type of health care coverage listed as the expected payer. The Michigan Occupational Safety and Health Administration (MIOSHA) program completed inspections at 11 worksites identified by the surveillance system. MIOSHA issued 24 violations and assessed $25,100 in fines related to occupational burns in 2017. According to the Centers for Disease Control and Prevention (CDC), male construction workers die by suicide at a significantly higher rate (45.3 per 100,000 persons) compared with workers in all industries combined (27.4 per 100,000 persons). For the construction industry, mental health awareness and suicide prevention are just as important as preventing injuries. Construction company owners, managers, supervisors, trade associations, unions, and workers can take steps to save lives by addressing suicide prevention in the industry. RECOMMENDATIONS: According to the Construction Industry Alliance for Suicide Prevention, the construction industry can take steps to address suicide prevention as a health and safety priority by, creating safe cultures, providing training to help those at risk, raising awareness about the suicide crisis in construction, normalizing conversations around suicide and mental health, ultimately decreasing the risks associated with suicide in construction. There were 159 work-related deaths in 2018, an increase of 6 deaths compared to 2017. There were 154 separate incidents (two deaths occurred in each of five separate incidents) and 157 separate employers. A narrative summary of each work-related fatality is in Appendix I. MIFACE educational material, including on-site Investigation Reports, Summaries of MIOSHA Investigations, MIFACE Data Fact Sheet, and Hazard Alerts can be accessed on the Michigan State University Division of Occupational & Environmental Medicine (MSU OEM) website, Work Related Injuries and Illnesses, Work Related Fatalities (MIFACE) webpage. Key findings for 2018: The number of work-related deaths (159) and the fatal injury rate (3.4 deaths/100,000 workers) were up compared to 2017 (153 work-related fatalities, 3.3/100,000 workers). Although there has been slight variation between the years, there has been no appreciable change in the number of deaths or rates since 2010. The overall rate of work-related deaths in Michigan is lower than the rate in the United States (3.5 deaths/100.000 workers). The industry sector with the highest employment-based industry rate was Mining (36.6 deaths/100,000 workers), followed by Agriculture (26.9 deaths/100,000 workers) and then Utilities (19.7/100,000 workers). Construction had the largest number of work-related deaths (29 deaths, 19.0% of all fatalities) followed by Agriculture, Forestry, Fishing and Hunting (23 deaths, 14.5%). Struck-by incidents were the leading cause of a work-related death (36, 22.6%). Motor vehicle crashes were the second leading causes of death (24, 15.1%) followed by homicides (22, 13.8%) and falls (21, 13.2%). Individuals who died were most likely to be men (91.2%) and Caucasian (88.0%). The average age was 49.2 years old and ranged from 20 to 89 years of age. Illegal drugs, alcohol or side effects of prescribed and over-the-counter medication were potential factors in 17.2% of the non-suicide and non-drug abuse deaths. By occupational group, Management had the largest number of work-related deaths (46) followed by Transportation & Material Moving (26) and then Construction & Extraction (18). Forty-nine of Michigan's 83 (59.0%) counties had a work-related fatality. Wayne County had the highest number of deaths (29, 18.2%), followed by Oakland (13, 8.2%), and Macomb and Washtenaw (10 each, 6.3%). Of the 159 work-related fatalities, 38 (23.9%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. Department of Labor and Industries AB - A 39-year-old framer died when he fell 20 feet through a stairwell opening in the second floor deck of a building under construction. The framer's employer was a framing contractor that was acting as a subcontractor responsible for framing a multi-story residential/office building. On the day of the incident, the framer was part of a crew of three workers doing various tasks on the building's recently completed second floor deck. A stairwell opening, fully decked with a plywood cover, was located near where the crew was working. By about 9:30 a.m., a subcontractor's workers had erected guardrails in preparation for removal of the decking. By shortly after 11 a.m., the subcontractor had removed decking from the stairwell opening. At noon, the framer and his coworkers broke for lunch. They gathered a few yards from the stairwell opening. No one observed what happened next, but a time-lapse security camera captured the movements of the framer. He walked toward the stairwell, climbed through the guardrail, and stood on the opening's inner concrete ledge. He then fell into the stairwell opening, landing on concrete 20 feet below. First responders took him to a hospital where he later died. Investigators do not know why the framer entered the area behind the guardrails. He had no work to do in this area. Investigators speculated that he went there to sit on the ledge and eat his lunch. It is possible that he had been in this area earlier in the day when it was safe to be there and he had not realized that workers had removed the decking cover. First responders found a bag containing a lunch near his body. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: 1. This tragic fatality could have been prevented if the worker had not gone on the other side of the safety guardrail system without the use of fall protection. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: 1) A schedule delay due to design change, and schedule pressure. 2) Lack of task checklists for key testing and inspection processes. 3) Deviation from planned procedures. 4) Inadequate safety communication. 5) Inadequate training. RECOMMENDATIONS: To help prevent similar occurrences, employers should: 1) Employers should ensure employees do not work on pressurized water systems. 2) Pipe systems should be pressure-tested and inspected before pipes are backfilled. Use checklists during tasks and the inspection process to reduce risk of overlooking critical steps that could endanger workers. 3) Prime contractors should establish a realistic project schedule in collaboration with subcontractors. 4) Prime contractors should develop and follow a communication plan so that subcontractors are made aware of project design modifications in a timely manner and to work out how design changes will impact project schedule, so as not to compromise workplace safety. 5) Employers should provide written procedures to employees prior to performing the work, and ensure employees receive adequate training so they understand the hazards and how to execute procedures safely. The decedent had landed on his head and was face down on the concrete floor. The coworker called for emergency response. The decedent was on his hands and knees, rocking back and forth when emergency response arrived. The decedent was transported to a nearby hospital where he succumbed to injury complications several days later. CONTRIBUTING FACTORS - Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in the injury or fatality. The following hazards were identified as key contributing factors in this incident: 1. Working from ladder rather than an appropriate work platform. 2. Improper ladder use and selection. 3. Appropriate personal protective equipment not utilized. 4. Did not perform a job hazard analysis. RECOMMENDATIONS/DISCUSSION - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Employers should try to minimize the use of ladders as a work platform. 2. Ensure proper ladder selection and safe ladder use. 3. Always use appropriate personal protective equipment. 4. Construction employers should develop and implement an accident prevention program. The truck driver's employer was a hay farm that had two separate business entities, farming and hay hauling. She worked as a semi-truck driver hauling hay in intermodal freight containers to a port. On the day of the incident, the driver brought the truck to the loading dock of her employer's hay barn to be loaded with hay bales. The employer required drivers to keep the containers clean, so she entered the container to remove debris. The workers had an informal practice of placing a cone or trashcan at the entrance of a container to indicate that an employee was inside. The driver did not do this, and she did not tell anyone that she was about to enter the container. Shortly after she went into the 40-foot container, a forklift operator entered the container with a load of hay bales. The forklift's hay clamp attachment held four double-compressed alfalfa sleeve bales stacked two high, each weighing nearly 1,100 lbs. Due to the size and height of the bales, the forklift operator could not see over or around them. The driver was in the far end of the container, and was unable to alert the operator due to forklift noise. The bales were stacked so that there was no room for the driver to escape. The operator continued to load the container with bales until it was full. When workers realized that the driver was missing, they reviewed video footage that indicated the driver was still in the container. They called EMS personnel and removed the bales from the container. The driver's body was found at the far end. She had been crushed against the container's front wall by hay bales. Investigators found that the employer's accident prevention program covered only the farming part of their business operations and did not address loading dock safety or trucking hazards. After the incident, the employer required that drivers not be allowed on the loading dock; installed lighting on the dock that shines into the container; and instructed forklift drivers to no longer stack bales two high while in transit, allowing them to see in front of their load. REQUIREMENTS: 1. Employers must ensure that their accident prevention program (APP) is tailored to the particular needs of their workplace and or operation and the types of hazards involved. See WAC 296-800-14005. 2. Employers must make sure that forklift operators do not drive up to anyone in front of a fixed object. See WAC 296-863-40005(6). 3. Employers must make sure that forklift operators look in the direction they are going and keep a clear view of the path of travel. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: Employers should conduct a job hazard analysis with the participation of workers to assess the hazards to all workers associated with truck freight loading and unloading operations at loading docks. Based on this assessment, employers should establish and enforce safety policies, hazard control procedures, and training for truck drivers and loading dock workers. At a nearby intersection, the southbound roadway widened to two lanes and the northbound two-lane wide roadway narrowed to one lane. The posted speed limit was 50 mph. There had been a water tap break on the west side of the roadway. The Department of Public Works (DPW) foreman instructed two workers to place temporary traffic control signs ("Work Zone Ahead" and "Work Zone Begins") on each side of the roadway. Channelizing devices were placed on the east and west fog lines of the roadway in the work zone AB - both the northbound and southbound travel lanes were open to active traffic. A DPW subcontractor dug an excavation on the west side of the roadway and then dug a second excavation on the east side of the roadway. A directional boring machine was set up on the west side. The plan was to bore under the roadway, west to east, pull the new water pipe through and then make the connection. While the boring took place, the location and depth of the bore were monitored. The decedent and a coworker, who worked for a company that developed, manufactured and marketed instruments for underground locating were at the site to demonstrate their new locating equipment. The decedent and his coworker placed sensors on both sides of the road to assist with locating the boring head. Throughout the boring operation, the decedent had walked back and forth across the open traffic lanes, assessing the depth and location of the bore head, demonstrating the new underground locating equipment. Checking the bore depth one last time, the decedent took an older piece of equipment into the northbound travel lane. He placed the equipment on the roadway and bent over facing west to read the results. A vehicle travelling in the open northbound lane at approximately 55 mph neared the work zone. A worker ran toward the vehicle yelling and waving his hands and hard hat. The vehicle struck the decedent, propelling him in the air. He landed on the asphalt. Emergency response arrived and transported the decedent to a nearby hospital. He died several days later from the injuries sustained at the time of the crash. The decedent was wearing a Class 2 high-visibility vest at the time of the incident. CONTRIBUTING FACTORS - Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in the injury or fatality. The following hazards were identified as key contributing factors in this incident:: 1. Lack of hazard recognition. 2. Temporary traffic control not appropriate for roadway speed limit, traffic volume, and work being performed. 3. Worker in active roadway without a dedicated spotter or flagger. RECOMMENDATIONS - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Employers and employees should ensure that work zones and traffic control plans are properly set up. 2. Employers should provide job hazard analysis training to employees that includes a jobsite survey and hazard assessment to identify all potential hazards including those associated with working near an active roadway. 3. Institute a hazard awareness program to educate employees about the potential and specific hazards noted during the jobsite survey and how to minimize exposure to these hazards. Occupational and Environmental Medicine AB - In summer 2018, a male farmer in his 70s was driving a farm-owned pickup truck (Pickup #2) southbound on a dry, blacktop chip-sealed asphalt two-lane road with a speed limit of 55 mph when his vehicle was hit in an intersection by a westbound pickup truck (Pickup #1). The east-west road was a similarly constructed two-lane, 55 mph roadway. The north-south road had the right of way. Stop signs were positioned at the intersection on east-west roadway AB - the stop signs had affixed signage indicating crossroad traffic does not stop. The east-west road also had advance warning stop signs with the name of the north-south road affixed below. The westbound pickup truck (Pickup #1) entered the intersection without stopping and struck the decedent's pickup truck (Pickup #2) on the driver's side. Both vehicles came to rest in a nearby farm field. The decedent was wearing his seat belt/shoulder harness. It was unclear to emergency responders if Pickup #1's driver was wearing his seat belt/shoulder harness. Emergency response was summoned by passing motorists. The decedent was declared dead at the scene. Contributing Factors - Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in the injury or fatality. The following hazards were identified as key contributing factors in this incident: 1. Driver not following traffic rules striking decedent's pickup truck. 2. Driver ability to see oncoming traffic at intersection obscured by corn. Recommendations - 1. Vehicle drivers need to comply with traffic rules and warnings. 2, State and local road authorities should periodically evaluate rural roadway traffic data to determine if existing road signage is appropriate. 3. Farmers and rural county road commissions should discuss methods to allow for sight lines for oncoming traffic at roadway intersections. 4. Employers should establish a Safe Driving program which includes communicating distracted driving principles/restrictions and reviewing defensive driving practices with all vehicle drivers. The tree was balancing on one of the "Y" branches and the stump/exposed tree root (flare). The decedent was working alone. He was using his chain saw to remove the larger limbs from one of the "Y" branches. During this activity, it appears the tree shifted and rolled. The tree trunk struck and pinned him against the ground. When he did not return home as planned, his wife called him on his cell phone. She looked for him, and when unsuccessful, called her son. Her son arrived at the incident site and, using a chain saw, cut away a section of tree lying on the decedent. Emergency response was summoned. He was declared dead at the scene. CONTRIBUTING FACTORS: 1) Inadequate hazard assessment of the hazards posed by the felled tree AB - a) Forest floor soil characteristics react differently than field hard packed soils AB - b) Forest edge trees react differently than in-forest trees. 2) Safe limbing and bucking work practices not followed. 3) Working alone. RECOMMENDATIONS: 1) When felling and limbing trees, recognize differences in soil characteristics between "forest" trees and "fence line" trees. Use safe work practices when performing limbing or bucking activities with a chainsaw including performing a hazard assessment, standing uphill when possible, not working alone and wearing appropriate personal protective equipment. 2) When working alone, establish a check-in procedure with another individual to help assure prompt emergency assistance. 3) Do not remove a crushing weight from an individual prior to contacting emergency response and receiving their instructions. The decedent was wearing a Type R (Class II) high visibility safety vest and using a hand-held stop sign with flashing lights. She also had a walkie-talkie and whistle. As she walked out into the crosswalk to stop motorists traveling east/west, she was struck by an eastbound minivan whose driver stated that due to sun glare, did not see her and did not apply the vehicle brakes. The decedent struck the hood and front passenger-side windshield of the minivan. Emergency response was called, and she was transferred to a local hospital where she died approximately one week later. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. No advance warning of school zone in speed zone of 35 mph. 2. Possibly not properly trained. 3. Sun glare on dirt/haze covered windshield of vehicle striking decedent. RECOMMENDATIONS/DISCUSSION - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Evaluate school crossings at least once every 5 years to determine if current crossing practices are the best option to ensure safety of both students and, if utilized, school crossing guards. 2. Municipalities should consider additional measures to increase visibility or slow traffic at intersections during school crossing periods. 3. Set and enforce speed limits of not more than 20 mph in school zones where crossing guards are required for student safety. 4. School districts utilizing crossing guards should ensure that the guard meets the requirements of the Michigan Vehicle Code and Michigan Manual on Uniform Traffic Control Devices. 5. Municipalities should ensure appropriate school signage is installed at school crosswalks. 6. To minimize the effects of sun glare, vehicle operators should take appropriate precautions, including regular cleaning of both the inside and the outside of the vehicle's windows and windshield.
