Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

When the victim fell to the ground floor inside the building, at least one of the 8-foot by 4-foot trusses fell with him or just behind him, striking his head when he impacted the ground. Co-workers notified the supervisor who immediately called 911. Co-workers and other witnesses went to the victim to assist him. Emergency responders arrived within a few minutes, and on observing the severity of the head injury, transported the victim to a local city hospital where he was pronounced dead. The rotating knives caught the limb and it began feeding through the chipper. As the limb fed quickly into the chipper knives, a branch that projected perpendicularly from the limb struck the victim in the back. As the limb continued to feed into the chipper, it pulled him toward the rotating chipper knives. The victim may have attempted to "push off" from the machine but was unable to free himself from contact with the branch, and the limb pulled him into the chipper. The chipper drum jammed and came to a stop after the victim's torso had been fed into the machine. A coworker who witnessed the incident notified 911. The victim was pronounced dead at the scene. NIOSH investigators concluded that to help prevent similar occurrences employers should: 1) ensure that supervision beyond that normally provided for experienced adult workers is available when youths are present on job sites AB - 2) ensure that workers involved in tree trimming operations are trained to recognize and avoid the hazard of being fed through an operating wood chipper AB - 3) prohibit workers less than 18 years old from operating or tending wood chippers AB - 4) consider using self-feeding wood chippers that incorporate devices to quickly shut off or reverse material feed. Prior to the fatal incident, he had left the bulldozer unattended on level ground. The unattended bulldozer was running at full throttle and both the transmission neutral safety lock and the parking brake had not been engaged. The dozer's blade was also left partially elevated. As the dump truck driver approached with another load, he noticed the unattended bulldozer. The truck driver then made his usual U-turn before backing up to position himself for dumping. Although there were no witnesses, it is believed that during this period, the victim began to climb up on the bulldozer on the same side as the transmission selector lever. Due to a trip, stumble, loss of balance or inadvertent hand placement, it is believed he bumped the transmission into 2nd speed reverse. It is also believed that he was either standing on the bulldozer's left track or was thrown onto the track by the sudden jerk of the transmission engagement. He then rode the track backwards until falling off. The bulldozer's left track ran over the victim's chest cavity. As the truck driver began backing up, he searched his mirrors for the unattended dozer and could not locate it. Opening his door and turning his body for better visibility, he spotted the victim who had been run over approximately 78 feet from the bulldozer's original position. He then saw the bulldozer, which had continued another 25 feet and was beginning to bury itself after having gone over an embankment. Realizing the severity of the injury, the truck driver immediately drove to another part of the site for help. After EMS was contacted, the site's supervisor returned, checked the victim, and then shut down the unattended bulldozer. EMS arrived within minutes. The victim was pronounced dead at the scene. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure that machines are not left unattended unless all precautions necessary to prevent motion have been taken. 2. Develop, implement, and enforce a written safety program which includes, but is not limited to, task and equipment specific safety procedures, work rules, and worker training in hazard identification, avoidance, and control. 3. Designate a competent person to conduct frequent and regular site safety inspections. She had accepted and was riding on the right side of the forklift, holding onto the overhead guard. They had traveled about 135 feet when the victim turned sharply to the left, and the forklift tipped and overturned to the right. The coworker jumped from the machine while the victim was thrown or jumped from the operator's seat. The victim's head was caught between the overhead guard and the ground while the coworker's ankle was pinned. Other workers heard screams and went to the forklift. They raised the machine to free the coworker and pulled the victim from under the overhead guard. Emergency personnel were notified and arrived within 15 minutes. The victim was transported to a local emergency room where he was pronounced dead. The coworker was transported to a local emergency room where she was treated for minor ankle injuries and released. Road traffic was diverted with a lane shift, so the activity area was free of non-construction related vehicular traffic. The employer was laying a new road at the site of a new freeway overpass. The dump truck driver was self-employed, and was subcontracted by the road construction company to transport and dump loads of gravel on the road construction site. Just prior to the incident, the truck had dumped a load of gravel in the area being prepared for paving. A grader operator was in a grader in line with the path of the gravel truck, waiting for the gravel to be dumped in front of him so he could spread the load. After dumping the load, the truck operator drove forward, lowered the dump box, and began to back up As the truck began to back up, the truck driver steered the vehicle to the left to avoid backing into the grader behind him. The truck's back-up lights and alarms were working, and the driver was watching the mirrors on both sides of the truck. At the time of the incident, the victim was pounding stakes into the ground adjacent to where the truck had been laying the gravel. The changing path of the dump truck brought the victim into the truck's path. The victim, wearing a traffic safety vest, was crouching down with his back to the easterly direction from which the truck was backing, moving west as he pounded stakes. He stepped into the lane where the truck was backing, apparently unaware of the truck's presence. The grader operator glanced to the side, saw the truck as it struck and began to pass over the victim, and tried to alert the truck driver by radio. The signal didn't go through to the driver's radio. At about the same time, the truck driver turned to look forward through windshield and saw the victim's body lying in the gravel in front of the truck. He jumped out of the truck and yelled to other workers in the area to call for emergency services. Although the incident occurred in a metropolitan area, it took extra time for the emergency vehicles to locate the scene and the victim due to traffic changes caused by the construction. The medical examiner pronounced the victim dead at the scene. The FACE investigator concluded that to help prevent similar occurrences, road builders should: 1. Develop an internal traffic control plan (ITCP) that project managers can use to coordinate the flow of construction vehicles, equipment, and workers operating in close proximity within the activity area, especially on large and multi-contractor jobs. 2. Design the workflow to minimize backing heavy equipment. 3. Ensure that a person is designated as a spotter to direct trucks that must back up within highway construction sites. 4. Consider equipping vehicles with devices to detect the presence of individuals or objects behind backing vehicles. In addition, EMS providers should: 5. The victim had been working with his son and grandson (co-workers) on the family farm, using a skid-steer loader to haul rocks from a cornfield to the rock pile in a wooded area at the edge of the field. After the last load of rocks was loaded into the bucket of the loader, the co-workers left the field to deliver a truck to another field. They planned to meet the victim at the farmyard after he dumped the load of rocks. The victim drove the loader to the rock pile, and exited the loader for an unknown reason with the bucket raised. It was his habit to exit and enter the loader this way, because knee and hip pain made it difficult for him to climb over the bucket. While returning from the second field, the co-workers noticed the loader was still at the rock pile, with blue smoke rising in the area. They went to the loader and saw the victim on the ground in front of the loader, pinned between the bucket and frame. The loader engine was still running, so they turned it off and checked the victim for signs of life. Although he appeared lifeless, they left the bucket in place to avoid causing additional injuries. Then they drove to the farmyard to call for emergency services. They immediately returned to the scene and directed EMS responders and sheriffs to the victim's location. An EMS responder started the loader engine and raised the bucket so the victim could be removed. EMS workers called for air ambulance services, which arrived within twenty minutes. The victim was pronounced dead at the scene by a physician from the air ambulance. The FACE investigator concluded that, to prevent similar occurrences, farmers should: 1. Enter or exit a skid-steer loader only when the bucket or other attachment is on the ground, or when lift-arm supports are in place. 2. Seek and use the services of organizations and agencies that provide technical assistance and/or adaptive equipment to agricultural workers with physical impairments. 3. Carry personal communication devices when working in remote worksites. In addition, farm family members should: 4. Develop an action plan for first response to emergencies. Develop, implement, and enforce a comprehensive safety program that includes training for all employees in hazard recognition and in the proper use of equipment. The truck initially skidded straight towards the victim, but at the last few seconds started to jackknife. The victim attempted to escape by running across the northbound lane, but was caught between the gravel truck and the tar equipment. The victim was pronounced dead at the scene. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences: 1. Employers should ensure that adequate warning signs are placed prior to the construction site. 2. When working in the middle of a highway while allowing traffic on both sides, flaggers should be used at both ends of the construction zone. 3. Employers should consider lowering the work site speed limit below that of the regular construction zone. 4. Employers may consider, where the work site is constantly changing, to have a vehicle following the workers with directional arrows, flashing lights and/or beacons to alert oncoming traffic and serve as a personnel barrier. The decedent was changing the fluorescent light bulbs in the sign. To accomplish this task, the decedent used a truck-mounted hydraulic/electric crane workbasket with joystick type controls mounted on the yoke of the basket. After the decedent positioned the workbasket at the base of the sign, he swung the sign face up from the bottom over his head to gain access to the fluorescent tubes. As the decedent pushed the sign face upward over his head trying to wedge a piece of PVC pipe in place, he inadvertently forced the panel against the workbasket controls, activating them, forcing the workbasket into the sign. As the workbasket came forward into the sign, it jammed the sign face into the controls, preventing the controls from returning to a neutral position. The decedent's co-worker, working on the opposite side of the sign, tried to rescue the decedent by lifting the panel, activating the main controls, and cutting away the sign face, but was unsuccessful. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Prepare a written job hazard analysis (JHA) for job assignments that have multiple ways to complete the job, to determine the safest procedures to follow. 2. Protect workbasket controls against unexpected activation 3. Develop rescue procedures that show how to override the auxiliary controls when someone is in the workbasket. As he stood in the engine compartment, the basket slowly descended, due to a hydraulic leak, and crushed the victim. When the owner arrived, he did not immediately see the victim. The cotton stripper's engine was still running. As he was about to leave, he decided to take one more look. He then saw the victim's leg extending between frame members surrounding the engine compartment. After raising the basket, he called Emergency Medical Services (EMS). The Texas FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure workers never enter the top of the engine compartment unless the basket is raised over center. 2. Ensure machinery is checked periodically for leaks or other mechanical defects. 3. Additionally, the manufacturer should include cautions in the operator's manual warning operators not to enter the engine compartment unless the basket is in the full over-center position. He died of a severe internal infection from a torn/ruptured colon 15 days later. The Texas FACE program officer concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure employees who are guiding trucks are on the ground and always visible to the driver. 2. Ensure employees do not climb onto moving trailers. 3. Redesign the driveway to eliminate the need for trucks to back up. 4. Install grooved and lowered tire paths for ease of tire alignment. 5. Install an electric eye mechanism onto the seed dispenser frame which provides oral and visual position warnings for truck drivers. 6. Provide regular training for seasonal workers on safety standards and procedures. The operator told the police investigator that when he released all controls, the bucket swerved to the left striking the victim on his left side and pinning him against the trench wall. The operator called EMS at 11:26AM, who arrived at the scene in 3 minutes. The victim had no pulse or respiration. EMS administered CPR. Ambulance personnel removed the victim from the hole and transported him to a hospital where he was pronounced dead of blunt force injuries to the chest. The manner of death was determined to be accidental. Hospital personnel told a police officer that death was caused by a ruptured myocardium (heart). The Texas FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure that backhoe operators shut down backhoes according to the manufacturer's directions and direct workers to remain an adequate distance from operating backhoes. 2. Designate a competent person to supervise trench activities. 3. Ensure that workers are protected from cave-ins by an adequate protective system. The vehicle apparently struck a small hay bale that was partially hidden in the grass on the uphill side, and rolled over sideways down the hill. The victim either jumped or was tossed from the vehicle as it overturned, and was pinned underneath. ATV's, including this model, are not equipped with ROPS or a seatbelt. When the victim did not return to the farmhouse for dinner as expected, his family searched the farm area but did not locate him. He frequently helped neighbor farmers with their work, so the family assumed he might be involved in a task off the farm and stopped searching for him. When he didn't return the next morning, they resumed their search and found him beneath the vehicle. Emergency services were summoned, and were onsite within six minutes. The coroner pronounced the victim dead at the scene. DNR conservation wardens were also summoned to determine if state all-terrain-vehicle (ATV) requirements applied to the incident. The FACE investigator concluded that, to prevent similar occurrences, farmers and other employers who use off-road utility vehicles should: 1. Select a non-ATV utility vehicle with ROPS and designed to maintain a low center of gravity. 2. Conduct a thorough evaluation of the terrain to identify hazards in the pathway prior to beginning an operation with an off-road vehicle. The farmer's habit was to place a 4-foot ladder on the tractor body in front of the left rear tire, climb two or three steps with the fuel pump in hand, remove the cap, fill the tank, replace the cap, and climb down the ladder. On the day of the incident, he apparently replaced the cap after adding the fuel, and fell toward the tractor wheel, striking the shift lever and throwing the tractor into a forward gear. As the tractor began to roll forward, the victim and the ladder rolled up and over the left fender and fell upside down. The victim was caught between the left rear wheel and under a drawbar attachment, while the tractor rolled forward about 6 feet until it was stopped when it struck a block wall. The victim was found by his sister-in-law, who went to the farm to investigate when the victim did not answer phone calls for several days. She immediately called for emergency services and notified the victim's neighbor and brother. EMS responders and the sheriff arrived within 15 minutes. The coroner was called, and pronounced the victim dead at the scene. The FACE investigator concluded that to help prevent similar occurrences, farmers should: 1. Shift the tractor into park, set the brakes and turn off the engine before dismounting from the tractor. For tractors without a park position, farmers should shift into neutral, set the brakes and turn off the engine before dismounting. 2. Seek and follow medical advice and take precautions to prevent falls if they have medical conditions that could cause loss of balance. The victim was not wearing the available company-required fall protection equipment, which consisted of a body harness and lanyard. The co-worker who found the victim went to get help and placed a call for emergency assistance. The local police and fire departments arrived and transported the victim to a local hospital. He was then moved to a larger hospital in a nearby city where he was pronounced dead. The MA FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Match new equipment to the tasks before implementing it into daily activities. 2. Develop a standard operating procedure (SOP) for new equipment before introducing the equipment into daily activities. 3. Train all employees in the proper and safe use of equipment and how to complete tasks. In addition, manufacturers of vertical lifts should: 4. Inform purchasers and operators of the potential tipping hazard of narrow lifts. An independent crane company was hired to erect supports for the water tower and lift the tank into its final position. This company erected its portable tower crane adjacent to the new foundations for the water tower, as close as possible. After hoisting the tank and while swinging it into position, with just a few feet to go, the rear crane outrigger facing the water tower slipped between cribbing timbers and sank into the ground. The entire tower crane fell towards the water tower, smashing everything in its path. The victim was sitting on a horizontal strut of the water tower base, approximately 80 feet in the air, preparing to adjust and tighten bracing rods once the tank was in position. Two other workers were injured, another member of the tank crew, who was positioned inside the ladder cage for the water tower, and the operator of the crane, who was sitting inside the control box of the crane 120 feet above the ground. Recommendations based on our investigation are as follows: 1. Crane owners and operators should ensure that cranes are properly set up with the outrigger pads supported by firm stable footing. 2. Crane set-ups should be closely observed during lifting operations to detect instability caused by changing load and ground conditions. 3. Prior to crane operations, construction companies utilizing crane services as well as the crane owners and operators should evaluate the soil bearing capacity at the lift site to ensure that crane equipment and procedures are compatible with site conditions. The horizontal boring process involves two employees that work together as a team. The first operator sets up the machine and inserts the engine block at the beginning of the line. The second operator walks the line at the other end of the machine, cleans and oils the bushings, removes engine blocks as they come off the end of the line, and gauges the bored hole. On the day of the incident, the first operator was called to assist on another machine-line. He told the second operator he was leaving, turned off the machine, and went to assist with the other machine. When he returned some time later, although he did not see the second operator, he turned the machine on to begin cycling more blocks. He looked around again for the second operator and when he did not see him, he walked to the backside of the machine and found the victim lying on the floor. The victim apparently had opened the safety gate to lubricate the machine and clean the bushing, and was crushed between the moving section of the boring machine and the machine frame when the first operator turned on the machine. Facility EMS personnel were notified and CPR started. Registered nurses, physician and EMS employees from the plant responded within minutes. A county deputy sheriff and ambulance personnel also arrived at the scene soon after being notified. The victim was transported to a local hospital where he was pronounced dead upon arrival. Recommendations based on our evaluation are as follows: 1. Employers should ensure that safety gates are locked-out or otherwise secured, while machines are in operation in order to prevent personnel from entering dangerous areas. A safety switch should be installed on the machine that disconnects the machine when the safety gate is open, and acts as a inter-lock gate. 2. Employers should ensure that employees follow safety procedures. A lock-out procedure must be followed during maintenance operations.
