Massachusetts Laborer Crushed in Baler at Paper Recycling Facility

• Description:
  On June 19, 2000, a 22-year-old male laborer (the victim) was fatally injured when he was crushed while inside a horizontal paper baler. The victim entered the hopper through a sliding glass door to clear a jammed paper bale. The operator of the baler left the machine's controls to get a paper roll for the victim. The victim was crouched between the gathering ram and the jammed paper bale preparing for the paper roll when he was crushed by the gathering ram as it cycled in the forward direction. A call was placed for emergency assistance. The victim was pronounced dead at the scene and was transported to a local hospital by funeral home personnel under the direction of a medical examiner. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future employers should: 1. Ensure that machines are operating properly to eliminate potential hazards to employees
  
  2. Develop and enforce a hazardous energy control program for all employees with procedures including specific lockout/tagout procedures for each machine
  
  3. Develop a training program that includes a protocol on how to clear jams and ensure that all workers are trained in the safe operation and the potential hazards of each machine
  
  4. Develop, implement, and enforce a comprehensive health and safety program.
