Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Shortly before the incident, the victim and his coworker placed "WORK AREA AHEAD" signs 1,000 feet in advance of their work area on both the south and north shoulders of the westbound lanes. After placing the signs, they drove west and parked their work truck on the north shoulder across the road and west of the section of guardrail they planned to work on. They walked across the traffic lanes and then turned and walked eastward on the shoulder facing oncoming traffic. They walked approximately 650 feet to an area where they began their work installing reflectors. They walked westward working their way back toward their work truck, stopping every 12.5 feet to install reflectors on the guardrail. Both workers were working from the traffic side of the guardrail when the victim's coworker looked up, saw a car approaching in the left lane headed toward them, and yelled a warning to the victim. The coworker was able to lift one leg over the guardrail and get out of the car's travel path before the car struck the guardrail and then the victim. The impact threw the victim over the guardrail and down a ravine into the 200-foot-wide median separating the eastbound and westbound traffic lanes. The driver involved in the incident did not stop. When stopped by police later that day, he said he had fallen asleep before the impact. The coworker was not injured and ran across the highway to his work truck and radioed his employer for help. The employer called 911. The coworker ran back to help the victim but, finding him unresponsive, went back up to the highway to wait for emergency medical services (EMS) personnel. EMS personnel responded within 20 minutes, checked the victim and determined that he had sustained fatal injuries. They called the coroner who pronounced the victim dead at the scene. NIOSH investigators concluded that, to help prevent similar occurrences, employers should, at a minimum: 1) adopt policies that require workers to work on the median side of the guardrail whenever it is safe to do so AB - 2) periodically monitor and evaluate employee conformance with safe operating procedures and provide retraining as necessary when the procedures are not followed AB - 3) establish a formal process for scheduling highway maintenance tasks, including installing reflectors, that identifies the need for consideration of appropriate crew size, traffic control devices, and safe work practices for the tasks to be performed. Additionally AB - 4) Federal, State and local highway transportation and enforcement agencies should continue their efforts in educating the public regarding work zone safety issues and safe navigation through work zones. The truck continued moving forward at 3 to 5 miles per hour. A coworker, who was driving a tamping roller behind the water truck, saw the victim get off the back of the moving truck after checking the fluid gauge and walk around the truck toward the cab on the passenger side. He then saw the victim stop just before the cab, where he stooped over, leaned under the truck, and was run over. The employer last saw the victim in his driver's side mirror when the victim poked his head around the tank from his position on the ladder attached to the rear of the truck. He yelled to the victim to get down and move to the driver's side to get ready to close an external control under the truck on the driver's side. When the employer stopped the truck approximately 30 to 45 seconds after last seeing the victim, he looked back and saw the coworker waving and the victim lying on the road. The coworker called 911 from his cell phone. Personnel from the county sheriff's department and county coroner's office responded within minutes and determined that the victim had sustained fatal head injuries when he was dragged and run over by the rear wheels of the water truck. The victim was pronounced dead at the scene. The supervisor was the only person allowed to perform Lockout/Tagout on the machine, but he wasn't asked, and he didn't request that the victim let him do so prior to any further work on the machine. The victim did not disconnect any of the sources of energy. In an attempt to correct the second problem, the victim opened the sliding front gate guard, shielding the hydraulic plates, and stepped in between them. When the supervisor approached the machine, he saw where the victim was positioned and told the victim to get out of the machine, turned around and closed the sliding gate guard. The closing of this gate activated the machine and crushed the victim between the plates of the injection mold. The supervisor tried but was unable to stop the machine in time to prevent this incident. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure that all employees follow all manufactures and company safety procedures when operating and performing maintenance on machines. 2. The helper started the truck and placed the truck in reverse gear, ready to assist the victim, who was using the tractor. While attempting to pull the truck out of the mud, the front of the tractor bounced up and down several times, then upset to the rear, landing on the victim. Unable to assist, the driver of the truck ran to the victim's house and called 911. The Fire Department responded, and determining that the victim had no vital signs, contacted the medical examiner. The medical examiner pronounced the victim dead at the scene. Recommendations: Proper equipment and procedures to free and tow a stuck vehicle, such as a 4-wheel drive truck. 1. Tractor owners can retrofit many older tractors with a Rollover Protection System (ROPS) that includes a seat belt. 2. Farm tractor operators should always follow recommended hitching techniques when using a tractor. 3. Only use farm equipment as intended by the manufacturer. Pulling motor vehicles or fixed objects, such as a tree stump or fence post, with a farm tractor is a misapplication of the intended use of an agricultural tractor. The tow truck arrived to remove the vehicle, and the operator (victim) was under the front end of the propane truck, trying to position the forks of the T-bar to connect the axle of the disabled truck. Suddenly, the propane truck slid forward on the ice and crushed the towman against the T-bar (stinger) of the tow truck. The man died instantly from crushing chest injuries. Recommendations based on our investigation are as follows: 1. Disabled vehicles should be secured safely before connecting them to a tow truck. 2. Operators of tow trucks should exercise caution when working between a disabled vehicle and their tow truck. 3. Mechanical solutions should be developed to avoid crushing injuries between a disabled vehicle and a tow truck. It was anchored by a single guyline to an uncut tree on the hillside above the road. At the time of the incident, the yarder was attempting to pull a set of logs up the slope. During two previous attempts or "turns," the logs had become hung-up on a stump. The two victims were standing near the yarder when a third "turn" was started. The logs hung-up again. The tree that was anchoring the yarder uprooted and fell toward the yarder. A co-worker working on the slope below the road (the witness) saw the tree fall toward the yarder. He yelled to a co-worker to radio for help and climbed up the slope to the road where other workers met him. Both victims were found under the uprooted tree. CPR was performed but stopped when there was no detectable response. Alaska State Troopers were notified, and the bodies recovered. The victims were pronounced dead at the scene. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure that all crewmembers are capable of recognizing hazardous conditions and are authorized to stop work so that work procedures can be modified in accordance with safe logging and timber harvesting methods. 2. The excavator had just finished excavating a load of dirt and was turning to the right to unload the dirt from the bucket into the spoil area. Without warning, the side of the excavation wall collapsed, pushing the victim against the diesel tank. 911 was called and emergency personnel arrived. Emergency personnel removed the victim from the excavation hole. The victim was taken by ambulance to a local hospital, and then air-lifted to a regional hospital where he died. Recommendations: 1. Employers should ensure that employees working in excavations are protected from cave-in by an adequate protection system such as shoring, sloping or excavation box designed in accordance with MIOSHA Construction Safety Standard, Part 9, Excavation, Trenching, and Shoring, R408.40941. 2. Employers should ensure that excavations are inspected by a competent person prior to start of work and as needed throughout a shift to look for evidence of any situation that could result in possible cave-in. 3. Employers should design, develop, and implement a comprehensive safety training program. This program should include training workers to recognize and avoid unsafe conditions, and required safe work practices that apply to their work environments. Department of Health & Human Resources AB - On July 30, 2001, a 47-year-old male tree feller (the victim) died of injuries sustained when he was pinned to the ground by the tree he was felling. The victim was felling trees on terrain with a 40% slope. He was in the process of felling a poplar, which was approximately 20 inches in diameter at breast height (DBH) and 100 feet tall. It was located 8 feet down hill from a large and lengthy pile of scrap timber and brush which obstructed his escape path. He made a conventional face notch which measured 50 degrees and whose length was 76% of the tree's diameter. The notch was oriented to direct the tree approximately 30-40 degrees to the right of directly down hill. The chosen fall path was partially obstructed by the top of a 60 foot tall leaning walnut tree with a 18" DBH and whose base was measured to be 37 feet from the tree being felled. After making the face notch the victim began to make a bore cut but jammed the saw while doing so. Unable to remove the saw, he disassembled the saw leaving the bar and chain imbedded. He asked a dozer operator who was working with him to bring over another saw. Continuing with his bore cut he left two small dog eared hinges. After making his final cut, the victim retreated approximately 6 feet along the obstructed escape path. As the falling tree contacted the walnut, it cantilevered into the air and swung towards the victim, striking and pinning him to the ground. Witnessing the incident, the dozer operator responded by starting to cut the tree off of the victim but decided to summon help instead. He drove his pick-up truck approximately two miles to the nearest house. He called 911and asked the homeowner to wait for the ambulance. A volunteer fireman heard the call over his radio and responded to the site before EMS arrived. Approximately 20 minutes passed from the time the incident took place until the fireman and the dozer operator reached the victim. The fireman checked for vital signs and finding none, informed the dozer operator that the victim was dead. Shortly thereafter the EMS and deputy sheriff arrived. The victim was transported to the nearest medical facility where he was pronounced dead. The WV FACE Investigator concluded that, to reduce the likelihood of similar occurrences, employers should: 1. Ensure that tree fellers prepare an escape path and move a safe distance from the base of the tree. 2. Ensure that tree fellers identify or clear an unobstructed fall path and landing zone prior to felling. 3. Develop, implement, and enforce a company policy which prohibits the felling of timber when the development and use of an escape path and clear fall path is not feasible. 4. Ensure that tree fellers utilize proper directional felling techniques. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Use a second person as a spotter when backing heavy equipment with blind spots. 2. Ensure that employees, when not operating equipment, stay out of the work area and in clear view of those who are operating equipment. 3. Consider educating employees on the concept of teamwork in safety as part of the documented safety-meeting program. 4. Consider using additional safety devices for heavy equipment to warn workers of a backing vehicle and to warn drivers when someone is in their blind spot. It is not known exactly how he was injured, but he apparently was using a steel pipe to clean a roller from under the moving conveyor when his head became entangled in the machine. He was found unresponsive under the machine by co-worker. NJDHSS FACE investigators concluded that, in order to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should develop, implement, and enforce a comprehensive safety program in the safe operation and maintenance of machinery. 2. Conveyor belts and other machines should be shut down, de-energized, and locked out before performing maintenance or repairs. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. 4. Machine openings should be marked and guarded to prevent unauthorized entry. The victim had been working on a large precision coating machine and needed to connect the section containing the drive motor mechanism to the roller section. Both sections of the machine were set on metal blocks that leveled the machine and allowed access to a forklift. As the victim stood between the two sections to hold up a wire, a forklift operator positioned the lift under the drive section. This apparently caused one of the blocks to slip from underneath the machine, causing the top of the machine to pitch forward as it dropped to the floor. This forward movement was enough to pin the victim's head between the two machine sections. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should ensure that all loads on forklifts are properly positioned and secured before lifting. 2. Employers should conduct a job hazard analysis of all work tasks with the participation of the workers. 3. A witness reported that as the victim leaned over and grasped the box by its nylon shipping bands, one of the bands broke and the victim fell backwards off the H-frame to the concrete floor. A registered nurse (RN) who was shopping in the store witnessed the incident and rendered cardiopulmonary resuscitation (CPR) to the victim. The store's manager called 911 and emergency rescue personnel and police arrived within minutes. The victim was life-flighted to a nearby trauma center where he died later that day from his injuries. The tractor was equipped with the required rollover protective structure (ROPS) and seat belt. The front of the tractor was equipped with a clamshell type bucket with a capacity of approximately five yards. The bucket was in a raised position when it rolled over. The employer had no written Injury and Illness Prevention Program, written job hazard evaluations, or documented training programs for employees at the time of the incident. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure that employees, when operating equipment, wear the seatbelt that is required with the equipment. 2. Ensure operators are trained and skilled in safe work practices, especially maneuvers that adversely affect the center of gravity and increase the likelihood of a tractor rollover. Occupational and Environmental Medicine AB - On January 28, 2001, a 48-year-old male died of injuries he sustained approximately two weeks earlier when he was clearing icicles from the church roof at the church where he was pastor. The victim stood facing the church building and used a snow shovel to knock off icicles from the west side of the building roof. He began the roof clearing operation by clearing the small icicles from the roof first. After clearing the smaller icicles, he began work on the bigger icicles. Starting from the tip of the icicle, he used the snow shovel to break the icicle into smaller portions. The victim apparently loosened an ice dam at the roof edge during the icicle clearing operations, and the icicle and the associated rooftop ice dam, estimated weight of one-half ton, fell on him. The icicle crushed his foot, and the ice dam hit his shoulder. He was rushed to the hospital, and due to the extent of the injury, his leg was amputated below his knee. The victim was treated and eventually released from the hospital to continue recuperating from the injury at home. Several weeks later, he began to feel ill, and had difficulty breathing. He was rushed to the hospital, where he died a short time later from a blood clot in his lung. Recommendations: 1. Increase truss space ventilation to alleviate accumulation of warm air at the peak of the truss space, subsequent melting of snow that refreezes at the eave edge resulting in an ice dam. 2. Increase truss space insulation to minimize heat loss from the building below. 3. Identify and seal ceiling openings that allow heat to escape from the heated area of the building to the truss space. 4. Use proper tools and removal techniques to remove accumulations of snow on the roof or other methods to minimize ice dam formation. A 20-ton capacity overhead crane was used to move the steel inside the plant. On this day, the victim was using the crane to lift a coil that had been stacked three high. As he lifted the coil, the movement apparently dislodged one or more neighboring coils, causing one 10,000-pound coil to slip forward off the row and pin the victim against a row of coils behind him. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should develop and implement a procedure for the stable storage of steel coils. 2. Employers should ensure that aisle widths meet applicable OSHA standards. 3. Employers should become familiar with available resources on safety standards and safe work practices. On the day of the incident, the victim used a farm tractor equipped with a front-end loader to remove a large rock from a farm field. The front-end loader was equipped with a hydraulically controlled general purpose bucket. The loader's hydraulic control levers did not always operate smoothly but would sometimes stick and cause either the loader to continue to rise or the bucket to continue to tilt until the operator was able to move the levers to their neutral position. The victim drove the tractor toward a rock that was on the surface of an unplanted farm field, lowered the loader to the ground and scooped the rock into the bucket. After the rock was in the bucket of the loader, the victim apparently raised the loader. After raising the loader the hydraulic control lever for either the loader or the bucket apparently stuck. This caused the rock to eventually fall backward out of the bucket and tumble down the loader lift arms. The rock struck the victim and pinned him in the seat of the tractor. He victim was not able to free himself from the rock and was found later in the afternoon by his wife who went to the field to look for him. She returned to the farm house and placed a call to emergency personnel who arrived at the scene shortly after being notified. They removed the rock from the victim and transported him to a local hospital. He was then transported to a major medical facility where he died several hours later. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. all items, such as equipment, machines, construction material, rocks, etc., hauled in loader buckets should be tied or chained securely to the bucket AB - 2. tractors equipped with loaders should also be equipped with an enclosed rollover protective structure or a general purpose cab AB - 3. while in motion, tractors with loaders should be operated with the loader in the lowest possible position AB - and 4. all equipment should be adequately maintained and serviced to keep it in safe and proper operating condition. The victim and coworkers had been preparing the work area, located just beyond a gently curved section of two-lane asphalt-surfaced state highway, for a trench excavation the following day. After installing temporary traffic control signs and marking the pavement for excavation, the crew was preparing to leave the area. The victim was sweeping spillage near the crew's work vehicle, parked on the shoulder near the roadway's northbound lane, when a northbound motorist veered off the roadway and struck the driver's side rear corner of the work vehicle. The motorist continued traveling north, scraping the entire side of the work vehicle and striking the victim, before coming to a stop in a roadside ditch. At the time of the collision, none of the crew members were looking toward the victim, however, they all heard the sound, turned toward the victim, and immediately went to his aid. Emergency personnel were notified via 911 and responded within 5 minutes. However, the victim died within minutes of the collision. NIOSH investigators concluded that, to help prevent similar occurrences, departments of transportation should: 1. consider the use of supplemental traffic control devices in addition to the minimum specified by the Manual of Uniform Traffic Control Devices (MUTCD) during highway construction work AB - and, 2. consider installing rumble strips along the roadway pavement edges to warn motorists when their vehicle is approaching the shoulder.
