Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim was standing on the ground operating the truck-mounted crane to lift a section of an irrigation system into place. He was lifting the end tower of a pivot irrigation circle system onto a dolly so that it could be moved out into the field where it could be attached to the rest of the assembled system. This section was 80 feet longer than previously assembled sections and extended under power lines. It came into contact with a 7620 volt ac power 3 phase system distribution line. The power went through the section, the truck, the operator and into the ground. The victim had one year of experience in the assembly of irrigation systems. He did not belong to a union. At the time of the incident the weather was clear and cold and it was near dusk. He had been working for the company for about a year. The disconnect switch at the circuit panel that fed power to the elevator had not been locked and tagged out. The power came on when a co-worker dropped the electrical circuit box, triggering a relay that started the escalator's movement. There was a mechanical blocking device on the escalator to stop movement during maintenance, but it was not used. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure employees follow company policy and procedures on lockout/tagout. 2. Ensure workers do not move electrical escalator equipment when all or part of someone is inside the escalator mechanism. 3. After using the electric hoist to remove a vibratory plate tamper from a customer's truck, he was seen operating the chain hoist without a load. Several minutes later, when he had not returned to the office, his coworkers found him suspended by the chain around his neck. He was removed from the chain by his coworkers, cardiopulmonary resuscitation (CPR) was initiated, and emergency medical services were called. He was transported by ambulance to the hospital in critical condition and died 6 days later. In recent years (1997-2000), 10 adult workers in Washington State drowned doing agriculture-related work. Children who live in agricultural areas or work on farms are also at especially high risk of drowning if water hazards are present. Here are a few important things to consider to prevent drownings. Know where hazards exist and know safe practices that help prevent drowning. Where are drowning hazards? There are a number of drowning hazards in agriculture. They include: 1. Irrigation canals or ditches. 2. Farm ponds and cisterns. 3. Liquid manure and slurry storage pits, ponds, lagoons or tanks. 4. Water-filled well tailings pits. 5. Wells. 6. Lakes, reservoirs, rivers and streams. How can drownings happen? The victim was using a 24-year-old wide front-end configured tractor. The tractor was equipped with a front-end loader and an aftermarket round bale spear. Just prior to the fatal incident, he elevated the bale and began to transport it to a feeding station. The usual route was a dirt road. This time he cut across the field, traversing a 16 degree grade with his right rear wheel on the downhill side. As the tractor traveled over the irregular surface, the tractor's center of gravity (C.G.) shifted forward and downhill. This shift placed the C.G. outside of the tractor's stability baseline causing the tractor to roll downhill to the right landing on the victim and pinning him to the ground. The tractor did not have a rollover protective structure (ROPS) or a seat belt. The victim's wife, who was waiting inside a pick-up truck behind the barn, noticed the cattle's irregular behavior as they ran from the incident site. She walked around the barn, and saw the tractor upside down on her husband. She drove to a neighbor's house and called 911. The EMS arrived within minutes and found no signs of life. The tractor was removed from the victim and he was pronounced dead. The coroner estimated that the victim had died immediately after tractor impact. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed by tractor owners: 1. Equip all tractors with rollover protective structures and a seat belt. 2. Evaluate the route of transport prior to use for hazards which can compromise a tractor's stability. 3. Use extreme caution and travel at a slower-than-normal rate of speed when transporting elevated loads. The victim was painting a bridge as part of a highway construction project. The scaffold platform was raised to approximately 12 feet. The scaffold was not secured to the forks of the forklift AB - the forklift tines were not in the platform sleeves and the platform was not secured to the back of the forks. The victim was not tied off to the platform. As the victim walked sideways on the work platform, it began to tip to the side, and the victim fell out of the platform onto the packed dirt under the bridge. The platform fell off the forklift, hit the ground, and rolled onto the victim. Emergency personnel were called, and the victim was taken to a local hospital where he died. Recommendations: 1. Employees must be trained in the procedures to safely perform a personnel lift using approved platforms and equipment specifically designed to support a scaffold platform. 2. Prior to each personnel lift with a rough terrain forklift, employers should ensure that a pre-lift meeting occurs to review appropriate requirements and procedures for safe work operation. 3. Employers should provide employees with a 100% fall protection system compatible with the work being performed, instruct employees in the proper use of the system and equipment, and ensure its use. 4. Employers should provide forklift operator training that complies with MIOSHA powered industrial truck (PIT) regulations, ensure that employees do not operate a PIT without supervision until the required training and evaluation has been successfully completed, and have a valid permit to operate a rough terrain forklift. MIOSHA has specific training requirements for operators of a rough terrain forklift being used to elevate employees. 5. Employers should design, develop, and implement a comprehensive safety program health and safety plan. 6. Employers should educate employees about the dangers of drug use and that working while under the drug's influence may impair their ability to recognize and respond appropriately to hazardous situations. The victim was operating the bobbin lift/dump in the yarn preparation department of the plant. Empty bobbins were collected in hoppers that were manually pushed up a ramp onto the lift/dump. The lift/dump was then raised and rotated in a counterclockwise direction, dumping the bobbins into a hopper equipped with a belt conveyor that transferred the bobbins to another area of the plant. A guardrail was placed 4 inches in front of the lift dumper. The machine was operated by reaching through the guardrail and pushing the control lever to the left or right to raise or lower the lift/dump. When released, the control lever would return to center. The lift/dump would not move unless the control lever was placed in the raise or lower position. Shortly after 11 p.m., a plant employee walking through the department spotted the victim, who was pinned in a kneeling position between the lift/dump and the guardrail. The employee did not know how to operate the machine but went to the department supervisor to tell him about the victim. A second employee entered the area, spotted the victim, then raised the lift/dump and pulled the victim clear of the machine. The supervisor ran to the victim, then returned to his office and called 911. The Emergency Medical Service (EMS) responded and transported the victim to the hospital where he was pronounced dead. Sometime after 10:00 a.m., the driver of a fuel truck arrived at the site to refuel two diesel compressors supplying compressed air for the jackhammers and other pneumatic tools. He drove into the work zone, which was marked with traffic cones, and parked between the two compressors. Quickly filling both compressors with fuel, he retracted his fuel line and got into his truck to leave. The driver then started to leave the work zone as another worker behind him stopped traffic. He did not see the victim, who was standing or working near the front of his truck. The truck had only moved a short distance when some other workers from the site yelled for him to stop, and the driver looked at his mirrors and saw the victim on the ground behind him. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should institute an internal traffic control plan as outlined in the NIOSH publication Building Safer Highway Work Zones: Measures to Prevent Worker Injuries from Vehicles and Equipment. 2. Truck owners should examine their vehicles for blind spots and install mirrors to increase visibility. 3. Employers should consider the use of electronic signaling devices or sensors to warn equipment operators of the presence of pedestrians in the blind spots of mobile equipment. He was working alone in the kitchen as the remaining staff cleaned the adjoining dining room. He apparently lifted the cover of the mixer, uncovering the 32-inch-diameter mixing bowl, and started the machine. As he reached into the bowl to clean it, he became entangled on a large mixing fork (beater) that rotated inside the mixing bowl. His co-workers heard him scream but were unable to reach him in time. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers should ensure that all equipment, including manufacturer's safety devices, is maintained in safe operating condition. 2. All machines should be de-energized before cleaning or performing other maintenance. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. 4. Employers should know and comply with all state and national laws dealing with child labor. 5. Employers should become familiar with available resources on safety standards and safe work practices. After punching out, the victim went downstairs to retrieve his coat. The victim's coat pocket may have contained several pieces of jewelry. The owner thinks that some jewelry may have fallen out of his coat pocket while he was putting on his coat and/or getting car keys. The victim may have thought that some jewelry fell into the open sump pump crock, and he attempted to retrieve the jewelry. During the retrieval process, the victim became pinned between the metal support pipe and the sump pump well rim. Another restaurant employee found the victim and called the restaurant co-owner. The co-owner called 911. The victim was found bent at the waist, headfirst in the sump crock with his head in the water. Rescue workers used a reciprocating saw to cut the metal support to free the victim. The victim was pronounced dead at the scene. Recommendations: 1. Employers should evaluate the workplace to identify any confined spaces and develop and enforce written work procedures for entry into these areas. 2. Employers should cover sump crock openings with a solid cover to prevent items and/or people from falling into the sump pump area. 3. Employers should develop and enforce written work procedures, including general electrical safety-related work practices. (An inhibited acid contains organic additives that protect the metal surface from the action of the acid). The well was approximately 300 ft. from the road, was approximately 150 ft. west of the residence, and was 20-25 ft. downhill from the livestock barn. A 5 ft. x 5 ft. woodshed was built around the well for protection. The shed was approximately 8 ft. tall, had a 30 in. door facing east, had 2 in. x 4 in. studs and had particleboard walls with batt insulation. There were no windows in the shed. The well pit had concrete walls. The pit was approximately 4-1/2 ft. deep, was 4 ft. x 4 ft. wide, and had a 2 in. x 8 in. wood plank across the pit opening. At the base of the pit, there was a driven point well with a 2-in. steel pipe and a jet pump. The emergency responders found an empty 64-ounce container of muriatic acid, a 1/4 full 64-ounce container of muriatic acid, funnel and 1/2 in. white polypropylene plastic tubing nearby. The funnel was outside of the well's protective structure AB - the tubing was inside of the well. The well casing was open. The well was running "slow" and the owner thought the problem could be a plugged well screen. The farmer used liquid muriatic acid to clean the screen. The event was unwitnessed, so the actual sequence of events is unknown. There appears to have been a reaction when the muriatic acid was added to the well water, resulting in a hydrogen sulfide emission from the well. A family member of the victim detected the muriatic acid odor and went to investigate. The family member noticed the individuals and went to the house to call emergency personnel. When the emergency responders arrived, the individuals were found in the well pit in approximately 4 in. of water/muriatic acid. Upon arrival at the scene, emergency personnel could detect a muriatic acid odor from the road. One of the individuals succumbed to hydrogen sulfide fumes and died at the scene AB - the other individual was transported to a local hospital, where he later died. Recommendations: 1. Private water well owners should not use uninhibited liquid muriatic acid to clean well screens AB - owners should explore acid alternatives that are in granular or palletized form, slower dissolving, produce less hazardous fumes and are more environmentally friendly. 2. Well owners should consider well location, well construction, and well management and maintenance issues so the well and water quality are protected from outside contamination sources. The two incidents below involve automobile mechanics who died when jack stands moved, causing the vehicles or machinery to fall and crush them. A MECHANIC IS KILLED BY A FALLING FORKLIFT: A 52yearold mechanic, Pedro G.* (*Not the victim's real name), died when a forklift slipped off a jack and the forklift hit him in the head. Pedro jacked up the forklift using a hydraulic jack. He did not use jack stands, wood blocks, or any other rigid, stable support to hold the forklift after it had been jacked up. The area of the shop yard under the forklift had a slight incline. Pedro chocked only one wheel of the forklift with a wooden block. The forklift slipped while he was lying on a creeper underneath the forklift. Some workers may assume they can safely work underneath vehicles that are only supported by a jack. Jacks should never be the only support of a vehicle when someone is underneath that vehicle. They should be used to lift the vehicle enough so that additional support can be positioned. A MECHANIC IS CRUSHED UNDER A FALLING BUS: A 48yearold mechanic, Jeremy L.*(*Not the victim's real name), died when a bus that fell off jack stands crushed him. Jeremy had jacked the bus up with lifts that cradle each rear tire. He then placed jack stands underneath the rear suspension. They were not of a standard design. The front tires had not been chocked. The bus slipped off the jack stands as he was performing a brake job. The bus crushed Jeremy between the rear axle and the concrete floor. Vehicles may move if the chocking is inadequate. At a minimum, chocks should be placed on both sides of the wheel diagonally opposite of where the jack is placed. RECOMMENDATIONS: Support vehicles adequately. Use ASME-approved jack stands, wood (not concrete) blocks, or other rigid, stable supports. Prevent vehicle movement. Work on level surfaces and adequately chock the vehicle's wheels. The victims were standing by during the testing of the gas well which had only been erected two days prior. The well was not yet in production at the time of the incident. The flow back line was approximately 4 inches in diameter and was attached to the pump assembly which ran horizontally along the ground approximately 2 yards to a waste pit. The victims, along with three other co-workers, were standing between two parked trucks in an area adjacent to the flow back line. The pump assembly was activated for testing. A build up of pressure caused the pipe to rupture. The flow back line struck the victims as it broke loose following the rupture. The victims were killed immediately. Two of their co-workers suffered catastrophic injuries. One of the two suffered an amputation of both legs. The TX Face (Fatality Assessment & Control Evaluation) Investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure that operating, maintenance, and repair procedures are established, implemented, and documented to include a thorough hazard analysis and a continuous safety-training program regarding all heavy machinery and equipment. 2. Place warning signs and/or barricades and ensure that all nonessential personnel are out of the area when testing of well equipment is being conducted. 3. Ensure that workers' assigned to hazardous tasks have close supervisory contact throughout the duration of the work being performed. The victim had gone to the local redi-mix concrete company earlier that morning and had driven the full concrete truck to the job location. He positioned it in an alleyway behind and adjacent to the work site and walked across an adjoining roadway to where the forklift involved in the incident was parked. With the assistance of his supervisor (company owner), he attached a cement bucket onto the forks. The supervisor walked away towards his truck while the victim drove the forklift back across the street and parked it directly behind the cement truck. He then exited the forklift and stepped in between the forklift and concrete truck to adjust the concrete chute on the rear of the truck. The forklift rolled forward, pinning the victim's abdominal area between the concrete bucket and the extended rear platform of the truck. Two workers heard him scream and were able to push the forklift backwards off of the victim. The supervisor also heard the victim, ran to the scene and immediately provided first aid until the two workers took over. He then ran a short distance away where an ambulance was kept and brought it to the accident site. The victim was transported by the local rescue squad to a hospital 32 miles away where he was pronounced dead later that morning. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences, employers should: 1. Develop, implement and enforce a forklift training program for all employees that may operate such equipment. 2. Ensure that all equipment has an established maintenance program as recommended by the manufacturer. 3. Develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard recognition. The victim was discovered by a coworker, who notified the plant manager to call police and emergency medical services (EMS). Both a fire department rescue squad and EMS arrived at the site. They extracted the victim from the baler. The victim was pronounced dead at the site by the medical examiner. Subsequent examination by investigators revealed that the baling machine was not shut off and locked out and that the safety interlock on the compression chamber door may have malfunctioned, allowing the machine to operate with the door in the open position. The victim was positioned on the ground to connect the manhole sections to the excavator while the coworker was located on the truck, to disconnect the sections after they had been landed on the truck bed. The operator had positioned the excavator's bucket near a manhole section and was waiting while the victim attached a three-leg bridle to the manhole section for lifting. The victim had connected two bridle legs and was connecting the third when the bucket suddenly disconnected from the excavator stick, fell onto the manhole section, and struck the victim, knocking him to the ground. Emergency responders were notified immediately. The victim was pronounced dead at the scene from blunt trauma to the head. The victims were working in windy conditions during the construction of a county sports stadium. The three ironworkers were suspended about 300 feet above the ground to observe the hoisting of a 450-ton roof section. The roof section had been hoisted to about 330 feet and transported over its connection location by the heavy-lift crane crew. As the roof section was being lowered into place, the heavy-lift crane began to tip over. The crane continued tipping, and the roof section collided with the personnel platform, knocking it and the victims to the ground. Fire department and emergency medical personnel were immediately notified and responded within 5 minutes. The victims were pronounced dead on site by the county medical examiner.
