Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim was working from the top of the elevator cage roof where he was greasing the elevator rollers, which were located along the corner legs of the mast and at the bottom of the counterweight. There were two workers in the elevator box who were serving as a signal person and an operator. The three workers had agreed to communicate by verbal and hand signals with the worker on the roof when to move the elevator upwards. At one point, the elevator operator thought he heard the worker on the cab roof give a signal to move the elevator up and he did so. When the elevator went up, the counterweight came down. The worker was crushed between the mast cross bracing and the counterweight and/or the counterweight hitch on the top of the elevator cab. Rescue workers removed the victim within minutes and he was taken to hospital where he was pronounced dead. None of the workers had read the elevator manufacturer's maintenance manual or safety instructions. They were not aware that the correct procedure for working from the roof of the elevator involved moving the operator's control box to the top where the person servicing the elevator has control of it. Also, they did not know that maintenance operations could be performed in different ways that would not have put a worker at risk of injury. The victim had worked for his employer for six years. The job involved tearing off ceramic tiles from a single- family residence in preparation for putting on a new asphalt composite roof. A crew of six laborers and a supervisor had been working during the morning stripping tiles from the roof and hand carrying the tiles and debris in metal garbage cans from the roof to a truck, across a plank, where the debris was dumped. The plank was a 20-foot long by 14-inch wide commercially available ladder jack scaffolding plank. At its highest point, the plank was 11 feet from the ground. At the time of this incident, instead of using a garbage can, the victim was carrying an armload of debris across the plank to the truck when he fell 9 feet to the asphalt surface of a driveway below. He died two days later of head and neck injuries. It is unknown whether the victim had any experience or training in construction or roofing. The attachment was estimated to have weighed 400 pounds. Just prior to the fatal incident, he was moving a large round bale from the storage area. The bale was estimated to have weighed an additional 800 pounds. He elevated the bale and began to transport it to the feeding ring. Traversing a 40% grade slope (22 degrees), with his left rear wheel on the downhill side, he began to further elevate the load to clear the top of the ring. As the total load, now weighing approximately 1,200 pounds, was elevated the tractor's center of gravity (C.G.) shifted forward and downhill. This shift placed the C.G. outside of the tractor's stability baseline, causing the tractor to roll downhill to the left and to land on the victim. The tractor did not have a rollover protective structure (ROPS) or a seat belt. The land owner came out to the field to check on the worker and found him lying on the ground. He called the victim's wife who in turn called emergency services. The EMS and county sheriff arrived within minutes and found no signs of life. The medical examiner estimated that the victim had died immediately after tractor impact. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed by tractor owners: 1. Equip all tractors with rollover protective structures and a seat belt. 2. Convert tricycle-configured tractors to a wide front-end configuration. 3. Prior to their placement, evaluate the location of feeding rings for hazards which can compromise a tractor's stability during bale transport. The victim mounted the tractor and began rolling down a snow-covered farm road which traversed a hillside. As the tractor began to roll, a bump, rut, or snow-related irregularity caught one of the front wheels and canted the front end to the right in a downhill direction. The victim was unable to correct the unexpected change in direction because the tractor had not yet started and therefore the hydraulically-controlled steering system was not functioning. The tractor left the road and began rolling down the hillside. As the tractor approached another snow covered-road approximately 75 feet below the first, the victim was able to start the tractor. He pulled onto the second road while in a sliding left hand turn. The right front and right rear wheels dropped off the embankment below the second road causing the tractor to rollover sideways, landing on the victim and pinning him to the ground. The tractor did not have a rollover protective structure (ROPS) or a seat belt. The victim's employee, who was helping with the starting operation, witnessed the incident. He ran to the victim's aid and tried to free him. Unable to free him, he called 911. EMS arrived and found no signs of life. The tractor was removed from the victim and he was pronounced dead. The coroner estimated that the victim had died immediately after tractor impact. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed by tractor owners: 1. The victim attempted to jumpstart the grader with jumpstart cables but was unsuccessful because the grader was in 1st gear. The victim left the cab. He stood between the rear tires and used pliers to jumpstart across the solenoid. When the engine started, the grader moved forward, running over the victim with the rear tire. The coworker shut down the grader after the blade struck a bank of dirt and stopped moving. The coworker ran to the highway and flagged down a car to call for emergency assistance. The motorist's cell phone could not get a signal. While the coworker was using the motorist's cell phone, another company employee arrived with a load of gravel. The truck driver drove to another area on the road, contacted the office. The office called for an ambulance service (911 emergency service is not available. The ambulance personnel arrived 15 minutes after being notified of the incident. The victim was declared dead at the scene. Recommendations: 1. Employers should discourage jumpstarting equipment AB - if absolutely necessary, jumpstarting a piece of equipment should be allowed only from the battery while the operator is in the equipment cab. 2. Company policy should prohibit use of equipment if not in proper working order, remove from service all equipment in need of repair until repairs are made, strictly enforce employee reporting of defective equipment. 3. Equipment owners should ensure that an owner and/or operator manual is available on the equipment and for use in service/maintenance procedures. 4. Employers must ensure that a person with a valid certificate in first aid training is present at the worksite to provide first aid. 5. Company management should consider developing a joint health and safety committee. 6. Employers should provide all field personnel a means for emergency communication in case of injury. The victim and a co-worker were working together as one of seven teams performing year-end product inventory. The victim, standing on a platform raised by an order picker, was writing down product type and quantity while the co-worker, standing in the operator's area of the order picker, was counting product. The victim fell with the platform's guardrail system when it became detached from the platform. A call was placed and emergency assistance arrived within minutes and transported the victim to a hospital were he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Perform a hazard analysis before introducing new equipment into the workplace. 2. Provide health and safety training to employees before allowing them to perform unfamiliar warehouse tasks. 3. Develop, implement, and enforce a comprehensive health and safety program that includes, but is not limited to, health and safety requirements for year-end inventory. Incident # 1: A 61-year-old male mechanic supervisor with over 30 years of welding experience was fatally injured while torch cutting a 55-gallon metal drum. The drum, which had previously contained diesel fuel conditioner, exploded while he was cutting off the lid to recycle the container. The explosion caused the drum lid to strike the victim in the head and chest knocking him to the ground. He was rushed to a hospital where he was listed in critical condition and died seven days after the incident. Incident # 2: A 48-year-old male welder with over 30 years of welding experience was fatally injured while using a torch to install a bung in a new oil delivery truck tank. The 5,000-gallon truck tank had contained approximately 2,800 gallons of fuel oil at the time of the incident. The explosion caused severe burns to the victim. He was transported to a local hospital where he died eight hours later. Incident # 3: A 28-year-old male service manager with six years of experience was fatally injured while torch cutting a 55-gallon metal drum. The drum, which had previously contained windshield washer fluid concentrate, exploded while he was cutting off the lid. The explosion caused the lid to strike the victim and land approximately 250 feet way. He was rushed to a local hospital where he was pronounced dead on arrival. Recommendations to prevent similar incidents: Treat every container to be torch cut as if it contains a flammable substance. Never weld, cut, or perform other hot work on a container unless the container has been cleaned and vented thoroughly. Test the internal atmosphere of the container, after cleaning and venting and before performing hot work, with a calibrated direct-reading instrument for flammable gases and vapors. Use a hand held manual lid remover when removing a lid from a barrel or drum (instead of torch cutting or using a pneumatic tool, which produces sparks). Never use a container as a hot work bench. In addition, employers should provide annual training for welders that reinforces proper torch cutting procedures including equipment use, awareness of hazards and safe work practices. Department of Public Health AB - The California Fatality Assessment and Control Evaluation (FACE) program has been investigating work-related deaths since 1992. The goal of the FACE program is to prevent these deaths by informing workers and managers of worksite hazards and how to avoid them. The two incidents below involve workers who died while they were working with punch presses. A PRESS OPERATOR: A 39yearold machine operator, John L.* (* Not the victim's real name), died while punching a piece of metal. The setscrews holding the die to the press were stripped. This let the die shift, and the punch and die became misaligned. This caused the machine to make a noise during a cycle. John backed off the machine, but then he stepped on the pedal that started a new cycle. The punch struck the side of the die. This caused a small piece of metal to break off of the punch, and shoot at high speeds into John's stomach. There was no record of when the machine last received maintenance, or of what training John had received. A MAINTENANCE REPAIRMAN: A 44yearold maintenance repairman, Carlo M.* (* Not the victim's real name), died while repairing a punch press. Carlo had been called to fix the machine after a piece of metal had jammed in the punch assembly. Company policy directed that jams be fixed by first cutting off the punch and die and then removing the jammed piece of metal. Carlo removed the jam with a crowbar and then removed a cover plate. The cover plate helped keep the punch and die aligned. The machine then cycled. When the punch started down it was not aligned with the die. The edge of the punch struck the die. This caused a small piece of metal to break off of the punch, and shoot into Carlo's chest. Carlo did not lock, block, and tagout the machine. There was no record that verified the effectiveness of his training. RECOMMENDATIONS: Workers should: Never operate punch presses if the die and punch are not aligned. Follow documented company policies and procedures. Employers should: Make sure that operators receive thorough, documented training. Prove that training is effective through employee testing. Maintain punch presses on a regular basis. Keep records of that maintenance. Install shields to protect punch press workers when metal pieces fly toward the worker. Safety and Health Assessment and Research for Prevention AB - On October 18, 1999, a 45-year-old female "flagger" died after being struck by a dump truck as it was backing up in a residential road construction site. The flagger (victim) was working with a construction company hired by the county to pave the residential street. The construction crew had already completed paving the west side of the street and was in the process of paving the east side when the incident occurred. The victim had been assigned to control traffic at a side street feeding the two-lane road being paved. Full and empty dump trucks were traveling through the work zone. A pilot car was used to bring non-road construction traffic up and down the west side of the road. As the pilot car approached the victim's flagging position during one of its runs, the driver of the pilot car noticed that the victim was in the roadway and in the path of an on-coming dump truck. The dump truck was in the process of backing down the west side of the road to drop its load of asphalt into a paver. Its backup alarm was activated at the time. Shortly after being seen by the pilot car driver, she was struck and killed by the dump truck. Within moments of the incident, the local emergency medical rescue unit was called and arrived at the incident site, but the victim died at the scene of the incident. To prevent future similar occurrences, the Washington State Fatality Assessment & Control Evaluation (FACE) investigative team concluded that flaggers involved in highway construction work zones should follow these guidelines/requirements: 1. Flaggers should not put themselves at risk attempting to stop vehicles intruding into work zones. 2. Employers need to have a continuing process for site and program evaluation and the identification, correction, and communication of hazardous conditions for workers within a changing work zone. 3. Flaggers should be equipped with two-way portable radio communication devices and other emergency signaling equipment. 4. Consider using a spotter to provide direction for trucks and heavy equipment backing up in work zones. 5. Dump trucks should be equipped with additional mirrors or other devices to cover "blind spot" areas for drivers when they are backing up. 6. Employers should develop methods to ensure that flaggers have adequate warning of equipment or vehicles approaching from behind. 7. Employers should continually train all workers regarding specific hazards associated with moving construction vehicles and equipment within a work zone. 8. Employers should develop and use an Internal Traffic Safety Plan (ITSP) for each highway and road work zone project. Fatality Assessment and Control Evaluation (FACE) Program AB - A 48-yr-old male truck driver (the victim) Died after the off-highway truck he was operating rolled 49 feet over an embankment and came to rest on its top in 4 feet of water. The victim was hauling dirt that was being cut from a bank to clear ground for a 3-mile stretch of a new freeway project. As the victim was traveling up a haulage road, a road grader that was scraping mud from the road was traveling toward him from the opposite direction. The victim steered the truck to the right to allow the grader to pass. As he did, a portion of the built-up haulage road gave way under the truck's right-side tires, causing the truck to overturn. The truck rolled over twice while traveling 49 feet down the embankment and came to rest on its top in 4 feet of water. The grader operator immediately ran to the truck and pulled the victim clear. With the help of the two flaggers, they placed the victim on the bottom of the truck cab. The two flaggers initiated cardiopulmonary resuscitation (CPR) on the victim while the emergency medical services (EMS) were called from a company truck. The EMS arrived at the scene within 8 minutes and summoned the county coroner, who pronounced the victim dead at the scene. The pallet was unsecured on the forks of rough terrain forklift. The victim was removing roof fascia boards during a remodeling project at a fruit processing facility. The victim stood on the pallet while the forklift driver raised the pallet to approximately 6 feet off the floor. The forklift driver exited the forklift to wait for the victim to drop the removed fascia board. The victim used a crowbar to loosen one end of the fascia board. The victim pulled the fascia board away from the wall. When the board came loose, the longer portion of the board struck the wooden pallet. The impact caused the victim to lose his balance, and he fell off of the pallet, landing on his back on a concrete floor. A coworker called emergency response. Emergency response treated the victim at the scene and transported him to a local hospital. The victim died ten days later at the hospital. Recommendations: 1. Employees must be trained in the procedures to safely perform a personnel lift using approved platforms. 2. Prior to each personnel lift with a rough terrain forklift, employers should ensure that a pre-lift meeting occurs to review appropriate requirements and procedures for safe work operation. 3. Employers should provide employees with all protective equipment necessary, including a 100% fall protection system compatible with the work being performed, instruct employees in the proper use of the system and equipment, and ensure its use. 4. Employers should develop, implement, and enforce a comprehensive health and safety program that includes, but is not limited to, training in hazard recognition and avoidance and job hazard analysis. 5. MIOSHA may wish to consider developing additional information to assist employers unfamiliar with construction safety standards to distinguish maintenance vs. construction activities and to identify safety issues unique to construction work. 6. MIOSHA may wish to consider whether there are circumstances in general industry settings where a pre-lift meeting would be beneficial in increasing the worker safety when elevating personnel in the General Industry Safety Standard Part 21, Powered Industrial Trucks. The victim apparently lost control of the forklift, which had a load on its forks and the mast fully extended, as he was making a right turn, causing the forklift to tip over 90 degrees onto its left side. The unrestrained victim was crushed under the extended boom/mast of the forklift. Fire department personnel responded 7 minutes after receiving a 911 call from company personnel and, together with police officers, pulled the victim clear as company employees used two forklifts to lift the forklift from the victim. The victim was unconscious and bleeding from head injuries. First aid was provided by emergency medical services (EMS) personnel and the victim was transported to an area hospital where he was pronounced dead at 1:35 p.m., approximately 25 minutes after the incident. The victim and another worker had been in the trench preparing it for a pre-formed concrete manhole that they were about to install. Their supervisor (the excavator operator) sat and waited in the excavator, with the engine running, and the bucket raised a few feet above ground level so that it would be out of the way. The second man in the trench had just turned away from the victim when he heard a "click" and turned back to see the bucket fall. He attempted to move the bucket, but could not. He and the excavator operator used the excavator and a chain to remove the bucket and then called for help. The victim was pronounced dead at the scene upon arrival of emergency medical workers. In order to prevent similar instances from occurring, FACE investigators recommend that: 1. Workers should not work under heavy machinery. 2. A manual-locking pin, although it would require the operator to exit the cab or another worker on the ground to disengage, would dramatically increase the safety of those working with and around the equipment and should be a part of all quick release couplers. Also, the control panel for quick release couplers for heavy equipment should have only two positions, lock and unlock. When in the unlock position, there should be indicator lights as well as audible warnings to alert the operator and others nearby to the coupler's status. The lights should be mounted in a highly visible area. 3. All equipment should be maintained properly, including routine maintenance as recommended by the manufacturer as well as unscheduled repair and replacement of missing, damaged, or worn parts. 4. Trenches deeper than five feet should have the walls shored for workers protection. The driver and the victim had been baling hay all day, and both were experienced farmers. The victim was seated on the fender to the left of the driver. The tractor was a 1969 tricycle-tired John Deere, which was well maintained but was not equipped with a Roll Over Protective Structure (ROPS) or a seatbelt. The tires were appropriately fluid filled. The driver was reversing the tractor down a slight slope in an open field in order to stab a bale of hay with an attached hayfork. The right rear wheel ran over the hay bale causing the tractor to overturn side-ways on top of the victim and the driver. The driver was pushed clear by one of the tractor's rotating wheels. The victim was pinned underneath the tractor's driver seat and died of compression asphyxiation. In order to prevent similar incidents, the Kentucky FACE investigators recommend that: 1. Individuals should not ride as passengers on an operating tractor, and drivers should not allow extra riders/passengers. 2. Tractor should be equipped with rollover protective structures and seatbelts. 3. Farmers should exercise extra caution using tractors when their ability to see is compromised: e.g. when it is getting dark which may limit visibility, and when operating in reverse. 4. Tractors should be equipped with front-end counterweights. 5. Co-workers, who witnessed the incident, immediately called for emergency assistance and tried to help the victim. The victim was freed from underneath the bucket and CPR (cardiopulmonary resuscitation) and first aid were administered. Emergency medical personnel arrived within minutes of the telephone call and the victim was pronounced dead at the construction site. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure employees are not inside excavations while construction equipment is in close range to the excavation edge. 2. Have a competent person conduct daily site inspections including inspections of excavations. 3. Ensure that employees and operators receive health and safety training. 4. Implement a warning system at excavation edges to help minimize injuries to workers from mobile equipment falling into excavations. 5. Develop, implement and enforce a comprehensive health and safety plan that includes hazards associated with excavations. Occupational Injury Prevention Program AB - On August 14, 2001, an 18-year-old male construction materials technician (the victim) died after contacting an energized power line while operating an all-terrain vehicle. The victim was a quality assurance technician at a rock quarry. The quarry site was also used to process, test, and stockpile materials for the nearby road construction project. At the time of the incident, the victim was riding the all-terrain vehicle for purposes other than assigned duties. While cresting one of several piles of crushed rock, the vehicle was stopped or became stuck in the soft material. Approximately 6 feet above the victim's location was a 14,400-volt power line. The incident was not witnessed, and it was surmised from the evidence that the victim stood on the vehicle's foot pedals. The energized power line contacted his back. A co-worker discovered the victim and alerted several other workers in the area. The victim was left in place on the vehicle until he could be safely removed from the vehicle and moved away from the broken power line, which had burned in half and was lying near the location. A supervisor called 911. State troopers, electric utility personnel, and emergency medical service personnel were dispatched. The victim was declared dead at the scene. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure that storage sites for all construction materials are inspected for potential hazards under, at, and above ground levels prior to beginning processing (crushing and storage) operation AB - 2. Maintain a minimum 10-foot safety zone near all power lines AB - 3. Develop a written comprehensive safety program that includes job hazard analyses AB - 4. Ensure all workers are given initial hire safety and health orientation that encompasses their duties and general worksite safety and that they are able to recognize and avoid hazardous situations. Ensure that workers are knowledgeable of proper emergency response actions when responding to a medical emergency or an injury scene near downed power lines and should never assume downed power lines and surrounding ground are de-energized. The victim was working with a co-worker, another laborer, to finish preparing an excavating form for a concrete slab in front of a residential building. The bucket of the skid-steer loader contained gravel and sand. The victim's co-worker was operating a skid-steer loader and being directed into position in front of the building by the victim. The bucket was raised so the operator could see the victim. The victim was standing in front of the building, facing the skid-steer loader to give directions. The operator was signaled to stop at the edge of the excavated form and then signaled to dump the load. The skid-steer loader rolled forward into the depression for the concrete slab and tipped forward. The victim was pinned against the building by the bucket. The operator immediately lowered the bucket and back away from the building. The victim walked a short distance and collapsed. The operator called to two other workers at the site, who called 911 on a cell phone. CPR was started. Emergency medical services arrived and transported the victim to a nearby hospital. The victim died from his injuries shortly after arrival. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure all workers are given initial hire safety and health orientation that encompasses their duties and general worksite safety and that they are able to recognize and avoid hazardous situations AB - 2. They were attempting to remove a linkage pin holding the boom and stick sections together. After several attempts to manually remove the pin, they solicited the assistance of a worker from another company who was operating a backhoe at the construction site. The worker (operator) was not a qualified equipment operator and had limited experience operating a backhoe. The operator drove the backhoe over to the excavator. In order to remove the pin, the workers used the edge of the backhoe's bucket to apply pressure to the pin. A 2-1/2-inch diameter pipe was used as an extension to the pin. The victim climbed up a ladder to hold the pipe in position until adequate pressure from the backhoe held it in place. The co-worker entered the cab of excavator to adjust the boom's position, if necessary, to facilitate the pin's removal. The backhoe operator could see the victim's torso and right hand below the bucket AB - his left hand and the pipe were visible above the bucket. The victim, using his right hand, gave hand signals to the operator to move forward and place the edge of the bucket on the pipe. However, the victim's position placed his head in front of the bucket. Before the victim could move down the ladder, to clear his head from the bite zone, the backhoe lurched forward. The victim's head has struck by the bucket AB - the force knocked him off the ladder and against the boom of the excavator. The co-worker came to the aid of the victim while the backhoe operator drove the backhoe to another area to call for help. The victim was transported to a nearby medical center by emergency medical service where he died from his injuries. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure workers are capable of recognizing and avoiding hazardous situations and should develop and implement a safety program facilitating safe worksite practices AB - 2. They found the victim on the ground, but due to his massive head injuries, they did not start CPR. They went to the office and called 911. The victim was declared dead at the scene.
