Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The bulldozer owner/operator and a laborer were having difficulty starting the bulldozer. Together, the two men had tried several different methods to start the bulldozer including: spraying ether into the air chamber, having the battery recharged, replacing the battery cables with new ones and tightening wires to the starter. They worked from the left side track of the bulldozer. The laborer sat towards the front of the track tightening the wires to the starter while the owner stood on the track to the right of the laborer three to four feet away. As the laborer tightened the main lead wire to the starter, the owner stood on the track, leaned into the cab of the bulldozer and pushed the start button. The two men did not realize the dozer was in reverse gear. When the owner pushed the button, the bulldozer jumped backward, throwing the laborer off the side of the bulldozer track and throwing the owner underneath the track. The owner died at the scene from massive injuries to his pelvis and chest. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Always start bulldozers and other heavy equipment from the cab with the seatbelt fastened. 2. Self-employed company owners should take continuing education courses including equipment maintenance and troubleshooting techniques. 3. Do not sit or stand on parts of bulldozers or other heavy equipment not intended for human occupancy. The victim hung a rope with knots to climb down into the silo, approximately 8-10 feet. The victim entered the silo AB - the coworker remained outside of the silo. No odor was present upon opening the silo hatch. "Silo gas" may not exhibit any detectable odor. The blower was not running. The victim entered the silo and cried out for help. The coworker reached for the victim's wrist but could not pull him out. The coworker called the victim's wife for help. The wife sent another family member to assist and also called for emergency assistance. The coworker and family member remained outside of the silo waiting for emergency assistance. On the morning of the incident, coal from an emergency stockpile was transported through a ground opening that was covered by a steel grid called a "grizzly"(14' X 14') with 12" by 12" grid openings. Under the grizzly there was a 15 feet deep hopper, a coal cracker, and a second hopper that was 12 feet deep connected to a conveyor belt. On January 22nd, nineteen days before the fatal incident, an operator had recorded damage to the grizzly in the team's daily log: the steel bars (1"X 10") that formed the grids were broken and bent resulting in a 2' by 2.5' hole in one corner of the grizzly. A decision was made within the team not to repair the grizzly until spring. On the morning of the incident, the victim was seen by two of his co-workers (an operator and an engineer) walking on the grizzly with a shovel in his hand. It appeared that the victim was using the shovel to break the frozen coal and clear the clogged grids. Then, at approximately 9:18 AM, the engineer noticed that the shovel was left on the grizzly but the victim was gone. He immediately told the operator to shut down the coal transporting system. The victim was found on the conveyor belt 50 feet away from the grizzly. The victim apparently fell through the hole on the damaged section of the grizzly into the hoppers and the frozen coal cracker and was carried out by the conveyor belt. The operator immediately called an incident commander in the plant's master control room. The incident commander summoned an ambulance that arrived at the site in ten minutes. The victim was transported to a local hospital where he was pronounced dead. New York State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to prevent similar incidents from occurring in the future, employers should: 1. Repair damaged equipment in a timely manner, and place warning signs and guards immediately on and around the damaged equipment to warn workers of the safety hazard AB - 2. Develop standard procedures for emergency situations in which employees may have to walk on a grizzly and mandate the use of personal fall arrest systems AB - and 3. Evaluate the effectiveness of current plant occupational safety and health management structures and establish a system that anticipates problems and provides clearly defined responsibilities and accountability. The victim and coworker were in a raised position, approximately 10 feet off the ground in the truck bed and had been working for approximately one hour. The control to raise and lower the truck bed was located near the bulkhead of the truck bed. They attempted to lower the raised truck bed, but it wouldn't move. They called down to another coworker, who told them to try to raise the bed a little, and then try to lower the truck bed. The victim and coworker were standing near the bed's bulkhead when one of them tried to raise the truck bed. When they attempted to raise the bed, the truck bed bulkhead was thrust upward, and the rear of the truck bed dropped quickly toward the ground (See Figure 1). The victim and coworker were crushed between the bulkhead and a bridge beam, and then slid off the angled truck bed to the ground. 911 was called and emergency responders arrived. Both individuals were taken to a local hospital, where the victim died of the injuries sustained in the incident. The coworker survived. Recommendations: 1. Manufacturers of dual use truck beds should consider using separate interlocked control systems to reduce or eliminate the risk of worker injury from unexpected machine motion. 2. Pre-operation safety checklists should be developed for equipment used in work operations. 3. Employers should ensure only trained operators operate aerial lift equipment. 4. Employers should ensure that all aerial lift controls are properly labeled in a language understood by the operator. 5. Employers should conduct a job hazard analysis for existing and new work procedures and provide employees job hazard analysis training. 6. A health and safety committee should be developed and implemented that includes representatives from both management and union. 7. Ensure that workers who are part of a multilingual workforce comprehend instructions in safe work procedures for the tasks to which they are assigned. The victim was using a hot stick to loop the cable but was not wearing his hardhat when his head came either in close proximity to or contacted the charged switch. Crewmembers saw a flash and came around the switch cabinet to where the victim was located. He was found slumped partially in the cabinet. A crewmember used a hot stick to move the victim away from the cabinet and then began CPR. Emergency Medical Services transported the victim to a nearby hospital where he was declared dead from injuries associated with high-voltage electrocution. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure workers use personal protective equipment and enforce its use AB - 2. Emphasize de-energizing, isolating, or cover energized work areas whenever personnel need to work within high voltage danger zones. The man was part of a construction crew preparing to pour a concrete foundation for a residential garage in a neighborhood. One man was operating a skid-steer loader, leveling the ground when another employee just arrived in a dump truck with a fresh load of gravel. Two other younger employees, one of whom was the victim, were locking down the tailgate so the truck could dump gravel on the ground. The operator of the skid-steer loader was not aware the dump truck had returned, and continued working with the loader facing in the opposite direction. At one point, the skid steer backed up to get another load of gravel. While working with the tailgate of the truck, with his back to the loader, the victim was struck from behind by the skid steer loader, and was crushed between the back of the loader and the back of the dump truck. The man staggered a bit, then dropped to the ground. He quickly became unresponsive and died shortly after from severe internal injuries. His co-worker was uninjured. The operator of the skid-steer loader was not initially aware of striking the man, for he did not make contact with the truck while backing up. Recommendations based on our investigation are as follows: 1. Employee safety training and education must be an integral component of every business mission, regardless of size or number of employees. 2. Employers must insure that employees are aware of hazards, limitations, or potentially dangerous conditions associated with using specific equipment on the jobsite. 3. Backup alarms with variable intensity and frequency should be installed on all skid-steer loaders, and owner/operators should ensure that mufflers are working properly to reduce machine noise. The force of the blow from the cap striking him in the face knocked him backwards over the rail of the scissors lift that he was standing on to a concrete floor 14 feet below. When his co-worker, working on the lift immediately to the left of the victim, heard the noise, he looked to his right, but the victim was already on the floor below the lift. Figure 1 is a photograph of the pipes on which the men were working with the scissors lift (shown in the retracted position) below. The second pipe from the left is the pipe involved in the fatality. It was recessed into the wall opening from the force of the end cap exploding off of it. Another co-worker who heard the noise came to assist and found the victim bleeding from the face and having difficulty breathing. EMS arrived shortly, treated the victim, and transported him to the emergency room of a local hospital. He died in the hospital eighteen hours later. Recommendations: 1. Employers should ensure that their written accident prevention plan identifies and describes all hazards that could be encountered in the worksite and how to recognize and avoid them. 2. Develop a specific standard operating procedure for the task including tagging the valves with time and date of pressurization. 3. Use an end cap that remains attached to the pipe during loosening as the pressure is relieved and prior to removal or outfit the end cap with a pressure relieving device. 4. The victim had entered the trench to measure the depth, while the excavator operator switched buckets from a narrow bucket to a larger one. After he attached the larger bucket, the operator raised the bucket to work on an adjacent trench. The bucket suddenly disconnected from the bucket arms, fell to the edge of the trench and rolled into the hole onto the victim. The operator immediately saw the victim was injured, and called 911 from a cell phone. He used a chain to pull the bucket away from the victim with the backhoe. EMS responders and Med Flight responders arrived within several minutes. The victim was pronounced dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Install and maintain positive locks on quick-disconnect equipment according to manufacturers' specifications and recommendations. 2. Train excavator operators to conduct visual and operational checks after changing attachments. 3. Prohibit workers on foot from entering the swing area of hydraulic excavators until they signal the operator to shut down the machine and receive acknowledgement from the operator. After the loader bucket was raised to the window, the backhoe/loader operator noticed that one of his coworkers was losing grip on the plywood. The operator, who had placed the loader bucket controls into neutral, jumped off the backhoe/loader to assist. As he jumped, the strap of his fall protection harness, worn as required by company policy, became entangled on the loader bucket control lever, moving it to the dumping position. When the bucket tilted forward, one coworker jumped off the bucket unharmed AB - the victim, who remained in the bucket, was pinned, and crushed against the building. The backhoe operator reversed the backhoe/loader to free the victim. Emergency Medical Services (EMS) personnel arrived within 5 minutes of the 911 call and transported the victim to a local hospital emergency room where he was pronounced dead. The openings had been covered with plywood siding and the roof membrane placed over the cover and loosely nailed. The covers were nailed loose enough that a person could pop them free by hand. The victim went on to the roof to install larger curbs as part of a three- man crew. Two workers on a lower roof level were to lift the curbs up to the victim. They pushed the first curb up to the higher roof level and the victim pulled it up. When they pushed the next two curbs up they got no response from the worker above. They went up to the higher roof level to look for him. They found that a cover had been removed from one of the openings and their coworker had fallen into the opening and landed on the floor 32 feet below. It is not known why the cover had been removed from the opening at that time. Safety & Labor Standards Division A2 - Nebraska Workforce Development AB - A 38-year-old cattleman died as a result of an accidental injection of an animal antibiotic known as Micotil which has no known antidote. On March 8, 2003, the victim was preparing to vaccinate a heifer inside a barn. He was carrying a 12cc plastic disposable syringe in his right hand when a cow that was in an adjacent pen charged him, striking the fence between the two. The victim was knocked to the ground. Either when struck or from the fall, he was injected with an unknown amount of the antibiotic. He immediately began to feel dizzy and nauseous. He was able to return to the "vet room" inside the barn and call his wife who was nearby in the house. An ambulance was called and the victim was rushed to a nearby hospital where he died less than an hour later. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences: 1. Veterinarians and animal health distributors, prior to releasing Micotil, should require the purchaser to sign a product information fact sheet that indicates Micotil can be fatal in humans, and that there is no antidote for this medication every time they purchase the product. 2. Users of syringe-loaded medications should practice safe handling procedures during all phases of animal treatment. 3. Veterinarians/Cattlemen, when practical, should consider using another less-hazardous antibiotic. 4. All companies/agencies responsible for the manufacture and/or approval of veterinary medicines and supplies should continue to devise new products that will reduce unintentional human exposure to accidental needlesticks/injections. Earlier in the day, the decedent had removed the electric cord to a single door, glass-fronted vertical cooler on rollers. He replaced the electrical cord from the unit with an electrical cord he had fabricated himself by attaching one end of a 10-foot length of 10/2 wire to the cooler and the other end to one-foot length of extension cord which plugged into the wall receptacle. The unit was moved around the kitchen all day while the kitchen was cleaned. The ground wire on the 10/2 length of wire came loose and came in contact with the metal frame of the cooler. The owner then moved the cooler eight to twelve inches cattycorner to a gas cook stove. The owner, who was hot, sweaty and shirtless bent down between the cook stove and the cooler. As he did so, his chest touched the outside aluminum casing of the cooler as his back touched the metal stove. He was electrocuted when the electric current traveled from the cooler through him to ground through the stove. To prevent similar incidents from occurring, Kentucky Fatality Assessment and Control Evaluation program recommends: 1. Perform a hazard evaluation at work site before any work is commenced. 2. Only persons properly trained in electrical procedures should perform electrical services. Although the tractor did not hit him, the attached brush hog ran over him, dragging him in the process. The tractor and brush hog then continued down the slope, and stopped upon running into a pick-up truck parked in the farmyard. A neighbor later noticed the tractor and went to see what had happened. Upon discovering the farmer, the neighbor returned home to have his wife call 911. The emergency squad and sheriff's department responded to the scene within approximately 10 minutes. Upon inspection, the county sheriffs notified the coroner who responded to the scene and pronounced the victim dead at the location. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, farmers performing this type of work should: 1. The victim had worked at the plant for three years and was employed as a rigger/spotter for a large mobile gantry crane used to move concrete slabs in the plant yard. The crane was a box-like assembly of steel I-beams riding on four 62-inch-high rubber tires. With a lifting capacity of 100,000 pounds, the 30-foot-high, 45-foot-wide diesel-powered crane was operated by a worker in an elevated cab. The victim was walking ahead to spot for obstacles as the crane straddled a large concrete slab which blocked the operator's view. A second spotter saw the victim on the ground after he had been run over by the right front wheel of the crane. The victim was airlifted and admitted to a regional trauma center where he died 15 days later of complications related to his injury. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Employers should ensure that all machine guards are in place before operating equipment. 2. Employers should consider installing devices that increase the visibility of the crane operator. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. The company did not have a written code of safe practices that covered the hazards of working in close proximity of moving heavy equipment. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure employees remain out of the immediate area where heavy equipment is operating. 2. Ensure that when visual contact is lost with workers on foot, the equipment operator stops the heavy equipment and does not resume until visual contact is re-established. 3. Ensure that the equipment being used has a working back-up alarm. 4. Ensure that Injury and Illness Prevention Programs include a written code of safe practices on all hazards associated with the work being performed. 5. Occupational Health Service AB - On August 30, 2002, a 39-year-old male forklift operator was fatally injured when he was crushed under a fallen forklift. The victim was a full-time forklift operator at a company that manufactured wooden pallets. At 2:48 p.m., the victim was using an 8,600-pound forklift to move waste material into a large, drive-in waste dumpster positioned at the company's outdoor loading dock. The victim had apparently just dumped the waste and was backing out of the dumpster when he backed off the side of the loading dock, falling 3 feet 9 inches to the asphalt. He was partly thrown from the forklift and was crushed under the lift's rollover cage. He was taken by helicopter to the area trauma center where he was admitted with injuries to his hip and leg. Despite treatment, he suffered complications related to his injury and his condition deteriorated. The victim died at the hospital on September 9, 2002, nine days after the incident. NJ FACE investigators recommend following these safety guidelines to prevent further incidents: 1. Employers should ensure that employees are fully trained and certified as per OSHA regulations before operating a forklift truck. 2. Operators should always wear a seatbelt while running a forklift or other industrial truck. 3. Employers should provide safety curbs and railings around the perimeter of outdoor loading docks. 4. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. The victim was assisting the roofer repair water leaks on the flat roof of a commercial warehouse. The incident occurred at the completion of the two-day project. The victim was walking backwards to roll up a torch hose when he apparently tripped or stepped into the skylight. The acrylic plastic domed skylight shattered under his weight and the victim fell approximately 35 feet to the concrete floor. The victim's father immediately called for assistance and notified emergency services. The victim was transported to a local emergency room where he died a short time later. The OR FACE team was notified by local news media reports on January 28, 2003. Contact was made with both the building owner and the roofing contractor. The roofing contractor was interviewed by phone. The building owner declined an onsite investigation of the incident scene. The OR FACE investigator visited the street location without accessing the building or the roof (incident location), on February 5, 2003. This report is based on the interview with the employer and the onsite walk around of the building. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Identify and guard known and potential safety hazards to all workers or visitors who access the work site. 2. Ensure that buildings with plastic (acrylic) domed skylights are retrofitted with fall prevention (i.e., steel grills) to ensure that the possibility of catastrophic falls is reduced or in the alternative use warning systems to alert workers of an immediate hazard to their life/safety. 3. Employers should implement site-specific safety programs to address and reduce work related risks through the integration of safe work practices into routine work activities. Within a 15-month period during 2000 - 2001, three Massachusetts municipal police officers were fatally injured while performing traffic details in work zones. Incident #1 A 60-year-old male police officer was fatally injured when he was crushed beneath a dump truck loaded with asphalt in a highway work zone. The work zone was located in front of an entrance way to a parking lot serving a group of retail stores. Motorists had to pass through the construction site to access the parking lot. The victim was assisting two dump trucks in the work zone to back in front of the parking lot entrance way. While walking away from the dump trucks, in preparation to help them back, the truck involved in the incident started backing without the officer's knowledge and ran over him. In this incident, there was no internal traffic control plan (ITCP) for the work zone; paving was performed during peak retail hours; the dump truck backup alarm was inoperable; and the officer was wearing the same color as the surrounding temporary traffic control devices. Incident #2 A 66-year-old male police officer was fatally injured when he was struck by a backing dump truck loaded with asphalt. The dump truck, which had a functioning backup alarm, was backing within a highway work zone to a small unpaved section of a turnout that was located approximately 1,000 feet away from where the truck was stopped. The victim, with his back to the dump truck, was walking towards the turnout at the edge of the work zone in a closed travel lane facing oncoming traffic when the dump truck started backing. The dump truck struck the victim and ran over him. In this incident, there was no internal traffic control plan (ITCP) for the work zone. Incident #3 A 38-year-old male police officer was fatally injured when a motorist intruded into a street work zone striking him. The victim was directing traffic through a detour at a four-way intersection located several hundred feet after a crest of a hill. Traffic heading west was turning right at the detour and traffic heading south was turning left at the detour. A motorist traveling west failed to turn right and skidded sideways through the detour, striking the victim, the victim's parked truck, a stone wall, and a tree. In this incident, the work zone lacked adequate signage, and the officer was wearing the same color as the surrounding temporary traffic control devices. To ensure the safety of all individuals in and around highway/street work zones, contractors should: Develop and implement Internal Traffic Control Plans (ITCP) to coordinate the flow of construction vehicles, equipment, and workers on foot within work zones. To reduce the hazard associated with backing construction vehicles, an ITCP should minimize backing of all construction vehicles and equipment on site. This can be accomplished by reviewing and planning tasks so that vehicles can navigate safely through the construction site while backing as little as possible. The ITCP should also include walkways for workers on foot that are clear of backing construction vehicles and equipment. In addition, some areas within the work zone might have to be designated as areas that are prohibited for workers on foot. Develop work zone backing procedures to include designated signalers to direct backing construction vehicles. Equipment operators and truck drivers should be aware of who the signalers are and the established backing protocol. Construction vehicle and equipment operators should never start backing in a work zone without a clear and understandable signal from a signaler that he is ready and it is safe to start backing. Ensure that work zones are setup, at a minimum, in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part 6, developed by the U.S. Department of Transportation Federal Highway Administration. The MUTCD sets forth the basic principles that govern the design and use of traffic control signs and devices and provides uniform work zones designs. In incident #3, a "road closed" warning sign indicating the distance to the closure should have been setup in the advanced warning area of the work zone. Consider traffic demands and construction site surroundings (residential, commercial, and/or retail) when scheduling highway/street construction projects. Construction site surroundings and traffic demands will impact the work zone design, and time of day and year work should be performed. In incident #1, the work zone was located in front of an active entrance way to a parking lot for retail stores, and the work was being performed during retail operation hours and a busy holiday season, Perform regular preventive maintenance on vehicles and equipment to ensure vehicles and equipment are in safe working condition. When a problem, such as an inoperable back up alarm, is identified, the vehicle or equipment should be taken out of service and repaired before being placed back in service. Local municipal police departments should consider: Work zone safety training for officers. Police officers employed by Massachusetts cities and towns typically do not receive training on work zone safety, although this training is currently available for the Massachusetts State Police. Work zone safety training would provide officers with the knowledge to better protect themselves, construction workers, pedestrians, and motorists. This training should include safe backing procedures and proper work zone setup and design that meet requirements of MUTCD, Part 6. The Effectiveness of Officer Personal Protective Equipment. Officers performing traffic details should wear personal protective equipment (PPE), such as high-visibility safety apparel. Safety apparel should routinely be inspected to ensure it has not faded and that retroreflective properties have not been lost. In addition, to ensure that officers do not blend into the background, choose safety apparel colors that are different than the surrounding construction equipment and landscape. Manufacturers of heavy construction equipment, such as dump trucks, should: Explore the possibility of incorporating collision warning devices on their equipment that would assist operators in backing. In incidents # 1 and # 2, the police reported that the truck operators stated they did not see the victims behind the trucks prior to backing over them. Equipment manufacturers should evaluate current research being conducted on navigation and warning aids for mobile equipment to develop collision warning systems for their products. The saw, manufactured in 1991 and equipped with a 10-inch woodworking blade, was located in the woodworking shop, near the shop door. The student was making a lengthwise cut (ripping) and reached around to the back side of the unguarded saw blade to grab the cut piece of wood. The student's two fingers and thumb came in contact with the rotating saw blade, resulting in the serious injuries to the left hand. To prevent similar incidents, schools that have woodworking shops should: Ensure that the manufacturers' supplied guards are installed on all table saws, and that the guards function properly and are used at all times. Position tables at the backside of saws to prevent the work pieces from falling off the saws and onto the floor. Design the woodworking shop layout to reduce distractions to equipment operators. Provide woodworking class conditions that at a minimum meet safety and health standards set by the Occupational Health and Safety Administration (OSHA) and train students about these standards.
