Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Around 8:15 a.m., the assistant informed the lead that plastic debris were escaping from a metal waste container near the stacker. The lead told the assistant not to worry about it and to get a backpack leaf blower to clean up the debris. The lead then walked toward the tail end of the energized stacker. As the assistant got the blower, he heard the stacker shut down and yelled to the lead. Getting no response, the assistant walked to the machine and saw the lead under it with his head, left arm and shoulder caught between its unguarded steel return idler roller and rubberized conveyor belt. The lead's sweatshirt hood was entangled in the roller, and a spade metal scraper with a long wooden handle was also caught between the roller and belt and pressed against his throat. When the assistant could not pull the lead out, he ran to the operations trailer 75 feet away, shouting for help. The crew supervisor and facility manager came and released the lead by cutting his sweatshirt and the conveyor belt and breaking the scraper's handle. They did CPR until first responders arrived. The lead died at the hospital after nine days on life support. CONTRIBUTING FACTORS: Conveyor belt safeguard not in place. Lockout/tagout (LOTO) requirements in place but not followed. Inadequate LOTO training and enforcement of policies and procedures. Periodic reviews of hazardous energy control program not conducted. RECOMMENDATIONS: Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to protect workers from similar hazards employers should: Ensure that equipment safety guards are not removed, displaced, or carried off. Identify LOTO procedure steps and how to properly use and test LOTO devices' effectiveness for specific equipment. Conduct and document periodic reviews at least annually to make sure workers know and can apply LOTO procedures, including how to provide notification before LOTO application and removal on affected equipment. Document that LOTO training had been done and kept up to date for all authorized and affected workers. The California Fatality Assessment and Control Evaluation (CA/FACE) program investigator determined that, in order to prevent similar incidents, construction companies that operate excavators should: 1) Establish procedures that will ensure workers are not working within the excavator boom swing radius or beneath an elevated load. 2) Ensure that operators maintain and inspect quick couplers to prevent malfunctions that may cause an unintended release of the excavator's bucket. Prior to the incident, the framer was setting up to nail a piece of plywood sheeting to a gable of the house. He positioned a 24- foot fiberglass extension ladder against the side of the house. The base of the ladder was resting on soft, uneven dirt. He began climbing up the ladder with the intention of securing the ladder to the house's top frame. As he was climbing the ladder, it shifted to the left and he fell eight feet to the ground. After his fall, the framer experienced numerous serious injuries that kept him from working for several months. REQUIREMENTS: You must place the ladder either: (a) With a secure footing on a firm, level support surface; or (b) Secure the ladder to prevent accidental displacement. You must make sure no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: Survey the worksite to assess the risks of working at height with ladders. Choose and provide employees with safer methods and equipment to work at heights, such as a scaffold or aerial lift. Train employees to use these methods and equipment. When using ladders: Re-check for stability and levelness while climbing the first few ladder rungs. Tie off the top of the ladder, and the bottom if possible, to prevent it from slipping sideways. Consider using ladder feet spikes to prevent the ladder base from slipping when placed on grass or dirt. Do not carry construction materials while climbing a ladder. Use a hoist to bring materials to a higher elevation. This facility has been used by many film crews over the years due to its unique visual characteristics. Shortly after 8:00 p.m., the crew began to wrap up filming for the day. When they returned to their hotel, they realized the grip from their crew was missing. Two film crew members later discovered the grip at the bottom of an empty elevator shaft where he landed after having suffered an 86-foot fall. CONTRIBUTING FACTORS: Facility hazards not assessed and communicated to workers by employer. Inadequate guard on the elevator shaft. Low light conditions. Not following the safety buddy or check-in system. No employer policies or training for crew regarding facility hazards. Fall hazards not communicated by facility managers. RECOMMENDATIONS: Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to protect their workers and themselves from similar hazards film industry employers should: Conduct a site safety assessment (SSA) before working in any structures or environments. Enforce safe work practices to protect workers from hazards identified in an SSA Develop policies prohibiting workers from entering structures or environments before approval to enter is granted and directions are given about what to do when hazards are encountered. Train workers to identify and avoid hazardous areas and follow procedures for safely responding to hazards. His employer was a roofing contractor hired by the general contractor to work on the roof of a newly constructed warehouse building. As the building was nearing completion, the general contractor notified the roofing contractor that there was a leak in the roof. On the day of the incident, three roofing workers arrived at the building to repair the leak. The general contractor superintendent offered them the use of a boom lift to access the roof. They did not have enough safety harnesses to use the lift. So at the suggestion of the superintendent, they decided to use a fixed ladder to gain access to the roof. The ladder was located in an electrical room inside the warehouse. The 38-foot steel vertical fixed ladder had a metal cover at its bottom secured by a padlock, to prevent unauthorized access. The ladder also had a safety system installed on it consisting of a climbing trolley attached to an aluminum track or carrier located between the ladder's side rails. The trolley had a locking carabineer to which the climber can directly connect the front waist D-ring of their full body harness. The trolley moves with the climber and if they fall, it immediately arrests the fall. The roofers did not have the required safety gear for the ladder, nor were they aware that they needed it. The superintendent then unlocked the ladder cover. One of the roofers climbed the ladder and went through a hatch onto the roof. Another roofer began climbing the ladder. Rainwater coming through the open roof hatch had made the ladder slippery, which caused him to lose his grip on a ladder rung. He then fell 10 feet to the plywood roof of the electrical room. He was hospitalized with injuries to his chest and legs. Investigators found that none of the roofer's or the general contractor's employees were trained on how to use the fixed ladder and its safety system. REQUIREMENTS: Employers must train employees to recognize ladder hazards and the procedures to minimize these hazards. Climbing and descending fixed ladders. Ladder safety devices. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: Ensure that employees have safety equipment, systems, and devices required for assigned tasks and that they are trained on how to properly use them. The decedent was employed by a town sanitation department. He was a helper on a three-men refuse collection crew consisting of two helpers and a driver who was the crew chief. On the day of the incident, the crew was doing residential curb-side pickups along a designated route. At approximately 2:30 p.m., the crew arrived at the last street of their collection route. The truck was to back approximately 0.34 miles all way to the end of the street before moving forward for the helpers to pick up garbage on right side of the street. While the truck was backing, the two helpers were riding on the riding steps. There was an SUV parked on the driver's side of the street, and a lawn service truck with a trailer parked on the right side of the street. The refuse collection truck backed approximately 330 feet and passed the SUV. When the driver was trying to back past the lawn service truck, his right-side mirror contacted the lawn service truck, and the impact broke the mirror. Due to the broken mirror, the driver could not see the right rear side of the truck where the decedent was riding. The driver continued backing and felt his truck contact the lawn service truck. He stopped and drove forward to pull away. According to the eyewitness, the victim was pinned and crushed in between the lawn truck and the refuse collection truck. As the refuse collection truck pulled away, the decedent fell to the ground, and he was subsequently run over by the rear right tire of the refuse collection truck. The crew member immediately called 911 and the Emergency Medical Services responded within minutes. The injured worker died on route to the hospital. The cause of death was hemorrhage due to blunt force torso trauma. CONTRIBUTING FACTORS - Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events. The NY FACE investigation identified the following key contributing factors in this incident: 1) Employees were on the riding steps of a refuse collection truck while it was backing up. 2) With an SUV parking on one side and a lawn service truck parking on the other side of the street, the total backing clearance for the refuse collection truck was estimated at approximately five feet, or 2.5 feet on each side of the refuse collection truck. 3) The exterior right-side mirror was broken due to the refuse collection truck contacting the lawn service truck. 4) The refuse collection truck driver could not see his right rear side where the decedent was riding due to the broken mirror. 5) Employer did not address frequent incidents of employee violation of safety requirements when using the riding steps. 6) The progressive disciplinary program was not followed through. 7) There was no refuse collection safety training four years prior to the fatal incident. RECOMMENDATIONS - NY FACE investigators concluded that, to help prevent similar occurrences: 1) Employers should consider prohibiting the practice of riding refuse collection truck riding steps by removing these steps. 2) Employers should conduct incident investigations to identify risk and contributing factors and develop specific prevention measures. 3) Employers should provide employee training to ensure that workers understand the hazards associated with riding on the riding steps of refuse collection trucks and safety rules. 4) Employers should conduct job hazard analysis (JHA) to identify hazards and risk factors and develop effective measures to prevent worker injuries associated with refuse collection truck riding steps. 5) When implementing a progressive disciplinary program, employers should ensure that appropriate disciplinary actions are carried out and followed through. 6) Employers should ensure that the riding steps are inspected regularly, and damaged steps are repaired immediately. 7) Employers should consider implementing an automated system for refuse and recyclable material collection. 8) In-depth studies should be conducted to assess the effectiveness of the current safety regulations on refuse collection truck riding steps. This is the 19th annual Michigan Fatality Assessment and Control Evaluation (MIFACE) report on acute traumatic work-related deaths in Michigan. There were 163 work-related deaths in 2019, an increase of 4 deaths compared to 2018. There were 155 separate incidents (two deaths occurred in each of four separate incidents, with one further incident resulting in five deaths) and 160 separate employers. A narrative summary of each work-related fatality is in Appendix I. MIFACE educational material, including on-site Investigation Reports, Summaries of MIOSHA Investigations, and Hazard Alerts are located on the MIFACE webpage on the Michigan State University Division of Occupational & Environmental Medicine (MSU OEM) website. Key findings for 2019: The number of work-related deaths (163) was up compared to 2018 (159 work-related fatalities). The fatal injury rate (3.4 deaths/100,000 workers) stayed constant. The overall rate of work-related deaths in Michigan is lower than the rate in the United States (3.5 deaths/100,000 workers). The industry sector with the highest employment-based industry rate was Agriculture, Forestry, Fishing, and Hunting (38.8 deaths/100,000 workers), followed by Mining (36.1 deaths/100,000 workers) and then Construction (17.9/100,000 workers). Agriculture also had the largest number of work-related deaths (32 deaths, 19.6% of all fatalities) followed by Construction (31 deaths, 19.0%). Motor vehicle crashes were the leading cause of work-related death (31, 19.0%), followed by suicides (23, 14.1%), struck-by incidents (21, 12.9%) machine-related incidents (20, 12.3%). Illegal drugs, alcohol or side effects of prescribed and over-the-counter medication were potential factors in 18.2% of the non-suicide and non-drug abuse deaths. By occupational group, Management had the largest number of work-related deaths (44) followed by Transportation & Material Moving (33) and then Construction & Extraction (23). Fifty-six of Michigan's 83 counties (67.5%) had a work-related fatality. Wayne County had the highest number of deaths (25, 15.3%), followed by Oakland (12, 7.4%), Macomb (7, 4.3%), Washtenaw, Genesee, and Kent (6, 3.7%), and Clinton and Ingham (5, 3.1%). Of the 163 work-related fatalities, 39 (23.9%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. The press frequently jammed, requiring the decedent to place two safety blocks between the ram and bolster and reach into the press to unjam the machine. The decedent would then remove the safety blocks and return to actuate the machine at a dual-button control panel mounted to a pedestal, placed in front of the long side of the press. The decedent and the shift supervisor had to unjam the press multiple times. The last time they removed the jam, the safety blocks were unintentionally left on the bolster bed. The decedent, who was standing in front of the press by the pedestal controller, actuated the press. The two safety blocks were immediately ejected from the press AB - one block struck the decedent in the neck and chest inflicting severe injuries. Immediately after the injury, 911 was called, and another employee tried to help with basic first aid. Emergency medical technicians (EMTs) responded in minutes, but the employee died at the scene due to blunt force injuries to the neck. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events. The NY FACE investigation identified the following key contributing factors in this incident: 1) Safety blocks were left in the press when the press was actuated. 2) Safety blocks were not interlocked to the press circuitry. 3) Safety blocks were not aligned with the longitudinal central axis of the ram, causing a pressure difference between the two halves of the press. 4) The operator was in the path of the projectile. 5) No written procedures for setting up the machine, placing dies, clearing jams from the machine, or locking out the machine existed for reference by the operator(s). 6) Limited training on the press was given to the decedent prior to his utilization of the press. RECOMMENDATIONS: NY FACE investigators concluded that, to help prevent similar occurrences: 1) Employers should ensure that interlock devices are used in conjunction with safety blocks when working with mechanical power presses. 2) Employers should ensure that the selected safety blocks meet the rated capacity of the specific power presses. 3) Employers should place safety blocks centered along middle length of press when servicing. 4) Employers should ensure that press controls are moved to short ends of press to avoid risk of being struck by materials ejected during press operation. 5) Employers should design machine guarding that allows for the safe movement of all employees around active machinery. 6) Employers should deploy a maintenance and inspection schedule of mechanical presses. 7) Employers should ensure employees are thoroughly trained on machines they operate. 8) Employers should conduct Job Hazard Analyses (JHAs) for specific tasks and instruct employees on how to safely work with and troubleshoot machinery issues during normal production operations. 6) Employers should train employees in Lock-out Tag-out (LOTO) procedures. For the past month, the contractor and his son along with three other members of their painting crew had been painting the exterior of a two-story church that had a 60-foot steeple. It was a windy day. The wind was blowing at 15 to 30 mph with gusts up to 40 mph. They had finished painting for the day and were cleaning up the site. The contractor and his son were moving the aluminum extension ladder that was at its full extension of 48 feet. The contractor and his son held the ladder in the vertical position as his son attempted to retract the ladder's extension. A gust of wind blew the ladder onto a 14,460-volt overhead power line. Electrical current traveled from the power line through the conductive aluminum ladder. Both were electrocuted. The contractor died at the scene and his son died nearly a month later. Investigators found: 1) Measures were not in place to protect against the ladder contacting the energized power line. 2) No account was taken of windy conditions when handling the ladder. 3) The distance from the power line to the face of the building was 10 feet 8 inches. REQUIREMENTS: If work is to be performed near overhead lines, the employer must deenergize and ground the lines, or they must provide other protective measures before work is started. If the lines are to be deenergized, the employer must make arrangements with the person or organization that operates or controls the electric circuits involved to deenergize and ground them. If using protective measures, such as guarding, isolating, or insulating, these precautions must prevent employees from contacting such lines directly with any part of their body or indirectly through conductive materials, tools, or equipment. Employers must use ladders with nonconductive side rails where the ladder could contact uninsulated, energized electric lines or equipment. RECOMMENDATIONS FACE investigators concluded that, to help prevent similar occurrences: Identify the location of overhead power lines as part of all initial worksite surveys for jobs involving the use of ladders. Note power line heights and distances from work areas on site diagrams and provide information to site supervisors and workers. Perform a job hazard analysis of the worksite. Use non-conductive ladders around power lines. Lower the extension ladder and transport it horizontally. Be aware of windy conditions while moving a ladder near power lines. They did not de-energize the panel before starting the job because they believed it was not required. Also, the customer was not required to have a disconnect ahead of the panel. The nearest disconnect was off site at the public utility transformer. They had done similar jobs in the past, and this panel had been previously inspected and approved by an electrical inspector. The apprentice was in the process of removing the heat shrink from the panel hardware when his side cutters slipped and contacted the b and c phases simultaneously, causing the line to fault. He suffered severe arc flash burns to his face and hands. He was not wearing arc flash protective clothing, gloves, or a face shield and his side cutters were not insulated. His coworker rushed him to the hospital. Investigators found: 1) A hazard assessment had not been done prior to starting the task. 2) The employer was unaware of the requirement to de-energize the panel before working on it. 3) The employer did not provide and ensure the apprentice used the proper personal protective equipment PPE). 4) The employer had their electrical installations inspected several times previously but had never been inspected for safe work practices. REQUIREMENTS-Employers must: 1. Not permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means. 3. Provide and ensure workers use appropriate PPE for electrical work. 4. Train electricians in the electrical safety work practices for construction and general industry. RECOMMENDATIONS-FACE investigators concluded that, to help prevent similar occurrences: 1. Before beginning a job, take steps to control hazardous energy instead of relying on work experience to protect workers. Always de-energize equipment and panels when possible before starting work. 2. Always use appropriate tools and PPE when working on electrical equipment, even when systems are de-energized. The national worker fatality rate in 2019 was 3.5 per 100,000 full-time equivalent workers. The agriculture industry had the highest number of fatalities, followed by construction and transportation. These three industries have been among the top four industries for fatalities in each year in the past decade. For events, fatal cases from transportation (motor vehicle) exceeded all other events, followed by slip/strips/falls, contact with objects, and incidents involving equipment (non-motor vehicle). Motor vehicle incidents have the highest number of cumulative events through 2019. He was working with two others installing roof trusses on a single-story home. His employer was a subcontractor. This was his first day on the jobsite and he was hired two days before. In the past, he had worked on other construction jobs. The crew arrived on site in the morning and it rained early in the day. The employer did not hold a safety meeting prior to starting the job. The laborer used an extension ladder to climb onto the 2" x 6" top plate of a framed wall. He was standing on the top plate guiding roof trusses into position while his coworker was on the ground supporting the trusses. Shortly after 3 p.m. they were positioning a truss when, according to the coworker, the laborer fell backward from the top plate and landed 9'7" on the dirt below. He was not wearing fall protection or a hard hat. The other coworker was cleaning up the site and did not witness the fall. After he fell, the coworker administered CPR and yelled to the other worker to call 911. He was able to revive the laborer before he was airlifted to the hospital. He died the next day due to blunt force head injuries suffered in his fall. Investigators found: 1) The employer had not held a safety meeting or conducted a walk-around safety inspection prior to the start of the job. 2) The employer had fall protection on site, but the laborer was not wearing it. 3) The laborer was not required to use fall protection because the top plate was not a walking/working surface. Fall protection is required at a height of 10' or more when engaged in the erection or placement of structural members, including trusses. 4) The first-aid certification of the worker who administered CPR had expired. REQUIREMENTS: Employers must 1. Hold and document crew leader-crew safety meetings and walk-around safety inspections at the beginning of each job, and at least weekly thereafter. See WAC 296-155-110 RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: 1. Before beginning a job, perform a worksite hazard analysis to identify fall protection needs and ensure systems are in place to protect workers. 2. Do not work while standing on top plates. They are not walking/working surfaces. Use mobile elevating work platforms (MEWPs), scaffolds, or platform or step ladders. 3. Always use fall protection when exposed to fall hazards and when working in MEWPs, regardless of height. A fall can be fatal from any height. 4. Do not tie off directly to wood trusses unless a qualified person has determined that the truss or series of trusses will meet the strength requirements of a fall-arrest anchor. On the day of the incident, the driver was delivering a load of 1,500-pound bales of recycled cardboard to a paper production mill. The bales were loaded into his semi-trailer at his employer's yard where they had been stored. The bales were stacked three-high and were aligned with the center of the trailer. The trailer was fully loaded. When the driver arrived near the loading dock at the delivery site, he first opened the trailer door on the passenger side and chained it to the side of the trailer to keep it open. He then opened the trailer door on the driver's side. As he opened the trailer door on the driver's side, a stack of bales became unstable and two of the bales fell out of the trailer. He was struck by a 1,500- pound bale that came to rest on him. According to the coroner, the driver died of blunt force injuries of the torso. Investigators found that the employer provided straps to prevent bales from tipping over and falling out when drivers opened the trailer doors, but the drivers did not use them. Additionally, the semi-trailer was parked on uneven ground, which may have destabilized the stack of bales. REQUIREMENTS: 1. Employers must make sure all loads transported on trucks and trailers are properly secured and distributed. 2. All Commercial Motor Vehicles (CMV)s must comply with the requirements of 49 CFR 392.9 (Subpart A)- Inspection of cargo, cargo securement devices and systems, and 49 CFR 393 Subpart I - Protection Against Shifting and Falling Cargo. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: Employers should 1. Ensure that drivers are protected from the hazard of falling bales. 2. Ensure that when bales are loaded into trailers that they are properly secured and will not fall over during transport or when drivers open the trailer doors. 3. Provide and require drivers to use cargo securement devices such as straps. 4. Ensure that drivers park truck and trailer are on level ground before opening trailer doors. 5. Ensure that drivers unlock and open one trailer door at a time and shield themselves from falling bales by standing behind the door. The man had suffered blunt trauma to his head and neck and was pronounced dead at a local trauma center. CONTRIBUTING FACTORS Key contributing factors identified in this investigation include: 1. Accessible areas with low height clearance AB - 2. Absence of a protective cab on the sweeper AB - and 3. Lack of a comprehensive safety and health program. RECOMMENDATIONS The Massachusetts FACE Program concluded that, to help prevent similar occurrences, employers should: 1. Ensure that ride-on equipment without a protective cab are not operated in the vicinity of overhead obstructions. 2. Ensure that a job hazard analysis is performed for each task assigned to employees. 3. Develop and implement a comprehensive safety and health program that addresses hazard recognition, avoidance of unsafe conditions, and proper use of equipment. In addition, equipment manufacturers should: 1. Adopt and implement the concept of Prevention through Design (PtD) to identify potential hazards associated with equipment and then eliminate these hazards through design changes.
