Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

He had been helping the farm owner, his friend, with the dredging operation intermittently over several days and they were nearly finished when the incident occurred. The victim had just moved the crane onto a slip of land between two ponds to clear a channel between them. After he finished this work he dropped the bucket into to dry pond to dredge out some additional mud and debris that he was unable to reach from a previous position on the shoreline. As he began to bring in the bucket the crane began to fall onto its right side. The victim jumped from the operator's cab and was crushed by the carriage mount of the crane. The owner believes that the victim would have survived if he had remained in the cab. The victim died before he could be extricated from under the toppled crane. Recommendations: 1. Know the proper use and limitations of the machinery you are operating. Operators of heavy equipment must have appropriate training and/or licenses before operating the equipment. 2. Proper set-up is essential to safe crane operation. Always deploy outriggers before operating the crane. Understand the center of gravity of the load being lifted and avoid side or end pulls. 3. The building was positioned on three dollies, two steer dollies at the "rear" of the building and one dolly at the "front" of the building that hooked to the tow vehicle. The lineman dropped the neutral wire from the pole and left the 14,400-volt electrical lines energized. The lineman was present in an aerial bucket positioned on the road shoulder to observe building clearance while the building was being moved. Approximately 75 feet of the building had proceeded under the lines when two employees assigned to the "rear" steer dollies went under the building and began to ratchet each dolly chain to steer the rear of the building onto the road. (See Figure 1). Near the front of the building a "bolt of lightning" was observed as the building antenna contacted the line. The two employees at the "rear" steer dollies received electrical shocks and fell to the ground. Bystander CPR was initiated, and emergency response was called. One of the workers was taken to the hospital and survived, the other worker was pronounced dead at the scene. Recommendations: 1. Employers should verify that a minimum of 10 feet is maintained by employees when working near an energized electrical line, gear or equipment exposed to contact with an energized overhead power line. 2. Develop a building assessment standard operating procedure that includes roof protrusions regardless of building height as part of a company specific Accident Prevention Plan that is in compliance with MIOSHA requirements. 3. Stress and routinely review the hazards regarding overhead power lines so that all employees are cognizant of these energized sources. 4. Select personal protective equipment and other equipment insulating materials to provide operator protection from inadvertent contact with electricity. 5. Companies should not use skip boards to raise electrical or other overhead lines. 6. Additionally, MIFACE recommends that electric utility companies review their policy for communicating with individuals prior to work that will be performed within the vicinity of an energized overhead power line and the documentation procedures of this communication. Once released from the forklift to the top deck of the truck, the unsecured car immediately rolled backward. The victim was struck in the back of the head and knocked down as the front end of the car fell to the main deck on top of him. His injuries were immediately fatal. The local police, fire and medical examiner were called to the scene. The medical examiner determined the incident to be an industrial accident and pronounced the victim dead at the scene. Recommendations: 1. Secure loads to the forklift, and be sure the load is completely secured in the transporter before removing the forklift. 2. A forklift operator must be sure that other workers are clear before releasing a load, particularly if direct line of sight is obscured. 3. Ensure effective communication between loading partners. 4. Employees who demonstrate loss of proficiency should be retrained. The die set had been purchased from the company from which the forging company had previously outsourced this operation. The forging company did not purchase the press in which the die set was used. The victim installed the die set in the "bender" press AB - a full revolution press (See Figure 1). The "bender" was selected because the bed size of the "bender" was appropriate for the die set. One rubber boot trim run had been performed, but the boot was not completely trimmed AB - hand trimming was necessary. The victim placed a piece of solid, cylindrical steel, measuring 4" long, 1 7/8" diameter on the front edge of the die stripping plate to act as a stop block. The victim activated the "bender" by pressing the dual palm buttons without removing the steel cylinder. He may have sat down in front of the press on a stool. When the ram came down, it contacted the cylindrical piece of steel. When the ram contacted the steel, it caused the stripping plate to unevenly depress at an angle toward the victim instead of horizontally. This caused the piece of steel to be forcefully ejected from the press and strike the victim in the chest. The co-owner working in a different shop area heard two loud noises, turned, and saw the victim fall to the ground. Another worker called 911 while the co-owner initiated CPR. Emergency response arrived and transported the victim to a local hospital where he died. Recommendations: 1. Install an electrically integrated safety block and barrier guard to permit safe work operations. 2. Conduct a task analysis and develop task-specific safe work procedures when beginning new work to identify potential safety issues and solutions. 3. Ensure 2-hand trip buttons are the appropriate distance from the work operation. 4. Although not a factor in this death, MIFACE recommends that after a new operator is assigned to a press, employers complement the 8 hours of press operator training recommended by Federal OSHA by extending the period of time the new operator works under the close supervision of an experienced operator to enable the new operator to understand the interaction between the press, die, and product. The following are hazards that these truck drivers may have faced on a daily basis, but didn't live to tell about it. Don't let it happen to you or your drivers. 1. Excessive Speed: Speed Kills - drive at the posted speed limit and take into account weather and road conditions. 2. Tailgating: Use the 3 second rule - there should be at least a 3 second gap between your vehicle and the one in front of you (this should be increased as the conditions indicate). 3. Driver Inattention: It is estimated that at least 25% of all vehicle crashes are caused by driver inattention. Keep your eyes on the road and hands on the wheel. 4. Load Stability: Keep you load within legal height and weight limits. It could cost you your life and that of others. 5. Equipment Failure: Inspect your vehicle prior to driving - It's the law. 6. Bulkhead Guard: Don't drive a log truck without a bulkhead guard (cab guard or headache rack) - they're there for a reason. 7. load and unload your truck and trailer. 8. Driver Fatigue: Stay awake - Stay alive. Investigators thought that fatigue may have contributed to a quarter of the log truck crashes. 9. Seatbelts: Buckle-up. Seat belts save lives! They left behind eight wives, eight children, and an untold number of grandchildren who will never again see, hear, or hold their loved ones. The monetary cost of those lost lives can't compare to the cost to the families and society. The purpose of this FACE Fatal Facts is to help prevent future log-hauling deaths. Of the 11 fatal incidents, six involved the truck leaving the roadway for various reasons, two were roadway collisions, and two were during the loading or unloading process. The truck was designed to carry a steel open top box that could be dropped off at a desired location. The victim arrived at a site to drop-off an empty container and pick-up a full container. He backed the truck into the home's driveway and dropped the empty roll-off box near the full container. After placing the empty container in the driveway the homeowner gave the victim a payment check, talked to the victim for a few minutes and then went back inside his house. The victim entered the truck and drove it forward with the rails on which the containers slid still elevated. After driving into the street and moving to the left he backed the truck into the driveway until the rails contacted the full container. He exited the cab and apparently noticed a leak in a hose to one of the hydraulic cylinders that raise and lower the rails. He stepped to a hydraulic reservoir tank located behind the cab and reached behind it with his right hand to close a hydraulic shut-off valve. While he reached to close the valve the rails lowered and he was pinned between a hydraulic ram and the edge of the reservoir tank. About 5-10 minutes after the homeowner had talked to the victim he noticed the truck was still in the driveway. He went outside and discovered the driver pinned against the hydraulic tank. He returned to the house and told his wife to call emergency personnel. Rescue personnel arrived a short time later, removed the victim and pronounced him dead at the scene. MN FACE investigators concluded that in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. employers should emphasize during safety training that employees should never approach a machine that has experienced any type of failure until an assessment has been done by a qualified person to insure that workers can safely approach it AB - and 2. machine and equipment manufacturers should ensure that all operating controls and devices are located in areas that workers can access without having to place themselves in a hazardous position. On the day of the incident, the victim, a co-worker (an assistant engineer) and a student intern drove to a landfill to replace a battery of a flow meter that had been placed in a manhole. Once they arrived at the site, the victim opened the manhole cover with a pickaxe. The manhole was 7'4" deep and 24" in diameter at the point of entry. There were four iron rungs mounted into the cement wall of the manhole to form a ladder. The flow meter was attached to the top rung that was 34 inches below the manhole opening by a "U" shaped spring loaded handle. The victim used a hook made of a wire hanger to catch a string that was looped and tied around the handle of the flow meter. When he was pulling and lifting the meter, the weight of the flow meter caused the wire hook to straighten and the meter fell to the bottom of the manhole. The victim quickly descended into the manhole to retrieve the meter. Once at the bottom, the victim picked up and placed the flow meter on the top rung. Just as he was about to ascend, he lost consciousness and collapsed in the bottom of the manhole. The assistant engineer immediately called "911" on his cell phone. The fire department arrived at the site and immediately started the confined space rescue procedure. The victim was extricated from the manhole in approximately 20 minutes. He was transported to a nearby hospital where he was pronounced dead. According to the fire department monitoring data, the oxygen concentration at the bottom of the manhole was 2.1% and the flammable vapors exceeded 60% of the lower explosive level (LEL) at the time of the rescue. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to prevent similar incidents from occurring in the future, employers should: 1. Implement a confined space entry program for all workers who are or could be exposed to confined space hazards AB - 2. Provide immediate training and periodic refresher training to all employees who may be exposed to confined space hazards AB - 3. Evaluate the sewer flow monitoring procedure and modify it to reduce workers' risk AB - 4. The farm owner was called, and upon finding no signs of life, the farm owner called 911. The emergency squad responded promptly, but was unable to resuscitate the farm worker. The victim was transported to the nearest major trauma hospital where he was subsequently pronounced dead. Bureau of Occupational Health AB - On May 13, 2002, a 52 year-old male machine operator (the victim), who was employed by a copper manufacturing company, sustained fatal crushing injuries as a result of being caught by an ingoing pinch point on a hot tin coating machine (tinning line). The pinch point was created by a moving metal strip that was to be coated and a steel bridle roller that tensioned the strip on the tinning line. At the time of the incident, the victim was one of two second-shift operators on the tinning line. A new coil of brass strip had just been loaded onto the machine and the tinning line was running at a speed of 50 feet per minute (FPM). There were no witnesses to the incident. It appeared that the victim's left glove became caught between a bridle roller and the moving brass strip while he was wiping the bridle roller with a piece of sand paper. He was pulled into the line and sustained crushing injuries to his head. At approximately 8:30 PM, the other second-shift operator noticed that the brass strip was out of alignment. He followed the brass strip to the end of the line and found the victim caught and crushed by the bridle roller and the brass strip. The operator immediately hit the emergency stop button to shut down the line and notified the shift supervisor. The shift supervisor phoned the plant EMT, who arrived at the scene within minutes. A state police investigator also responded to and secured the site. The medical examiner was called and pronounced the victim dead at the scene. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, employers should: 1. Train and closely supervise employees in standard machinery cleaning procedures and strictly enforce adherence to these procedures AB - 2. The victim turned off the mixer for the spray liner and walked to a side pedestrian door, which was open and had a small portable fan placed to provide air circulation for the general shop area. He disconnected his airline from the respirator and proceeded to walk around the outside of the building to the front of the store. A co-worker who had helped him set up the job was waiting in the basement apartment. When the coworker came upstairs to the shop reception area, he saw the victim kneeling outside in front of the store in respiratory distress. (See Figure 1) The coworker took the victim to a nearby urgent care facility. The victim lost consciousness and stopped breathing while at the urgent care facility. Emergency response was called while CPR was administered. The ambulance took the victim to the hospital emergency room where he was declared dead. Recommendations: 1. When spraying isocyanate-containing material, employers should: 2. Provide a ventilated spray booth or room and evaluate the effectiveness of the ventilation. 3. Establish a MIOSHA compliant written respiratory protection program and require a supplied-air full facemask respirator to minimize employee exposure. 4. Develop, implement and maintain a written hazard communication program and train employees about the program and chemicals they work with. 5. Institute medical monitoring of employees exposed to sensitizers or other asthma-causing agents. 6. Conduct a workplace hazard assessment to identify health and safety issues, types of personal protective equipment to be used, and standard operating procedures to permit safe work. 7. Additionally, manufacturers/suppliers/distributors should emphasize the health and safety aspects for their products when conducting training about their product at end user worksites. The victim was placing large rocks on the banks to prevent erosion and sediment accumulation in the pond. Immediately before the incident a wheelbarrow load of rocks had been delivered to the victim. Following the incident, the wheelbarrow was found lying on its side with the rocks strewn down the slope, towards the victim's position. According to the employer, the victim may have slipped at the water's edge and lost his footing while attempting to avoid being struck by falling rocks from the wheelbarrow. As the victim was falling into the pond he reportedly reached out and grabbed the hand of his coworker and they were both pulled into the water. The coworker attempted to locate the victim, who was now under the water, but was unsuccessful. The employer stated that neither employee knew how to swim. The coworker managed to get out of the pond and went to a nearby house on the property to call 911. Local Fire and Rescue units arrived on scene but were unable to immediately locate the victim. Another fire district, with a dive team, located and recovered the body 47 minutes after the original 911 call. Resuscitation efforts were attempted but were unsuccessful and the victim was pronounced dead at the scene by the Deputy County Medical Examiner. 1. Special precautions should be observed when working on or near the edge of the water. Grab lines, life preservers or rings, fall prevention or poles should be strategically situated or worn for immediate use. Workers should be instructed in basic water safety or lifesaving skills and should wear approved personal flotation devices. 2. A competent person should inspect and evaluate the work site to identify hazards and ensure that workers receive appropriate instructions in safe work procedures and the use of personal protective equipment. 3. When working at a location that is more than 3-5 minutes response time for emergency medical services, workers should be trained in CPR, first aid, and basic emergency response skills. 4. Consider alternative tools or equipment if work practices or the environment make standard equipment choices impractical or unsafe. Approximately 20 minutes later, a coworker entered the area and noticed that the grinder was running unattended. He then called out to the victim and received no response. After looking into the mixing/grinding area of the machine he realized that the victim had fallen into the operating grinder and immediately stopped the machine. Emergency Medical Service (EMS) was summoned via 911 and responded within minutes. The coroner pronounced the victim dead on site. A loose wire inside the portable auger's electric motor caused the metal auger housing which laid across the bin's access door frame to become energized. When the victim grabbed hold of the frame and stepped backward out of the bin, his foot made contact with the ground, creating a completed circuit, causing the electrocution. The victim was pronounced dead at the scene. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences, farmers should: 1. Ensure all electrical wiring and fixtures, both permanent and temporary are in good repair. 2. Ensure that all electrical wiring and fixtures (outlets, extension cords, pull boxes) are of the type best suited for the environment in which they will be used. 3. Ensure that adequate means of grounding are provided for all structures and equipment. 4. Consider installing either permanent or portable Ground Fault Circuit Interrupters (GFCIs). The framing crew was busy setting and nailing joists into place when they heard a sound like that of a "nail gun falling and hitting something". They went to investigate and discovered the victim, at the bottom of the shaft, seriously injured. At the time of the incident, the shaft opening was not covered according to witnesses. Shaft opening now covered and guarded. A 911 call summoned a fire and rescue team who found that the victim had expired, and a deputy medical examiner was called to the scene. Recommendations 1. Employers should design and use comprehensive fall-protection programs to reduce the risk of serious or fatal injuries. 2. An unprotected opening, side or edge which is 6 feet or more above a lower level should be protected by the use of a guardrail system, safety net system, or personal fall arrest system. 3. Establish routine follow up to assure hazard correction or abatement. 4. In remote or obscure locations, make sure that workers know their location, and can provide directions to emergency responders, if needed. The decedent's wife found him after 9:00 p.m. that evening and reported the incident to local authorities. The victim was pronounced dead at the scene. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to prevent similar occurrences, farm equipment owners/operators should: 1. Follow manufacturer's recommendations and safe work practices when operating farm equipment. 2. Inspect work areas and equipment before performing a task to identify actual and potential hazards and determine methods of control. 3. Ensure that personnel maintaining or servicing farm equipment are trained, competent to perform the required work, and practice proper farm safety procedures. A crew of six men were installing guardrail at the incident site. One of the men was seated in the rear operator station of the truck-mounted post driver and was attempting to back the truck and move the guardrail horizontally when the truck suddenly jerked and traveled backward. Coworkers positioned in front of the truck saw the truck jerk, then saw the guardrail hang up briefly on a post, flip up into the air, and strike two coworkers (the victims) who were placing bolts in a previously installed guardrail section approximately 25 to 35 feet behind the truck. The truck came to a stop in the traffic lane on the opposite side of the road. After stopping the truck, the operator used his cell phone to call 911. Emergency Medical Services (EMS) and police personnel responded within minutes. EMS personnel provided emergency care to the 20-year-old victim, who was then transported by medical helicopter to a regional trauma center where he was pronounced dead later that day. The 33-year- old victim was pronounced dead at the scene by the county coroner.
