Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

A call was placed for emergency assistance and personnel from the police department responded to the scene within minutes. The victim was transported to a local hospital where he was pronounced dead. Provide supervision for all employees, which should include but not be limited to, regular communication between employers and employees about safe work conditions. 2. He did not pull down and shut the safety gate before he pushed the "down" button to start baling. As the ram, a hydraulically-driven flat plate that exerted pressure on the material to be baled, started its down stroke, the victim suddenly climbed on the lower chamber door and extended his entire body into the baling chamber. He was crushed by the ram. Two associates working in the area stopped the baler and called 911. The EMS crew arrived at the site within minutes. The victim was transported to a local hospital where he died later the same day. The post-incident baler examination found that the safety interlock had been bypassed, allowing the machine to operate with the safety gate open. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, employers should: 1. Inspect all baling and compacting machines periodically to ensure that all safety features are functioning properly AB - 2. On the morning of the incident, the crew had closed two lanes of a three-lane highway and had left only one southbound driving lane open to all traffic, both northbound and southbound. Warning signs were placed ahead of the work zone at each end. The victim, who wore a reflective vest and a hard hat, was directing the southbound traffic with a "Stop/Slow" sign at the north end of the work zone. At approximately 9:05 a.m., a black pickup truck suddenly pulled out of the southbound traffic into the passing lane and accelerated to approximately 60 miles per hour (mph) into the work zone. The victim was struck by the vehicle and thrown in the air by the impact. He landed on the northbound shoulder approximately 30 feet from the collision point. The emergency squad and the New York State Police (NYSP) arrived at the incident site within minutes. The victim was transported by helicopter to a hospital trauma center where he died thirteen days after the incident from the injuries sustained in the collision. The driver of the pickup truck was arrested for multiple traffic violations. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, employers should: 1. Consider the use of law enforcement officers in cruisers at each end of highway work zones and the use of radar surveillance for traffic speed control AB - 2. A mechanic who responded determined that there was not a mechanical problem so he called the Electrical & Instrumentation (E&I) Technicians. Three E&I Technicians responded and began to troubleshoot the wrapper. While troubleshooting the system, the wrapper was left in its operational mode, a normal procedure since it would be nearly impossible to determine the problem if the system were shut down. After troubleshooting the wrapper, it was suggested that it be cleared of all rolls of paper and the system be shut down and rebooted to reset it. While the E&I Technicians were troubleshooting the system, operations personnel were reviewing the lockout/tagout procedures for the wrapper. After the system was rebooted, one roll of paper was transferred to the first of two roll stop positions. The roll stop again failed to release the roll. Without notifying any of his co-workers, the victim went to the side of the wrapper opposite the computer control room. He crawled under the wrapper platform apparently to look for a switch called the ejector down switch. The switch senses the position of the wrapper's two roll stops and either allows or prevents rolls from moving across the wrapper platform. A diagram in the computer control room clearly indicated that the switch was located outside the wrapper frame. A steel plate that activates the ejector down switch is located beneath and near the center of the wrapper. The victim apparently proceeded to look in the area underneath and near the center of the wrapper. While he attempted to locate the switch, he was eventually guided by following the switch arm to the switch location on the outside of the wrapper frame. After locating the switch, he put his upper body through an opening in the side of the wrapper such that his torso was above the roll wrapper frame and directly below a raised control arm for one of the roll stoppers. A co-worker who happened to walk around the wrapper saw the victim at the moment he reached up and struck the switch with his fist. When the switch was struck, the wrapper immediately cycled. When the wrapper started, the roll of paper was released and it rolled forward to the raised bumper that was above the victim. The bumper descended as the roll came against it and crushed the victim between the wrapper frame and the bumper arm. A call was immediately made to emergency personnel who responded to the scene. The victim's co-workers immediately began to try to free the victim. He was eventually freed and removed however he had sustained traumatic crushing injures and was pronounced dead at the scene. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. employers should ensure that employees access restricted "areas" only by means of interlocking safety doors or gates. 2. Employers should ensure that employees shut down and lockout/tagout equipment before they enter any restricted hazardous areas. 3. Employers should securely install permanent barriers on all non-essential equipment "openings" that may pose a hazard risk to employees, and AB - 4. Since the bulk of the weight of the tractor was in the rear, the tractor flipped back and over, and landed on the victim, crushing him. There were no witnesses to the incident. NJ FACE investigators recommend following the following safety guidelines to prevent similar incidents: 1. Alternative vehicles should be utilized for maintaining a motocross track. 2. A post-incident baler examination found that the electrical wiring had been altered to bypass the "On or Off" switch, and the gate safety switch had been short-circuited by a piece of two-inch wire. By doing so, power was always supplied to the machine and the baling machine could operate with the safety gate up. HOW CAN THIS BE PREVENTED? Employees should: Inspect all baling machines periodically to ensure safety features are working properly. Develop, implement, and enforce a baling machine safety program. Provide training and ensure management and other employees know and understand the importance of baler safety features, such as the safety gate and lockout/tagout program, and how they work. Ensure authorized baler operators follow standard safe operating procedures. Follow the manufacturer's recommended schedule for baling machine maintenance. The 21-year-old, second-year apprentice was employed by a construction company to work on a single-story building to be used as an annex to a county sheriff's office. The victim was standing on the top plate of the building's exterior wall getting ready to lay plywood sheeting on the roof trusses. He was not using fall protection. He reached for a sheet of 5/8 inch 4'x 8' plywood from the 46-sheet stack of plywood being held in place by two wood bunks nailed to the 3/12-pitch roof trusses. The movement of the plywood caused one of the bunks to collapse. The plywood sheets began to slide toward the victim and pushed him off the top plate causing him to fall 11 feet to the ground where he landed on his back. Twenty-nine sheets of plywood cascaded down, many of them edgewise, onto his chest and body. First aid was performed on the victim, before he was transported to a hospital, where he later died. At this point the victim was walking next to the forklift's left rear wheel, which was a few feet from a stack of panelized roofing insulation. The forklift had rear-wheel steering so when the forklift turned right, the back-end of the forklift swung left and the rear wheel caught the victim's foot, causing him to pitch forward under the wheel. When the forklift continued to move forward, it ran over the victim's left leg and torso. The victim died at the scene. The decedent was using a rubber-tired front-end loader and backhoe for lighting and to transport the concrete saws to the maintenance building. The backhoe, which did not have a boom swing lock pin installed, was left running to maintain a charged battery. After completing the work, the victim laid a handheld portable concrete saw on the floor of the backhoe through the opening in the rear. The saw was placed on the right boom swing pedal, causing the boom to swing to the side, pinning the victim between the boom arm and the rear of the backhoe. The decedent was found later that morning by the superintendent and pronounced dead at a local hospital. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to prevent similar occurrences, employers should: 1. Ensure that machine guarding is in place, and that any additional safeguards needed to eliminate hazards are utilized prior to equipment operation. 2. Ensure that all employees receive documented training on existing and potential hazards on or around mobile and stationary equipment. 3. Ensure that special work procedures are in place for all employees working alone, including training on any unique hazards that exist. 4. He was filling in ruts on a dirt road close to the edge of a bluff by his field and planned to cut up a tree that had fallen across the road. He was using a new Case Model 40XT skid steer loader that he had owned about two months. The victim's father came to the farm and realized the victim had not returned home. He took the tractor up to the field to check on the victim and found the skid steer loader lying on its side over the embankment. The victim was lying beside it. The father returned to the house and notified the Emergency Medical System. The sheriff was dispatched within 20 minutes and the emergency personnel were present when he arrived. They detected no signs of life and contacted the County Coroner. The Coroner received the call and pronounced the victim dead at the scene. The seat belt on the skid steer loader was not fastened and the safety restraint bar was up. The chainsaw the victim was going to use to cut the tree was located about 15 yards from the top of the hill lying in the vicinity of the path taken by the skid steer loader. The FACE investigator concluded that to help prevent similar occurrences, farmers should: 1. Always fasten the seat belt and keep the operator's restraint bar down at all times when operating a skid steer loader. 2. Walk around the work area before starting to work and look for hazards. Use extreme caution when operating machinery on or near sloped terrain or embankments and consider doing work by hand where machine use may be too hazardous. 3. Read and follow safe operating procedures for skid steer loaders recommended by the Equipment Manufacturer's Institute (EMI). The co-worker called for emergency services located about 14 miles away. The victim was pronounced dead at the scene. The spreader was transported to a facility where the body could be extricated. FACE investigators concluded that, to help prevent similar occurrences, farmers and employers should: 1. Remain seated on the tractor while the spreader is in operation. 2. Stop all augers before operators attempt to clean, maintain or work close to the auger. 3. Ensure that employees are trained in and are aware of the potential hazards associated with operating farm machinery by reviewing the operator's manual and other safety precautions intended for specific machinery. Shut off the engine and lower the bucket before boarding, exiting or performing maintenance repairs. 2. Inspect machines before use and ensure that they are operating properly. Equipment with any malfunction found during inspection should be taken out of service. 3. Remain seated in the operator's compartment while operating the machine. 4. Operate the loader with the seat belt fastened and restraint bars down in place. 5. Work with another person when possible, while operating heavy equipment. The tractor, which was not equipped with a cab or rollover protection structure (ROPS), was left running in neutral. The farm owner went to the field to investigate when the victim did not return to the farmyard as expected. He found the victim lying in blood under the drawbar of the manure spreader and immediately called for emergency services. EMS responders and the sheriff arrived within seven minutes and pulled the victim from beneath the manure spreader. The coroner was called, and pronounced the victim dead at the scene. The FACE investigator concluded that to help prevent similar occurrences, farmers should: 1. Only use tractors that are fully equipped with an operator restraint system and rollover protective structures (ROPS). 2. Seek and follow medical advice and take precautions to prevent falls if they have medical conditions that could cause loss of balance. Three construction workers were involved in this incident: the primary operator of the chip spreading machine AB - a secondary side-gate or front hopper operator (the victim) AB - and the dump truck driver. The victim had worked with this equipment 2 previous summers. It was the first day of road resurfacing at this worksite and everything was considered routine. The chip spreading machine was moving slowly forward on the roadway, pulling the rear-facing dump truck behind it. While the equipment was in operation, the side-gate operator (victim) got off the rock-chip spreading machine and went into an adjacent wooded area. He returned to his work station at the front right of the chip spreader and began adjusting the gate levers. Subsequently, he fell in front of the chip spreading machine and was run over first by the chip spreader and then by the left rear axle of the dump truck, at which time the dump truck became uncoupled from the chip spreader's hitch. The driver got out of his truck to investigate. He found the victim lying between the first and second sets of duals on the driver's side rear axles and moved the truck forward. The victim died at the scene from severe head and neck injuries. RECOMMENDATIONS based on our investigation are as follows: 1. Employers must educate and instruct each employee in the recognition and avoidance of unsafe work conditions and applicable regulations associated with their work environment to control or eliminate any hazard(s) or other exposure to illness or injury. 2. When two people are required to simultaneously perform operational tasks their work must be coordinated, which requires effective communication. 3. Manufacturers of chip spreaders should employ engineering controls to reduce excess noise and designs that do not require an operator other than the primary operator. The lift arms were not in alignment with the vehicle chassis when the vehicle was raised just prior to the incident, because the vehicle was too long for the lift to accommodate. He was working with his son, using the front bucket of his tractor to push a wooden fence post into the ground. The men were working in an area of pasture which had a slight slope to the right front of the tractor. The son was holding the 6-inch post upright, while his father on the tractor positioned the front bucket, half-filled with dirt, on top of the post. They were using the hydraulics of the tractor to push and/or ram the post into the ground, a procedure successfully completed with "thousands of fence posts" on their farm without apparent problems. However, that day the ground was fairly dry, and the post did not penetrate the soil more than six inches. Suddenly, the bucket slipped off the post, and the bucket snapped downward with sufficient force and momentum that it caused the rear of the tractor to lift up. The wide-front tractor then rolled over on its right side down the slope, stopping momentarily, then continuing to roll until it had righted itself again. The farmer was fatally crushed as the tractor rolled over, and was found on the ground to the left of the machine. There was no ROPS on the tractor. Recommendations based on our investigation are as follows: 1. All tractors should be equipped with Roll Over Protective Structures. 2. Factory installed Roll-Over Protective Structures (ROPS) should not be removed from tractors. 3. Alternate methods of installing fence posts should be considered. The workers had just stopped their utility truck on the shoulder of the road and had walked to the rear of the truck to get their equipment. At that time, a motorist was witnessed driving erratically down the highway. As he approached the crew, he drove his Volvo sedan onto the shoulder of the highway, striking the victim and sideswiping the truck. The force of the impact severed the victim's leg and threw him into the back of the utility truck. His co-worker, an experienced Emergency Medical Technician (EMT), was uninjured and tried to help the victim, who died at the scene. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Mobile highway work crews should use truck-mounted traffic attenuators and sign boards as per the USDOT Manual for Uniform Traffic Control Devices. 2. Highway authorities should consider the use of automated machines for the routine filling of potholes. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. He then turned away from the truck to light the cigarette as the truck started moving and turning at the intersection. As the truck turned, the side of the truck's bed struck the victim, knocking him to the ground. He was conscious as he was transported to the local emergency room, where he died of his injuries later that day. NJ FACE investigators recommend following these safety guidelines to prevent further incidents: 1. Employers should set up roadway work zones as per the USDOT Manual on Uniform Traffic Control Devices. 2. Employers should develop an internal traffic control plan for the safe operation of vehicles within the work zone. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers.
