Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

At the time of the incident, the victim's cousin was operating the tractor and wagon and the victim was on the ground near the wagon. The forage wagon was connected to the tractor's drawbar and its power-take-off shaft. When the tractor's PTO drive mechanism was engaged, it operated the wagon's unloading mechanism including two unloading beaters mounted across the front of the wagon. The beaters broke apart the forage in the wagon as two "conveyors" apron chains that were also driven by the tractor's PTO shaft moved it to the front of the wagon. While the victim and his cousin were unloading hay from the wagon, clumps of hay lodged near the front of the wagon. Before the victim's cousin could stop the tractor's PTO, the victim climbed on the front of the wagon to dislodge the clumps of hay. He placed one or both of his legs inside the front end of the wagon in an attempt to kick free the clogged hay. When he did, the legs of his pants became entangled in the beaters. The tractor operator noticed the victim becoming entangled in the beaters, stopped the tractor's PTO and rushed to the victim. He saw that the victim was caught between the beaters so he ran to others at the farm and notified them of the incident. A 911 call was placed to emergency personnel who soon arrived at the scene. They assisted in removing the victim from the beaters. After the victim was removed, he was taken to a nearby medical facility where he was pronounced dead. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. Working youth should only be assigned age appropriate tasks. 2. Operators should, whenever possible disengage the power-take-off before dismounting from a tractor, and, 3. Workers should not wear loose-fitting clothing near or while operating machines. These machines created a great deal of residual heat, so management ordered the installation of fans to exhaust the heat from the room. The two employees were working from a scissor lift to do the "non-live" installation of the wiring for the fans, which would later be inspected, connected, and energized by a licensed electrician. As the workers ran conduit along the ceiling of the room, they approached a partially-exposed, 480-volt, three-phase electrical bus bar that supplied power to the thermoforming machines. The victim reportedly used a voltmeter in the scissor lift to test the exposed electrical conductors at the uncovered end of the bus bar. He apparently connected the voltmeter across two of the phases, which overloaded the meter and caused it to explode. The explosion ignited the victim's clothing and caused an electrical breaker to trip, plunging the area into darkness. The victim's co-worker managed to lower the lift, but his own clothing ignited while trying to extinguish the victim's burning clothing. Another employee put out the fires with a fire extinguisher. The victim was taken to the area burn unit with burns over 35% of his body, where he died of complications 14 days later. NJ FACE investigators recommend following these guidelines to prevent similar incidents: 1. Employers should permit only properly trained and qualified persons to carry out electrical work. 2. The company should develop, implement, and enforce an electrical safety program. 3. A qualified person should inspect work areas prior to permitting employees to work near electrical or other hazardous equipment. 4. Employers should ensure that all personnel lifts are properly maintained and inspected. The crew of five workers started at the site in the morning to work on the interior and to excavate a passage for the stairs. Shortly after arriving, the foreman at the site was called away by the owner of the company. The foreman instructed one of the carpenters on how to use the excavator before he left. The excavator operator/carpenter and a helper worked in the driveway, while the remaining two carpenters worked in the basement. At approximately 11:00 a.m., one of the carpenters came out of the basement to deposit some trash in a dumpster. The excavator operator saw and warned the carpenter (victim) before moving the excavator to the dumpster. He did not see the victim, who was struck on the head as he walked in the narrow driveway. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Excavator operators must be thoroughly trained in the safe operation of the machine and demonstrate proficiency before operating machinery. 2. Hazardous work sites should be closely supervised and marked out before operating heavy equipment. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. 4. The employer should develop, implement, and enforce a comprehensive safety training program in the languages and literacy levels of the employees. The force of the impact caused a sheave (cable pulley guide) to sheer off from the side of the delimber's boom. The falling sheave struck the chaser on the head, and punched through his hard hat. Life Flight was summoned. The victim was pronounced dead at the scene shortly after emergency help arrived. Recommendations: 1. Plan the landing site for yarding requirements beforehand. Make sure nearby anchor stumps are left at an appropriate height to secure guylines, and the guylines will not interfere with decking the logs at the landing. 2. Set up yarder guylines according to manufacturer's recommendations, making sure lines are at no more than a 50-degree angle, equally sharing the load, and secured by additional support for the anchors. 3. Check integrity of the guyline anchors daily, before and during operation. 4. Maintain proper deflection in the skyline to reduce stress on the yarder tower. He was cutting scuppers, or water drains for downspouts, in the roof. His battery powered saw lost its charge and he walked to another part of the roof to get a new battery. After retrieving the battery the victim turned and took two steps and fell through an unguarded, uncovered 44"x 44" skylight opening. He fell 15 feet 8 inches and landed on an interior stairway. Members of the local fire department gave first aid and he was transported to a hospital where he died twenty-five days later from severe neck and head injuries. The deceased had driven the tractor into the field located behind and next to the back yard of his family home to retrieve a trailer to haul wood. The trailer became stuck in the mud. The victim connected a log chain to the trailer and tried to pull it out of the mud with the Ford tractor, Model 8 N. This model has widely-spaced front wheels. Figure 1 shows the tractor after it overturned backwards. Figure 2 shows the front of the tractor facing the trailer. The victim's friend was driving past the property on the road and saw the tires of the tractor in the air. The friend turned around on the road and drove into the yard. He went to the house to see if his friend (the victim) was there, but no one answered as he called for him. He then ran out to the tractor in the field and found the victim pinned underneath the tractor. He drove to a neighbor's home to get someone to call for help. He drove back to the victim's home. In the meantime, the victim's sister who had heard the friend's truck but was unable to answer the door at the time, went out and saw the tractor upside down. She ran over and saw the victim under the tractor. She ran back into the house and called emergency medical services (EMS). Within fourteen minutes the emergency medical personnel had arrived from a nearby town and had removed the victim from under the tractor. He was taken to a local hospital where he was pronounced dead. The NIOSH FACE investigator concluded that, to help prevent similar occurrences, farmers and employers should: 1. Provide tractors equipped with rollover protective structures (ROPS) and seat belts. 2. Ensure that youth are trained in and are aware of the potential hazards associated with operating farm machinery through a formal tractor safety-training program intended for youth. 3. Ensure that the tractor is equipped with an appropriate hitch before using it to pull equipment. 4. Ensure that youth are informed that they should seek help whenever situations occur for which they lack experience or skill. 5. Know and comply with Federal and State child labor laws that are related to employment of youth in an agricultural setting. The tractor had a tricycle-type front axle with a front-end loader mounted to its frame but did not have a rollover protective structure (ROPS). He salvaged an open, metal tank of the kind typically used for watering livestock, lifted it with the loader and steered the tractor back down the steep trail. The tank repeatedly came free from the loader. In the process of reloading the tank, then backing the tractor to reset his direction on the trail, the tractor's rear wheels spun in reverse gear and slipped off the trail's edge. The tractor-loader combination overturned, coming to rest on its top against a small tree. The tractor operator was pinned between the tractor and the tree with the lift arm of the loader frame across his abdomen. His brothers who worked with the victim in their scrap metal salvaging enterprise found him hours later, after midnight. Emergency medical services were called and arrived within ten minutes. No life signs were detected and it was determined the victim died at the scene. RECOMMENDATIONS based on our investigation are as follows: 1. All tractors used with front-end loaders should be equipped with a ROPS and seat belt. 2. ROPS should be designed for all tractor makes and models in common, productive use and owners should have ROPS installed or retire these tractors from active use. 3. Tricycle-type, narrow-front tractors should not be equipped with front-end loaders. Tractors equipped with a front-end loader should be used with special caution, and preferably only on level ground, have a wide front axle, rear wheels set as wide as possible, and adequate counter weights installed. 4. Tractor operators should be aware of the overturn hazard and how to reduce the hazard and associated risk, including securing loads and ballasting with counterweight. 5. Operators should use extreme caution when operating tractors on or near steeply sloping terrain and on pathways with ditches or embankments. 6. A communication plan should be established and implemented when the work to be done includes working alone. This crane was comprised of a horizontal steel I-beam mounted on two vertical A-frame supports bolted to the concrete floor. A maintenance crew was assigned to move the 10-foot-high, 28-foot-long, 1,058-pound crane a short distance to a new location. The crew unbolted the crane and set one end on a wheeled dolly. A forklift truck supported the opposite end. The victim had planned to control the dolly as the forklift pushed the crane. As the forklift truck started to move, the top-heavy crane toppled over, striking the victim who apparently tried to stop its fall. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Employers should service cranes and other equipment as per manufacturers' directions. 2. Employees should be kept at a safe position and distance from hazardous operations. 3. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. The Gradall had a safety interlock that prevented starting from the ignition switch while in gear. The contractor was apparently unaware of this safety feature. He checked the batteries, and then crawled underneath the cab area and reached up into the engine compartment with a screwdriver. The screwdriver made contact between the two terminals on the starter, effectively jump-starting the engine and bypassing the safety mechanism that prevented ignition while in gear. The Gradall started and moved forward. The parking brake was not set. The back left tire rolled over the contractor. Recommendations: 1. Powered industrial forklifts should only be operated by knowledgeable and experienced individuals certified by the employer, with proficiency demonstrated through operator training. 2. Before exiting any powered industrial truck, even briefly, completely shut down power, place controls in neutral and apply parking brake. 3. Do not start equipment by overriding safety features. Assisting the truck driver to tie down the load, the operator backed the forklift to the opposite side of the trailer, put the automatic transmission into neutral, engaged the parking brake, and jumped out to take the strap thrown across the load by the truck driver. The forklift's engine was left running. While the operator was facing the trailer and busy with the strap, the forklift engaged in reverse and backed into him, crushing him against the trailer with enough force to make the trailer shudder. The truck driver looked under the trailer and saw the victim's legs dangling in the air. He ran to engage the gearshift on the forklift into forward to release the victim. Efforts to revive the victim were unsuccessful, and he was pronounced dead at the scene. Recommendations: 1. Before exiting any powered industrial truck, even briefly, completely shut down power, place controls in neutral and apply parking brake. 2. Employers should provide ongoing evaluation and feedback for operators of mobile machinery to ensure that operator proficiency and safe work practices are maintained. 3. Maintain the machine in safe operating condition. 4. Employees should be encouraged to report concerns related to the safe operation and maintenance of mobile machinery. The container of weed spray was attached to the back of the seat of the tractor. The tractor was not equipped with a cab or rollover protection structure (ROPS). When her husband did not return home, the victim's wife became worried and contacted neighbors and then the sheriff's department. Having no success in locating the victim, the helicopter from a nearby hospital was contacted. The emergency personnel arrived in the helicopter equipped with spotlights to join the search. The tractor was located upside-down in tall weeds down the incline of a hill. The victim was found pinned underneath the tractor. Emergency medical persons were present when the victim was located and they examined the victim. No signs of life were found. The medical examiner was contacted and pronounced the victim dead at the scene. The FACE investigator concluded that to help prevent similar occurrences, farmers should: 1. Only use tractors that are fully equipped with an operator restraint system and rollover protective structures (ROPS). 2. Wear a seat belt when operating a tractor equipped with a ROPS. 3. Use extreme caution when operating tractors on or near sloped terrain or embankments. 4. Use caution when pulling or carrying any equipment attached to the tractor. The victim, coworker, and foreman were working at an electrical substation, which serves a town of approximately 3,500. The city was in the process of switching their electrical service over from a three-phase 4 kV system to a 12 kV system. The victim and co-worker were on the steel framework of the substation tightening up nuts and bolts on the conductors they had installed. The victim worked his way over to the incident point where he sat down on the structure next to the energized lightning arrestor. He then contacted the arrestor with his left hand and forearm. He fell backward breaking contact with the electricity. The co-worker, foreman, and a city worker climbed up to the victim and immediately started CPR. Emergency personnel were summoned to the scene. The victim was transported to a local hospital where he was pronounced dead. The purpose of the FACE Program is to identify risk factors that contribute to worker injury and death, and to make recommendations to employers and individuals on how similar events can be avoided. From the information collected about this incident, the MO FACE investigator concluded that employers should: 1. ensure that all energized conductors are covered by line hoses or line blankets while employees are working in the vicinity of the energized conductors AB - 2. ensure that comprehensive hazardous energy control measures, including proper lockout/tagout procedures, are fully implemented and enforced. The crane tipped up onto the outriggers twice before overturning completely onto its side. The operator tried to escape from the tipping crane but became pinned by the crane as it tipped to its side. Workers used two other near-by cranes and EMS used inflatable pillows to lift the overturned crane off of the operator. He was taken to a local trauma center where he died a short time later. The MO FACE investigator concluded that, to prevent similar occurrences, all employers should: 1. ensure that cranes are operated within their limits of stability as determined by the manufacturer-supplied capacity charts AB - 2. ensure that the weights of objects lifted by a crane be determined by a competent person, as defined by OSHA, and the weight be posted clearly and visibly on the object prior to being lifted AB - 3. develop, implement, and enforce a comprehensive written safety program which includes, but is not limited to, strict conformance with the equipment manufacturer's recommended safe operating procedures for crane set-up and lift configurations AB - 4. ensure that all cranes are equipped with load moment indicators and maintain those indicators according to the manufacturer's specifications. The boy was returning from a pasture with the tractor and a homemade two-wheeled round-hay-bale trailer attached. He was operating an Allis-Chalmers WD, tricycle-front-end tractor on well-maintained gravel county road. He and his father had just dropped two round hay bales in their farm pasture. The father was driving a pick-up truck and had driven ahead of the son on the county road as they were returning from the pasture. The accident was not witnessed, but apparently the farm-boy lost control of the tractor as it neared a drainage ditch and culvert. The tractor veered off of the left side of the road and overturned into the ditch. The MO FACE investigator recommends that to prevent similar occurrences all farmers and employers should: 1. Provide tractors equipped with rollover protective structures (ROPS) and seat belts for use by all persons trained and certified to operate tractors. 2. Ensure that workers are trained in and are aware of the potential hazards associated with operating farm machinery. The victim had arrived at the site during a heavy rainstorm, exited the vehicle, and accessed the roof from a low hanging eave. The barn was built against a slope so the eave of the roof on the uphill side of the barn was about 2 1/2 feet above ground level. The victim began to walk up the slope of the roof with a measuring tape trailing behind and a notebook and pencil in hand. As he reached an area of about 16 feet above the eave the roof gave way and he fell through the roof landing on the concrete floor thirty feet below. A witness went immediately to his aid but was unable to find a pulse. The witness went to the farmhouse and called 911 to notify the local emergency medical services unit (EMS) which responded a short time later and pronounced the victim dead at the scene. The Maryland FACE investigator suggests that, to prevent similar occurrences employers should: 1. perform a hazard assessment prior to employees performing any work activities at elevations above six feet to identify the safest feasible means to assure task completion. 2. develop, implement, and enforce a safety and health program that addresses safe work procedures and hazard recognition. The skidder operator yelled a warning and the victim began running away from the skidder. He ran directly into the path of the falling tree and was struck on the head and shoulders. The co-workers came to the aid of the victim but he died within minutes at the scene. The purpose of the FACE Program is to identify risk factors that contribute to worker injury and death, and to make recommendations to employers and individuals on how similar events can be avoided. From the information collected about this incident, the MO FACE investigator concluded that employers should: 1. ensure that employees are trained in hazard awareness and proper escape procedures AB - 2. develop, implement and enforce a written safety program which includes, but is not limited to, worker training in hazard identification and avoidance. Division of Labor and Industry AB - A 47-year-old male construction foreman (the victim) was crushed by the crawler treads of an excavator that was moving into position to dig a trench across a two-lane road. The victim had given directions to the operator regarding placement of the excavator. After speaking with the operator, the victim walked around to the opposite corner of the excavator. The victim, facing away from the excavator, leaned over to move an orange safety cone. The west track of the excavator caught the victim's leg and he fell to the ground face first into the path of the excavator. A witness ran toward the operator shouting to stop the machine and move backward off the victim. Another member of the crew radioed the company to notify 911. Paramedics arrived within minutes of the call. The victim was flown by helicopter to the University of Maryland Shock Trauma Center where he died from his injuries approximately 45 minutes after the incident. The Maryland FACE investigator suggests that to prevent similar incidents: 1. Employers should ensure that employees are instructed to stand clear of heavy equipment that is maneuvering in a work zone. 2. Employers should ensure that heavy equipment operators who must maneuver equipment with limited visibility in the direction of travel utilize an employee to signal the operator from a safe distance. 3. Employers should ensure that heavy equipment operators make every effort to establish a clear view in the direction of travel before maneuvering a piece of equipment. 4. Heavy equipment and other vehicles should be, when feasible, equipped with devices for visualizing or sensing the presence of humans or obstacles that are in the path of travel. As his body passed between two steel drive rollers and the paper rolls he was crushed by the paper product and machinery components weighing in excess of 7000 pounds. His body was discharged into the interior of the machine. The victim was pronounced dead at the scene. The Maryland FACE investigator suggests that to prevent similar occurrences: 1. Employers should install guarding on all machinery to prevent employees from coming in contact with rotating parts. 2. Employers should develop, implement, and enforce written safe work procedures that separate the employee from exposure to hazards that could result in injury or death. 3. Operations that must be performed near moving machinery should be automated or designed to eliminate the risk of employee injury. Durante el turno de trabajo nocturno, la víctima entró en la máquina mientras los otros siete trabajadores, todos hispanos, se encontraban fuera del área inmediata. Cuando los compañeros de trabajo regresaron, no pudieron encontrar a la víctima en el área de trabajo de la planta. Uno de ellos se fijó en la máquina y encontró a la víctima adentro. Llamó al 911 y luego llamó al gerente de la planta a su casa. El personal del servicio médico de emergencias (EMS, por sus siglas en inglés) llegó al lugar dentro de los 7 minutos. Cuando llegó el gerente de la planta, apagó y bloqueó la fuente externa de energía al densificador. El personal del EMS, que había ingresado a la máquina a través de una puerta lateral luego de quitar los pernos fijos, retiró los restos de la víctima. Los investigadores de NIOSH concluyeron que, para ayudar a prevenir incidentes similares, los empleadores deben: 1.
