Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim climbed inside the machine with a brass chisel and a hammer to try and cut the plastic product off the mold. While inside the machine, reportedly with the gate open, the machine cycled bringing the dies together crushing the victim. Coworkers notified Emergency Medical Services (EMS). EMS responded to the incident site within minutes and pronounced the victim dead at the scene. The Medical Examiner's Office was notified and arrived to remove the victim's body. The Massachusetts Fatality Assessment and Control Evaluation (FACE) Program concluded that to prevent similar occurrences in the future, employers should: 1. Develop, implement, and enforce a comprehensive hazardous energy control program including a lockout/tagout procedure and routinely review and update the program and training. 2. Ensure that their comprehensive safety program includes training on hazard recognition and the avoidance of unsafe work practices and conditions by conducting a job safety analysis (JSA). The American National Standards Institute (ANSI) should: 3. Consider revising standard ANSI/SPI B151 to require all plastic injection molding machines, regardless of size, to have an additional safety device to protect workers from the moving parts in the mold processing area. While standing between the drill control console and the exterior wall, the victim drilled four holes into the ground. While attempting to drill the fifth hole, it appears that the victim inadvertently activated the controls causing the boom to extend and pin him between the control console and the wall. Hearing the engine of the track drill racing, a laborer found the victim unconscious and pinned against the wall. The laborer ran to get a track hoe operator for assistance. After arriving back at the track drill, the track hoe operator and the laborer ran to get another track hoe operator who knew how to operate the track drill. The first track hoe operator then ran to notify the foreman, while the second track hoe operator backed the track drill off of the victim. The foreman called 911 and then checked the victim. Approximately 3 minutes later, Emergency Medical Services (EMS) responded and determined that the victim's chest had been crushed and he was not breathing. The victim was transported by ambulance to an area hospital where he was pronounced dead. The plant supervisor asked the victim if the old wash plant could operate while he worked on the electrical installation at the new plant. The victim indicated that the old plant could be operated. The victim then went to the motor control center where he accessed the 480 volt electrical panel which supplied power to the old wash plant and was to be the power source for the new wash plant. Shortly after 9 a.m. the power went off at the old wash plant. An employee went to the motor control center to investigate and found the victim leaning against the energized 480 volt components of the main breaker panel. Emergency rescue personnel were contacted and arrived to find the motor control center on fire. The fire was extinguished and the victim was pronounced dead. The cause of death was electrocution. An investigation determined that power to the electrical panel had not been shut off. The decedent was removing aluminum siding while standing on an aluminum ladder on the west side of a house he and his two coworkers were remodeling. The ladder was positioned over a cyclone gate and fence of a neighboring home. It appears that while he was removing the siding, he lost his balance and fell. One of his coworkers, who was carrying debris to the dumpster on east side of the home, heard the decedent moan. He returned to the decedent's work location and found the decedent's midsection folded over the gate. The decedent's head and legs were not touching the ground. His midsection was lying across the end cap of the gate, which was about two inches higher than the gate's top bar. He was also lying across a small nut and bolt, which was about one inch above the top bar. The ladder the decedent was working from was standing against the house, still positioned over the gate. The decedent rolled off the gate and landed on the ground. The decedent told his coworker to call 911. His coworker panicked and began to yell for help. A neighbor heard the calls for help and called 911. Passersby provided emergency first aid until emergency response arrived. The victim was transported to a local hospital where he was declared dead. Recommendations: 1. Construction employers should consider using scaffolds or other work platforms to work from instead of ladders. 2. Employers should be familiar with safe ladder use and train their employees to follow safe ladder use procedures. 3. Employers should ensure that at least one person on the jobsite is certified in First Aid/CPR, should strongly consider having an individual certified as a Medical First Responder or Emergency Medical Technician (EMT), and hold at least semi-annual workplace rescue/first aid practices. 4. Homeowners should inquire whether their potential building/remodeling contractors are licensed to perform the contracted work and licensed builders should offer to present their pocket card with their license number. Occupational Health Service AB - On October 2, 2005, an 18-year-old groundman for a small tree trimming company was killed when he was struck by a falling section of tree. The incident occurred at a private residence where the company had been hired to remove four trees and to prune a fifth. The employer hired two workers as groundmen to assist him in doing the ground work as he cut down the trees. The victim, who was referred to the employer by another employee, was working his first day as a groundman with the company. His training that day was on-the-job, and included warnings from the employer to stay clear when he was working in the trees. At approximately 4:00 p.m., the employer had climbed to the top of a 50-foot-high sweetgum tree, his last tree of the day. He had already removed the branches and was cutting the tree in sections, beginning at the top and working his way down. The employer had just made a cut with his chainsaw and was pushing a section clear when he saw the victim walking on the ground beneath him. He shouted a warning and the victim tried to run clear, but the tree section struck him on the head. The victim was airlifted to the regional trauma center where he died of his injuries approximately seven hours later. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. All employers and employees involved in tree work should receive training in arborist methods. 2. Employees should use proper tree removal and communication methods as outlined in the ANSI Standard Z133.1-2000. 3. The employer should assess each job for safety hazards and design a work plan that addresses methods of dealing with the hazards. This article describes the methods used to measure gas concentrations and the results obtained from a pilot study by the Washington Fatality Assessment and Control Evaluation (FACE) program of two oxygen-limiting forage tower silos in eastern Washington State. The silos were monitored for oxygen (O2), nitrogen dioxide (NO2), and carbon dioxide (CO2). Measured O2 concentrations were low in all areas of the headspaces monitored for both silos. After beginning forced-air ventilation, O2 concentrations within the silo headspace returned to ambient levels within 8 to 20 min. Nitrogen dioxide levels exceeded the Washington Industrial Safety and Health Act (WISHA) short-term exposure limit (STEL) in the silo that was filled six days earlier (silo 2), but not in the silo that was filled four days earlier (silo 1). The NO2 concentration in silo 2 decreased to below the WISHA STEL within 15 min of starting ventilation but began to rise shortly after ventilation was stopped. Carbon dioxide, which was only measured in silo 1, was detected at 2% within the headspace of this silo. The carbon dioxide concentration decreased to <1% within 3 min of starting ventilation. Due to the severe winter cold, the employees regularly drained a recycled water supply line, which fed the generator, to keep it from freezing. With the failure of a critical check valve, other valves in the open position, and no water pressure in the line, this open pipe created a pathway from the acetylene generator to the lime shed, where the drain valve remained open. The acetylene collected in the shed and was most likely ignited by the hot surface of a sealed space heater inside the shed. The explosion completely destroyed the shed, killing and injuring the victims and severely damaged the adjoining buildings. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Follow the recommendations issued by Federal OSHA and the US Chemical Safety and Hazard Investigative Board. 2. Develop procedures and thoroughly train personnel in operating the acetylene generation system. 3. Develop electronic monitoring systems for critical processes and valves. 4. Ensure that process pipes are properly designed to prevent freezing, provided with proper venting, and equipped with redundant valves for critical applications. 5. Use proper ventilation and an acetylene detection system to prevent and monitor for dangerous gas concentrations. 6. Conduct a job hazard analysis of all work activities with the participation of the workers. They were preparing to secure a steel beam into place that was being supported by a second forklift and wooden post. The steel beam consisted of two 20-foot long sections welded together. The middle of the beam rested on the forks of the second forklift 11 feet, 6 inches above the ground. As the victim and his coworker were preparing to bolt the beam into place, the forklift operator raised the beam to shift it to the south. As he did that, the steel beam came off the south support post and tipped over. The forklift's hydraulic side shift was malfunctioning, but it is unknown whether that contributed to the injury. The beam struck the decedent in the head and knocked his coworker off the platform. A call was immediately placed for emergency response. The decedent was taken to the nearest hospital by emergency medical services (EMS) and then airlifted to another hospital where he was pronounced dead later that same day. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to help prevent similar occurrences, employers should: 1. Ensure that employees do not position themselves under elevated loads AB - 2. Ensure that employees wear hard hats when working in areas where there is a potential for head injury from falling or flying objects and fall protection when working at elevations of six feet or more AB - 3. Ensure that forklift operators elevate personnel only with approved lifting cages that are properly attached to machinery AB - 4. The decedent was mowing an unevenly sloped area between the property's driveway and a bean field with a rotary field mower, commonly known as a brush hog, attached behind the tractor. It appears that while mowing the slope, either the tractor wheel and/or the brush hog struck a stump, causing the tractor and brush hog to turn over to the side. The tractor was not equipped with a rollover protection structure (ROPS)/seat belt. The decedent's wife left the home that morning at 7:30 a.m. and returned home at about 9:00 p.m. Her husband was not home, and she assumed that he went to the county fair. She awoke at 3:00 a.m. and noticed he had left his wallet. She immediately began searching the property and finally found him under the tractor. She called 911. Emergency response arrived and he was declared dead at the scene. Recommendations: 1. Retrofit older tractors with properly designed, manufactured and installed rollover protective structure (ROPS) and seat belt when available. 2. Even if an older tractor has been retrofitted with a ROPS, owners of older (including classic restored) tractors should evaluate the tractor's capability to perform the tasks and evaluate the risks of using such tractors. 3. When trees are cut down in areas where farm equipment and tractors will travel, the stumps should be cut as close to the ground as possible, removed, or ground down. 4. Ensure accessory equipment is properly sized to the tractor. 5. Practice safe work procedures when operating tractors and/or tractors and accessory equipment on slopes or where there may be unseen obstacles. The decedent, who was the on-site supervisor, and his crew had delimbed the smaller branches and were in the process of removing the three remaining major limbs. One limb grew over the pedestrian sidewalk toward a home, one limb grew almost straight up in the air, and one limb grew over the street. The crew decided to remove the limb that grew over the pedestrian sidewalk because it was near the home. The decedent was responsible for the friction rope used to lower the cut limb to the ground. The crew had successfully made three cuts to this limb AB - the cut limb fell away from the remaining limb, struck the tree trunk, and then was lowered via the friction rope to the stake truck below. The incident occurred on the fourth cut. The decedent was standing approximately 25 feet away from the tree in the street and under the tree limb that grew over the street. When the tree limb being cut struck the trunk, it may have caused the tree itself to vibrate, which may have caused the limb growing almost straight up to break at a weak spot. The fully barked, 8-inch diameter center limb broke at its center section due to a knothole that had rotted out. The knothole had a bird/squirrel nest covering it. The center limb section fell and struck the decedent. He was wearing all required protective equipment - hardhat, safety vest, and safety glasses. He died approximately two weeks after the incident. Recommendations: 1. Employers should review tree-trimming activities and determine if existing rigging methods could be updated with existing technology. 2. Employers should ensure that crew sizes are large enough to enable a site supervisor to effectively perform their safety-related activities. 3. Employers should standardize tree trimming safe work practices, such as prohibiting an employee to work under the drip line of a tree and sounding dead tree limbs with an axe handle. EMS personnel arrived at the site to attend to the victim who was pronounced dead at the airport. The Massachusetts Fatality Assessment and Control Evaluation (FACE) Program concluded that to prevent similar occurrences in the future, employers should: 1. Purchase and use deicing equipment that does not obstruct the view of the vehicle operator while driving. 2. Designate pedestrian walkways within the airport apron for ground crew. 3. Prohibit employees from wearing loose hoods while working around moving vehicles and equipment on airport aprons. 4. Supply and ensure that employees wear appropriate personal protective equipment, such as the American National Standard Institute (ANSI) compliant high visibility safety apparel. 5. Ensure that their comprehensive written health and safety program includes specific training for deicing operators and workers on foot regarding areas around vehicles and equipment where operators would have obstructed views. Also, employers using deicing vehicles where operators have obstructed views while driving should: 6. Require a second employee to assist the vehicle operator during driving of the vehicle. 7. Consider installing after market devices (e.g., camera, radar, and sonar) on vehicles and equipment to help monitor the presence of workers on foot. In addition, manufacturers of aircraft deicing equipment and vehicles should: 8. Design equipment such that the operator's view is not obstructed while driving. 9. Explore the possibility of incorporating new monitoring technology (e.g., radio frequency identification (RFID) tags and tag readers) on equipment to help monitor the presence of workers on foot and in blind areas. The oval tee box was on a plateau with three gently sloping sides except for the east-side bank, which dropped off 19 feet with a 49-degree slope into trees. The deceased had made three passes in a diagonal motion across the tee box, turning at the edge of the tee green on the collar (longer grass area) after each pass. When he reached the collar area, he raised the reels, turned the mower, and then engaged the reels down. After this third cut, the incident occurred. The back wheel of the mower went over the edge of the collar, and the mower began to slide down the slope. The mower hit a small tree, and the mower overturned. The mower came to rest on top of the deceased at the base of the slope against some trees. A coworker found him under the mower. 911 was called and emergency personnel arrived. They pronounced him dead at the scene. Recommendations: 1. Golf course superintendents should conduct a full, hole-by-hole vehicle rollover risk assessment, measuring slopes and drop heights, in order to determine the best method of machine operation and machinery to be used. 2. Course designers should provide adequate clearance for mowers to operate safely on the perimeter of an elevated tee or green. 3. MIOSHA should consider updating the General Industry Safety Standard Powered Groundskeeping Standard, Part 54 to reflect the updated ANSI B71.4-2004 safety specification standard for commercial turf care equipment. 4. MIOSHA/OSHA should consider requiring a certified operator protection system (such as rollover protection structure (ROPS)/seatbelt) for groundskeeping equipment that could be operated in a location where a rollover potential exists. 5. Riding mower manufacturers are encouraged to develop a ROPS/seatbelt retrofit attachment for existing equipment under 20 HP. 6. Employee training should include review of written material, such as the machine operator's manual. Additionally, MIFACE recommends that: 7. All workers on the course, when working alone, should be equipped with a company-supplied communication device. Ensure workers are properly trained and supervised. Exercise caution when using jacks to support vehicles or equipment. Use lifts or maintenance pits, instead of portable jacks, when possible. Ensure all vehicles/equipment with a potential to fall are properly jacked, supported with jack stands, and chocked prior to getting under them. Follow manufacturer's instructions for safe lifting and securing. Do not place jack stands under portions of vehicles that are not fixed in place. Ensure all tools and equipment, including jack stands, have been tested, certified, and rated for the work that is being done. Exercise caution when working on vehicles on inclined or uneven surfaces. Always chock the vehicle's wheels. Ensure the vehicle's transmission is set to the park position and set the vehicle's parking brake, where possible. Ensure the ground is sturdy enough to support the vehicle's weight, jack or block. Do not conduct work if safe working conditions cannot be met. Repair trucks in a shop equipped for large vehicles, if at all possible. The victim prepared the sheet steel to be wound on the cylinder and then gave the machine operator the signal to start the machine. The victim was clear of the machine at that time. The machine jammed, and when the operator went to find the cause, he found the victim caught in between the rolled steel sheet on the rewind cylinder. No one witnessed the actual incident. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure employee safety by installing guards around the moving parts and pinch points of machinery to protect employees from accidental contact. 2. Ensure employees do not operate machinery without proper guarding. 3. Ensure employees are properly trained on machine operation and safety, and their achievement of skills is verified through a testing program. 4. Periodically reassess job assignment hazards. 5. Establish work policies that comply with employment standards for youth less than 18 years of age in nonagricultural employment. Fatality Assessment and Control Evaluation (FACE) Program AB - On January 30, 2006, a 37-year-old male Hispanic carpenter's helper (the victim) was fatally injured while assisting in disassembling a lattice boom on a truck-mounted crane. The crane operator and the victim were working together removing pins that secured the 40-foot center section to the base boom section. After dislodging the second bottom pin, the boom hinged on the top two pins and fell on the victim's back pinning him to the ground. The supervisor summoned a coworker who contacted 911 and company managers immediately. A hydraulic crane located nearby was brought to the site and used to lift the boom off of the victim. Approximately 5 minutes after receiving the 911 call, emergency medical services (EMS) personnel arrived and extricated the victim. They attempted resuscitation but were unsuccessful. EMS personnel notified the EMS medical director of the victim's condition, and resuscitation efforts were suspended. The victim was transported to the hospital where he was pronounced dead about 45 minutes after the incident. The traffic control crew had re-arranged the work zone for a new paving operation, and the victim was picking up some extra cones in the work zone. The victim was facing away from an asphalt dump truck that was traveling in reverse towards the paver. The driver felt a "bump" and saw some cones tumbling into the road. He stopped, pulled forward and parked the truck. The driver and a traffic control worker found the victim lying face down on the ground moaning while clutching some traffic cones. They yelled for someone to call 911. Approximately 4 minutes later, Emergency Medical Services (EMS) arrived on scene, assessed the victim and found he had multiple injuries. The victim was transported via ambulance to an area hospital, where he was later pronounced dead. His employer was a high voltage utility contractor working as a subcontractor for an electric utility company on a project to replace underground cable. At about 10:15 PM a four person line crew, including the victim, was working on a section of underground primary cable in a concrete vault. The workers had tested and grounded all the old cables but not the three new cables located at the bottom of the electrical vault. They removed the old high voltage cables and modules and had begun to install new high voltage modules. During this procedure the employees discovered that one of the primary 7,200 volt underground elbows needed to be rotated 180 degrees in order to be installed on the new high voltage module. The victim used a probe wrench to rotate the elbow and while doing so made contact with an energized 7,200 volt cable. The victim was electrocuted and died at the scene. A second employee was blown clear and received injuries for which he was hospitalized. An investigation of the incident determined that 1) the primary underground cables were never tested and grounded; 2) the crew assumed that the new primary underground wires were numbered correctly and that the cable numbers went to the correct underground vault location; 3) the three-phase transformer bank in the vault that had the primary underground conductor replaced the previous week was never checked to see if it was re-energized after the scheduled power outage. Methods: Kentucky self-employed worker injury fatalities were identified using Fatality Assessment and Control Evaluation (FACE) program data (1995-2004) and compared to non self-employed worker data. Occupations and industries listed on death certificates were compared to those in which the decedent was actually engaged. Results: Of 1,281 Kentucky worker injury deaths, 28% were self-employed. Death certificates failed to identify 31% of these deaths as work-related; industry and occupation were incorrectly identified in 27% and 16%, respectively. Fifty-seven percent of the deaths were in agriculture, primarily tractor-related. For Kentucky, the self-employed crude death rate was higher (27.6/100,000) than the non self-employed worker (5.4/100,000) rate or the US (11.5/100,000) self-employed rate. Conclusions: Multiple information sources improve identification of self-employed status in work-related injury fatalities. Effective prevention requires accurate surveillance and examination of contributing factors. Self-employed worker injuries in high-risk industries should be more fully examined for development of effective injury prevention programs.
