Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The dump truck speed was 2-3 miles per hour (mph) and the distance backed was approximately 30 yards when the incident occurred. The back-up alarm was working and the truck had adjustable mirrors on both sides of the cab. The victim was wearing a high visibility vest and hard hat at the time of the incident. The CA/FACE investigator determined that in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure that workers who are on foot stay out of the work area where heavy equipment is operating, and in clear view of operators. 2. Ensure that when visual contact is lost with workers on foot, drivers of trucks and heavy equipment stop and do not resume work until visual contact is re-established. 3. Minimize the distance trucks need to back up in order to gain access to the work area. 4. Consider using additional safety devices for trucks and heavy equipment to warn workers of a backing vehicle and to warn drivers when someone is in their blind spots. 5. Consider educating employees on the concept of teamwork in safety as part of the documented safety meeting program. Shortly thereafter the son walked between the buildings of the farmstead, which blocked full view of the feedlot, on his way to join his father. He noticed several cows milling around near a building and gate at the southwest corner of the feedlot. He rounded the corner of the building and discovered his father lying in the springtime mud among the cows, against a cluster of dead tree stumps, midway along the building's east wall. As the son moved in to help his father, the bull approached forcing him to scramble back under the gate and around the corner of the building. The son then summoned emergency assistance. Although the bull had retreated to another part of the feedlot out of sight, rescuers remained anxious and watchful as they attended to the unresponsive victim, who had no pulse or spontaneous respiration as they prepared him for transport to the local hospital. The bull, which remained agitated throughout, was shot and killed by local law enforcement personnel with additional assistance from a local rifleman. Recommendations: 1. Keep a safe distance from bulls, watch them carefully when with them in open yards, and learn to identify aggressive postures bulls exhibit before they attack. 2. Install protective obstacles and "person-gates", and plan escape routes from open cattle yards. 3. Cull aggressive bulls from the herd and, in general, do not keep bulls past 2 years old. 4. Protect rescue personnel by creating an effective barrier between the bull and the victim. Although un-witnessed, it appears that the victim entered the machine's hoist area to adjust the position of a pallet when the hoist began to rise with the victim on top of the pallet. A co-worker heard the victim yelling for help and ran to press one of the machine's emergency stop buttons. The victim was crushed between the empty pallet and the top portion of the hoist before the hoist stopped rising. The co-worker manually lowered the palletizer's hoist and the victim fell to the floor below. The co-worker in training was also an Emergency Medical Technician (EMT) and attended to the victim while Emergency Medical Services (EMS) were notified. When EMS arrived at the incident site, the victim was breathing on his own but was unconscious and unresponsive. EMS transported the victim to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Minimize the number of times palletizers cycles are interrupted because of failures to detect pallets in the hoist area by routinely: (A) inspecting pallets for cracks and broken pieces, and, (B) evaluating the mechanism that transports pallets into the hoist area to ensure pallets are loaded into the hoist area properly AB - 2. Ensure that accessible moving parts of machines are guarded at all times to minimize access by employees AB - and 3. Develop, implement, and enforce a comprehensive hazardous energy control program including a lockout/tagout procedure and routinely review and update the program and training. In addition, palletizing system manufacturers should explore the feasibility of adding sensors to detect unplanned obstructions during the hoist's movement which would stop the hoist when obstructions are detected. The fuel building housed both the pit concessions and the fuel storage and dispensing area. The decedent had filled several plastic fuel containers for the racers at the racetrack. It is unclear if he was filling another container, or was transferring methanol from a 55-gallon drum to a 1,000-gallon methanol holding tank when the explosion occurred. Although on fire, he was able to exit from the burning building. A racecar driver who was also a firefighter wrapped a shirt around the decedent's face and pulled him away from the burning building. Emergency response personnel who were already on site provided medical assistance and transport to a local hospital. He sustained burns over 80 percent of his body. He died the next day at the hospital. Michigan Occupational Safety and Health Act (MIOSHA) personnel found that there were no grounding straps used and that the wiring was not rated for the area. Recommendations: 1. Racetrack owners should ensure that flammable material use and storage is in compliance with applicable safety standards and NFPA recommendations. 2. Racetrack owners should train employees who dispense/transfer racing fuel to do so in a manner that will minimize the production of static electricity. 3. Racetrack owners should develop, implement and enforce a comprehensive health and safety program that includes, a job hazard analysis and employee training in hazard recognition and avoidance. The safety program should include provisions for fuel distribution personal to use fire rated clothing. 4. Racetrack owners should contact MIOSHA Consultation, Education and Training (CET) or other safety and health professional to conduct an inspection of the racetrack to identify potential hazards and racetrack compliance with applicable health and safety laws. 5. Racetrack owners should comply with NFPA 610, Guide for Emergency Response and Safety at Motorsports Venues standard. 6. Racetrack owners should contact their local jurisdiction's building inspector to review any changes in existing building use. After speaking with the decedent and giving instructions to the excavator operator he had hired, the site contractor left the location. The excavator hired by the site contractor was operating a Caterpillar Model 225 DLC excavator. The excavator cab was positioned perpendicular to the tracks of the excavator with the boom in an upright position. The cab window adjacent to the excavator boom was broken. The decedent jumped onto the excavator tracks under the raised boom and leaned through the cab window. His torso/arm contacted the excavator boom joystick-type control lever and the boom lowered pinning the decedent between the boom and the cab frame. Emergency response was called and arrived and transported the decedent to a local hospital where he was declared dead. Recommendations: 1. Equipment owners should ensure machines are red-tagged and taken out of service immediately when safety features are compromised or the machine is in poor condition. 2. Employers should ensure that excavator operators have been trained on the use of the equipment. This training should include safety training in the importance of lowering the boom to a safe position with the bucket on the ground before permitting any site worker (including subcontractors) on foot to approach the machine. 3. Employers should instruct and ensure that any worker (including subcontractors) on foot approach the excavator only after they signal the operator to lower the bucket and the bucket is lowered to the ground, and the machine turned off. 4. Employers should develop, implement, and enforce a comprehensive written safety program for all workers, which includes training in hazard recognition and the avoidance of unsafe conditions. The victim was a member of a five-man crew that had been contracted to prepare an excavation for a concrete footing that would support the concrete basement for a new single-family, private residence in a housing development. The crew consisted of the company owner, a Hispanic crew leader who spoke little English, and three Hispanic laborers that spoke only Spanish. The depth of the excavation ranged from approximately two to three feet AB - however, due to a change in the building plans that included the addition of an additional garage stall, the crew had reached a backfilled area where the depth of the trench reached eight feet when the incident occurred. The company owner was operating the mini-excavator until he had to leave the site to run some errands. When he left, he instructed the crew leader to operate the mini-excavator. As the crew leader was operating the mini-excavator, the victim entered the trench at the shallow end and walked to the deep end to clean loose dirt from the floor with a shovel. As the crew leader yelled for him to exit the trench, the side wall of the trench began to collapse. The victim tried to run to the shallow end, but was covered up. The crew tried to dig the victim out while a nearby homeowner called 911. Upon arrival, fire department personnel ordered the workers out of the trench. They then installed trench bracing, entered the trench, and removed the victim, who was pronounced dead at the scene. The bale was held high by the homemade forks mounted to the loader's lift arms. The yard sloped downhill and to the right ahead of the tractor. The tractor's left rear wheel came off the ground as the tractor moved forward then tipped to the right and continued to overturn onto its top. The driver of the delivery truck, and the farmer's friend who was tending the gate to the cattle yard, rushed to extricate the farmer who was pinned face down in the muck under the left rear wheel of the overturned tractor. Emergency assistance was summoned immediately and law enforcement personnel arrived within ten minutes. Continued resuscitation efforts by emergency medical responders were unsuccessful and the farmer was pronounced dead at the scene. RECOMMENDATIONS 1. Agricultural tractors should be equipped with roll-over protective structures (ROPS) or be used to perform tasks with negligible risk of an overturn. 2. ROPS should be designed and readily available for all makes and models of agricultural tractors in common use. 3. Front-end loaders should not be installed on narrow-front-axle tractors that do not have ROPS. 4. Tractor operators handling large round bales should follow manufacturer recommendations for proper equipment, installation, set up, ballasting, and safe operating practices. The forklift had been left in neutral with the engine running, emergency brake set, and forks elevated. While unattended, the forklift rolled toward the boom truck and pinned the victim between the load of sheetrock supported on the forks and the flatbed of the parked truck. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. 2. Employers should institute a safety and health plan that includes standard operating procedures. 3. Employers should establish a training program for operators of forklift trucks. 4. Employers and employees should become familiar with available resources on safety standards and safe work practices. This was his third run of the day. The SUV had sustained front-end damage when it left the roadway and struck a tree in a residential yard. Both the front tires were flat and the passenger's front tire had separated from the rim. The decedent arrived and pulled the SUV onto the roadway with his tow truck. He repositioned the tow truck to the front of the vehicle. The decedent lowered the truck boom and attached the chains. It is unknown how the decedent placed the chains under the SUV. He did not utilize wood blocks to chock the SUV's rear tires or secondary jack to support the SUV, both of which were available on the wrecker. He told police that he was going to perform a wheel lift, although based on the description of the lift in the police report he performed a sling lift. It does not appear he used the wheel lift arm to lift the disabled vehicle or to provide a backup means of support for the raised vehicle. The wheel lift arm was at ground level when the incident occurred. After attaching the chains and raising the vehicle, the decedent positioned himself on his back, approximately mid-chest under the SUV, apparently to either free the transmission or the transfer case so that the rear wheels would rotate when in tow. When he popped the transmission, the vehicle lurched and fell, landing on the decedent across his neck and chest. 9-1-1 was called. Emergency responders declared the decedent dead at the scene. Recommendations: 1. Towing and recovery industry employers should instruct and ensure employees use a secondary means of support when placing any part of their body under a raised vehicle. 2. Tow truck operators should review the Automobile Association of America (AAA ) Towing and Service Manual, AAA towing service bulletins, or the vehicle owner's manual prior to towing the vehicle to determine the proper towing method. 3. Towing and recovery industry employers should consider equipping each wheel lift tow truck with dollies if a flatbed tow vehicle is not available. 4. Tow truck operators who may be/are under the influence of drugs (illegal, prescription or over-the-counter) and/or alcohol that may place them at a risk of injury should not operate vehicles or machinery since their ability to recognize and respond appropriately to hazardous situations can be impaired. 5. Towing and recovery industry employers should ensure that a copy of MIOSHA General Industry Safety Standard, Part 72, Automotive Service Operations is available to all employees if applicable to the operation. On August 1, 2006, he started work at 7 a.m., had a short break between 9 and 10 a.m. that included soda and crackers, and ate lunch between noon and 1 p.m. The weather was hot and humid with a heat index (a measure of the combined effects of high temperatures and high humidity on the body) between 100 and 110. He had been working in a tobacco field when around 3 p.m. he complained to the crew leader that he was not feeling well. The victim drank some water and was driven back to the workers' housing and left alone to rest. At approximately 3:45 p.m. the victim was found unconscious on the steps of the house. Emergency medical service (EMS) personnel were immediately called and responded within five minutes. The victim was transported to the hospital where his core body temperature was recorded at 108º F and he was pronounced dead. Kentucky Injury Prevention and Research Center AB - In the winter of 2005 a 52-year-old male emergency roadside technician (ERT), (providing traffic control support for police), died when he fell approximately 75-80 feet from a bridge after being struck by a single-unit truck. A policeman had responded to a call from a semi-truck driver on the interstate who had blown a tire while driving on an interstate. After the tire blew, the driver immediately pulled the semi onto the right shoulder and called police for assistance. This section of the shoulder was on a bridge on the blind side of a hill, and was too narrow to accommodate the width of the semi. The back left corner of the semi extended out into the right-hand travel lane of the interstate. A police officer arrived and with emergency lights flashing, parked his cruiser on the shoulder at the top of the hill behind the semi. The officer requested assistance with traffic control. An employee of a company contracted by the local police to provide roadside assistance arrived in a panel truck. Upon arrival, he was instructed by the police officer to park his vehicle with yellow emergency lights flashing at the bottom of the hill, approximately 100 yards behind the police cruiser. After parking his vehicle on the shoulder as instructed, the contractor then exited the vehicle from the driver's side and proceeded to walk up the hill toward the police cruiser. A single-unit truck approached from behind in the right hand travel lane and tried to switch lanes when he observed the yellow emergency lights on the panel truck. When the driver of the single-unit truck switched to the middle lane, the driver struck the right-rear end of a semi trailer in the middle lane, lost control, swerved back into the right lane, sideswiped the ERT's panel truck, then struck the ERT. Upon being struck, the ERT was thrown over the side of the bridge 75-80 feet to the ground below. The police officer called emergency medical services to the scene. They arrived and detected no vital signs in the ERT. The coroner was called and upon arrival, declared the ERT dead at the scene of "multiple blunt force injuries secondary to motor vehicle versus pedestrian". To prevent future occurrences of similar incidents, the following recommendations have been made: 1. A traffic control plan should be implemented and enforced immediately when the travel lane is obstructed. 2. Ensure that the placement of various types of warning devices (portable signs, orange traffic cones, flares and/or portable changeable message signs) informs drivers of what to expect when approaching an incident scene. 3. In addition to flashing lights, a permanent message board should be placed on the back of emergency response vehicles that routinely need to control traffic. 4. Ensure that personnel receive training in the proper procedures and the hazards associated with emergency operations for highway incidents. 5. When providing emergency roadside assistance, company policy for exiting emergency vehicles away from the traffic lanes should be enforced. 6. Braking mechanism performance checks should be routinely performed on all motor vehicles before entering roadways. A concrete floor slab was 26' (feet) 5½" (inches) long, 8'3" wide and 8" thick, and weighed approximately seven tons. The floor slabs were positioned on the building's steel structural frame composed of vertical steel columns and horizontal steel beams. The slabs were not secured to the steel framework at the time of the incident. At approximately 9:20 a.m., the victim and a co-worker were on the 6th floor using pry bars to adjust the precast floor slabs into their final positions before they were to be permanently secured to the steel framework. The southern section of the slab rested on the horizontal steel beams and its northern section was suspended (cantilevered) without the support of the steel beams. After the two workers finished adjusting the slab, they both walked northward on the slab near its east end. The slab suddenly tilted downward at its north side, rotated to a near vertical position, and became wedged in between the steel supports. The co-worker secured himself with his elbows onto the slab in front of him and climbed up. The victim fell to the floor below. The failed slab then fell, struck the victim, and crashed through the cement slab floors below. The victim fell through the opening created by the fallen slabs and landed on the 3rd floor. The co-worker immediately ran down to the victim and removed him from the collapsed zone. One of the workers called 911. Both police and an emergency service unit responded within minutes. The victim was transported to a hospital where he died an hour later. The post incident investigation found that a number of steel columns including the ones in the collapsed zone were erected out of plumb. The out-of-plumb columns led to increased spacing between the columns resulting in reduced beam support for the slabs located in-between these columns. The integrity of the initially failed slab was further compromised by its notched shape and cantilevered section. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators recommend the following measures to help prevent similar incidents from occurring in the future, employers should: 1.) comply with the structural dimensions and tolerances specified by the erection design and check vertical and horizontal alignment of the steel structural frame before erecting any precast slabs AB - 2.) ensure that workers use personal fall protection arrest systems when working on the precast concrete floor slabs that are not permanently secured to the steel framework AB - 3.) provide employees with training on the prevention of fall hazards associated with working on the unsecured precast concrete floor slabs AB - and secure the precast slabs to the steel structural frame by the method specified in the erection plan as soon as the slabs are erected. Additionally: 4.) the integrity and support strength of odd shaped or notched precast concrete members should be carefully evaluated AB - supplemental fastening should be provided if necessary. 5.) quality control procedures should be implemented to ensure that proper tolerances are maintained by all trades involved in a precast concrete construction project. 6.) pre-construction meetings should be held to clearly define the responsibilities of all key participants of a precast concrete construction project. The victim and four other laborers had been hand digging the trench over a two-day period at a private residence. The victim was kneeling to inspect a broken drain pipe at the bottom of the east end of the trench. At approximately 3:00 p.m., a laborer on top of the trench saw a crack in the soil and yelled to the victim. The wall collapsed and the victim was completely covered with soil in seconds. Three laborers along with the company owner who was checking on the job began frantically hand digging to reach the victim. The fourth laborer remained above and outside of the trench. The owner stopped digging after a couple of minutes to call 911 with his cellular telephone. All four men (three laborers and company owner) continued to hand dig until the local fire and police departments arrived on the scene. Fire and police personnel continued hand digging. The victim's head was uncovered approximately 45 minutes after the trench collapse. Emergency personnel on the scene checked for vital signs. The victim was pronounced dead at the scene. Recommendations: 1. Employers should ensure that employees working in excavations are protected from cave-in by an adequate protection system such as shoring, sloping or excavation box designed in accordance with MIOSHA Construction Safety Standard, Part 9: Excavation, Trenching and Shoring, R408.40941. 2. Employers should ensure a qualified person inspects the excavation, adjacent areas and supporting systems on an ongoing basis, and the qualified person take the appropriate measures necessary to protect workers. 3. Employer should develop, implement and maintain a comprehensive health and safety program. 4. Employers should provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments in the language of the workers. 5. Employers should develop a trench emergency action plan that describes rescue and medical duties and ensure that all employees are knowledgeable of those procedures. 6. In addition, MIFACE recommends MISS DIG be contacted prior to excavation activities to identify the location of utilities in the area. However, there has not been a standard survey or reporting tool used to assess the prevalence of work-related asthma among school employees in the Commonwealth. To learn the best way to survey school employees regarding their respiratory health and inform future research, the Massachusetts Department of Public Health, in collaboration with the Massachusetts Teachers Association and the Healthy Schools Network, developed and piloted a short questionnaire to gather information on the prevalence of asthma and respiratory symptoms in school personnel. This pilot project demonstrated that anonymous, self-reported information on asthma and respiratory symptoms in school staff and observed environmental conditions in schools can be collected using a brief self-administered questionnaire. Self-administered surveys were completed by 359 staff in a convenience sample of 13 public elementary schools (response rate 44%). Survey findings corroborated the occurrence of respiratory problems among school staff. The design of the pilot project precluded generalizing findings to estimate the prevalence of respiratory problems among all elementary school personnel in Massachusetts. The proportion of staff reporting current asthma (10%) was no different than the proportion of all adults (9.7%) and somewhat less than the proportion of all females (12.2%) reporting current asthma in the Massachusetts Behavioral Risk Factor Surveillance System (BRFSS). However, the proportion of school staff reporting that their asthma was related to work (25%) was greater than expected from the 2001-2002 Massachusetts BRFSS (8.8% all; 9.0% females). An additional 21% of respondents reported respiratory symptoms associated with work. In response to an open-ended question, staff expressed concern about poor indoor air quality, mold and dust. The findings of this pilot project, while limited, taken together with sentinel surveillance data on work-related asthma and other published reports, suggest that further research on respiratory health among school personnel and its association with environmental conditions in schools is both feasible and warranted. Department of Labor and Industries AB - On September 12, 2006, a road construction laborer was fatally injured while attempting to stop a runaway work vehicle. The 19-year-old victim had a little over one year of experience as a construction laborer, most of which was with his current employer, a manufacturer and installer of roadway signs. On the day of the fatal incident the victim and a co-worker were completing the installation of a road sign alongside a stretch of rural mountain road where they had been installing signs for the past week. The two workers had used a truck- mounted auger to drill a hole and set the sign post, and were in the process of backfilling the hole. The auger truck was parked on an incline with a grade of about 1 ½ to 2 percent. As they were finishing the job, the unoccupied auger truck began to roll down the road. The victim ran after the moving truck and entered through the passenger door. He attempted to control the vehicle as it gathered speed along the road until it crashed through a guardrail, went down an embankment and caught fire. The victim was declared dead at the scene. An investigation determined that wheel chocks were not used to block the truck's tires, the truck's engine had been running, and both the PTO and the power lock braking system were engaged. It was unknown whether the parking brake had been set. At the time of the incident, the victim was traveling south in her car in the northbound lane placing newspapers in the curbside mailboxes along the east side of the highway. The deputy sheriff was on his evening shift patrolling the same area. At 2:28 A.M., the deputy sheriff was dispatched to respond to an emergency call. The deputy sheriff started driving north on the same highway towards the crest with his emergency lights activated. Meanwhile the victim was approaching the crest heading south in the northbound lane. Because of the obstruction of the hill and the vehicle speeds, neither the deputy sheriff nor the victim had sufficient time to react to avoid the collision. The victim's car was straddling the northbound fog line when it collided with the police car just north of the crest. The victim was not wearing a seat belt and was pronounced dead at the scene. The deputy sheriff was wearing his seat belt during the collision AB - he was treated for minor cuts and bruises. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring, employers should: 1.) instruct newspaper delivery personnel not to drive on the wrong side of roadways while delivering newspapers AB - 2.) instruct newspaper carriers to use their seat belts whenever the vehicles are in motion AB - 3.) require newspaper carriers to use high-visibility strobe lights on their delivery vehicles AB - 4.) evaluate delivery routes, assess the hazards and develop a comprehensive delivery safety program AB - 5.) provide delivery safety training to all newspaper carriers AB - 6.) consider using right-hand drive cars for delivering newspapers AB - and 7.) promote customers' understanding of the delivery hazards so that they would be more receptive to alternative paper delivery methods. The decedent had delivered several loads of sugar beets to a local processing facility. Prior to re-entering the wet beet field for another load, he wanted to dump the tare weight (dirt, sugar beet parts) from the dump trailer bed. According to known past work practices on the farm, while inside the tractor cab, he activated the dump trailer to rise. The event was unwitnessed. A probable incident scenario was developed during the interview of the decedent's employer, who was the farm owner. The farm owner suggested that the decedent would have wanted to ensure that the tare weight was leaving the dump trailer. While the trailer was rising, he exited the truck cab and walked along the side the truck trailer body to take a look at the exiting tare. Sometime during this activity, the top of the trailer bed contacted the overhead line. It is unknown if the decedent was aware of the contact. A second truck driver whose truck was being loaded with sugar beets nearby in the same field noticed a flash of light and saw the truck on fire. The farm owner, who was also the employer of the decedent, was operating the beet harvester and loading this truck. The truck driver and his passenger, a paramedic, immediately ran to the decedent and began CPR. Emergency response arrived and transported the decedent to a local hospital where he was declared dead. Recommendations: 1. Farm employers should conduct a field survey prior to fieldwork to determine any hazards at work locations, (such as overhead power lines), determine the work tasks to be performed, and identify safe areas to perform the work away from the hazard. 2. Employers should talk with the electrical power company to gather safety information to develop and implement safe work procedures if the lines cannot be de-energized and thus necessary to perform fieldwork near the energized overhead electrical lines. 3. Farm owners and truck owner/operators should consider installing high voltage proximity alarms on truck trailer dump beds in case of inadvertent operation of machinery near overhead power lines and train employees in electrical principles and proximity alarm use and limitations. 4. Employers, truck owner/operators, and truck leasing companies should measure the raised dump trailer height of each of their trucks and post this height prominently in the cab compartment. 5. Employers should establish emergency procedures for fellow workers to follow in case of an electrocution and train the workers in these procedures. The wood piece injured both hands of the student. After the incident the student went to the school nurse's office where a call was placed to the student's mother who came to the school and drove the student to the hospital. The second student was injured eight days later when the student's hand came in contact with the rotating blade of the table saw the student was operating. The student had just finished making a cross cut on a wood piece at the time of the incident. After the incident the instructor called the school nurse for help. A call was placed for Emergency Medical Services (EMS), which responded and was at the incident site within minutes. EMS transported the student via ambulance to a local hospital. The Massachusetts Department of Public Health concluded that to prevent similar occurrences in the future, schools with mill/carpentry shops should: 1. Provide shop conditions that at a minimum meet occupational safety and health standards set by the Occupational Safety and Health Administration (OSHA), and train students about these standards AB - 2. Ensure that the machine guarding used is the most effective guarding available AB - 3. Ensure that the appropriate equipment/tool is matched to the task being performed AB - 4. Ensure that students are comfortable operating equipment before being allowed to operate the equipment without direct supervision AB - 5. A fire resulted and ignited the victim's clothing while he was seated in the cab. The victim bailed from the bulldozer. The 76-year-old owner, working nearby, rolled the victim on the ground and extinguished the fire by smothering it with loose dirt and shale. The owner helped the victim to his truck and called the office to have them call 911. Emergency Medical Services (EMS) arrived approximately seven minutes after receiving the call and transported the victim to an area hospital. After being treated at the hospital for approximately three hours, the victim was airlifted to another hospital and died the next day. The owner returned to check on the bulldozer and found it in a ravine with some light fire visible. After turning the bulldozer off and realizing he was burned, the owner drove himself to the hospital. The owner was admitted to the hospital for second and third degree burns over twenty percent of his body. He was airlifted the following day to another hospital, where he remained for three weeks before being discharged to a rehabilitation hospital for an additional three weeks.
