Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Upon falling asleep, the unit travelled to the right, across the rumble strip and the emergency lane, behind a guardrail, down an embankment and crashed into a grove of trees. A man driving a car behind the semi truck saw dust rise from the crash, stopped, called emergency medical services and spoke with the driver. The driver stated that he fell asleep. EMS arrived six minutes later, spoke with the driver who was trapped and restated that he fell asleep while driving. The driver died 20 minutes later while emergency services tried to extricate him from the semi. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Semi tractor-trailer drivers should be trained to recognize signs of fatigue and drowsiness and when to seek appropriate rest areas. Recommendation No. 2: Employers should establish worker safety programs that include recommendations for addressing sleepiness and fatigue. Recommendation No. 3: Commercial transportation companies should explore the types of drowsy-type driver alert systems that are available on the market and consider providing their drivers with this type of technology. Recommendation No. 4: Manufacturers of semi trucks should explore the possibility of incorporating driver alert systems technology (e.g., face recognition, vibrating steering wheels and seats, audible and visual alarms) to assist drivers during driving operations. Recommendation No. 5: Policy makers should consider using Fatality Assessment Control and Evaluation reports to inform federal and state transportation regulatory policy making activities. The job site work crew was erecting a steel rebar cage weighing about 5,600 lbs as part of a column structure. It measured 20x56 inches and 30 feet tall. They had previously erected 11 similar cages. The rebar cage was braced using 2x4's nailed together and attached to the building floor. A tower crane placed the cage into position and then released the cage. The victim and another crew member were climbing the cage to make adjustments to the rebar and set the braces. Both workers had fall protection gear and were working on the same side of the cage when the crew noticed the cage began to lean. The crew attempted to reconnect the crane to the cage, but before they were able to one of the 2x4 braces failed and the cage collapsed. The victim was struck by the falling rebar cage and died of blunt force injury of the head at the scene. The other worker escaped without injury. The carriage was 8-10 feet overhead and was still rolling a bit. The yarder engineer was just in the process of setting the mainline brake when the line suddenly slackened. The carriage dropped and crushed the chokersetter, killing him instantly. The medical examiner reported the victim's blood alcohol content at .02. Department of Public Health AB - On June 3, 2008, a 31-year-old male construction laborer (the victim) was fatally injured when he was struck by a backing dump truck. The victim was on foot walking away from the dump truck, towards oncoming traffic, while painting a guideline for the operator of an asphalt milling machine to follow. The dump truck operator started to back the dump truck, striking the victim with the right rear double wheels. The dump truck operator realized that he had struck something, placed the truck in drive, moved the truck forward approximately 54 feet and exited the truck. When the dump truck operator walked to the rear of the truck he noticed the victim and started to yell for help. Calls were placed for emergency medical services (EMS). A state trooper assigned to the work zone was radioed by dispatch, scanned the immediate area for the victim, and then got into his police car and drove to the incident location. Within minutes EMS arrived and pronounced the victim deceased at the incident location and the Medical Examiner's Office was called. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Develop, implement, and enforce an internal traffic control plan (ITCP) specific to each construction site to help protect workers on foot AB - 2. Ensure backing protocols are in place and that designated individuals are assigned as signalers to direct backing vehicles on construction sites AB - 3. Ensure that communication exists among equipment operators and workers on foot AB - 4. Ensure work zones are properly illuminated AB - 5. Implement a buddy system for employees working on foot around mobile construction equipment AB - 6. Develop and enforce policies that prohibit non-employee passengers from riding within the cabs of construction vehicles and heavy equipment while being operated within construction sites / work zones AB - 7. Consider installing monitoring technology on construction vehicles and equipment to assist operators in detecting workers on foot within blind areas AB - and, 8. Ensure that all employees performing work on Massachusetts publically funded construction projects costing more than $10,000 have completed the OSHA 10 hour training. The victim, who was working alone, had driven his van inside the empty warehouse and parked near the offices he was assigned to clean. He ran his truck-mounted gasoline-powered carpet cleaner for several hours until he succumbed to the CO generated by the carpet cleaner. To prevent similar occurrences, the Washington State FACE Program published the following recommendations for businesses engaged in commercial carpet cleaning: 1. Do not use gasoline or other fuel-powered engines, equipment, or tools inside buildings or areas where carbon monoxide can build up. 2. Work with manufacturers to re-design the truckmounted carpet cleaner so that the unit more effectively reduces or eliminates carbon monoxide in the exhaust gases or prohibits exhausting into the van and work areas. 3.Ensure carbon monoxide detectors with alarms are used when employees work with fuel-powered engines in environments where CO is likely to build up. 4. Use a "Risk Assessment" or "Job Hazard Analysis" tool to identify potential carbon monoxide sources and exposures and how to avoid them. 5. Educate workers regarding hazards, sources, symptoms and control of CO exposure. 6. Ensure that employees work in pairs on new or unfamiliar jobs and that a continuous communication plan is in effect. The co-worker went to the back of the house to perform other duties and when he returned to the front of the house, he found the victim at the base of the tree. No one witnessed the incident. The CA/FACE investigator determined that in order to prevent future incidents, gardeners who trim trees should ensure that: 1. They have received training and certification on how to safely trim trees by qualified instructors. 2. Fall protection is worn when climbing trees. In addition, homeowners should hire only tree trimmers who are certified by organizations such as the ISA (International Society of Arboriculture) or the TCIA (Tree Care Industry Association). While the work crew was removing the planter box from the wall, it suddenly shifted and struck the scissor lift, knocking it over. The scissor lift was rated for 500 pounds. Ensure that employees use scissor lifts that are the appropriate size and capacity for the job. 2. These activities are also a regular and frequent source of workplace injuries and fatalities. In Washington State from years 2005-2010: 12 workplace fatalities occurred during truck loading or unloading. Six of the 12 incidents involved machinery including forklifts and cranes. Five incidents involved truck drivers, 3 involved laborers, 2 involved supervisors, 1 involved a landscaper and 1 involved a crane operator. Some of the incidents that happened during truck loading and unloading in Washington State are described here: On January 15, 2010, a freight operations supervisor was assisting a forklift driver secure freight on a storage rack inside a trailer. The supervisor reached up and grabbed a piece of freight to steady himself when the 500 lb. pallet of freight shifted and fell on him. On June 25, 2007, a truck driver was unloading a delivery of a 2-ton manufactured piece of steel. He was standing beside the truck unfastening a securing strap when the piece of steel, which was improperly secured, fell off the truck and crushed him. On March 24, 2006, three workers were securing a load on the deck of a boom truck. The load came in contact with the levers of the boom crane, causing it to elevate and come into contact with an overhead high voltage power line. One worker was electrocuted. The other two received severe electrical shocks and were hospitalized. On December 6, 2005, a truck driver was picking up waste drums of cooking oil at a restaurant. He was using the truck's hydraulic lift gate to lift the drums when he was crushed between the truck's bed and the lift gate. How to Prevent Truck Loading and Unloading Incidents: 1. Restrict personnel on foot from the loading zone during operations. 2. Train loaders and drivers to identify hazards and apply safe work practices. 3. Secure loads on trucks, forklifts and cranes properly. 4. Establish procedures to assure workers are clear of trailers/wheels before moving. He was riding in the hopper area because the step he usually rode, located at the rear driver side, was broken. Another worker rode the rear step on the passenger side of the vehicle, and a third worker, the driver, drove. After approaching a stop, the driver looked into the rearview mirror and saw the decedent on the pavement. The driver stopped the truck, and he and the other solid waste collector tried to assist the decedent. There was no cell phone service in the area so the driver drove the truck four miles to a store, and called emergency medical services and his supervisor. Emergency medical services arrived at the scene and transported the worker to the nearest trauma hospital where he died six days later. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Municipal solid waste divisions should require drivers to perform daily post-trip safety checks on solid waste collection vehicles and timely repairs should be performed. Recommendation No. 2: Municipal solid waste divisions should ensure that employees are never located on solid waste trucks' rear riding steps when the trucks are traveling faster than 10 miles per hour or traveling more than 0.2 mile. Recommendation No. 3: Municipal solid waste divisions should ensure that employees have access to a communication device that can be used during emergency situations. Recommendation No. 4: Municipal solid waste divisions should perform job hazard analyses. Recommendation No. 5: Municipal solid waste divisions should have a written worker safety program that includes safety training for employees. Recommendation No. 6: Municipal solid waste divisions should provide an alcohol/drug free workplace for employees. They were returning surveying equipment to the company truck. Surveyor 1, company owner, was walking with traffic on the east side of the road, in the grass, a few feet to the right of the pavement. Surveyor 2 was walking on the edge of the pavement on the west side of the road. Both men were on the road on the blind side of a hill and there were no emergency shoulders. A passenger car traveling north, driven by a young driver, crested the hill. After cresting the hill, the driver saw the pedestrians, applied the brakes, and lost control of the vehicle. The vehicle skidded off the pavement toward Surveyor 2 on the left side of the road. Surveyor 2 saw the vehicle coming towards him and ran down the embankment away from the oncoming vehicle. The car left the pavement, became airborne, and landed on top of Surveyor 2. Emergency services were called. Upon arrival of EMS personnel, Surveyor 2 was transported via helicopter to the nearest trauma hospital where he died later that same day. He was 47 years old. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: When surveying on land near roadways, safety cones and warning signage should be erected to slow down motorists. Recommendation No. 2: A hazard assessment should be performed before land-surveying work commences. Recommendation No. 3: Land surveying companies should have a comprehensive written safety program. The victim and two co-workers were in the process of moving the large "L" rack of mirrors closer to a glass cutting table, using two pallet jacks. The "L" rack was designed with forklift pockets at the bottom, with the intention that it be moved using a forklift only. As the employees moved and repositioned the rack using the two pallet jacks, it fell rearward and struck the victim. To prevent similar occurrences, Washington State Fatality Assessment and Control Evaluation Program officials recommend that businesses engaged in handling, shipping or receiving flat glass or similar products: 1. Develop a safer method of glass transportation and handling that eliminates use of the "L" glass rack. 2. Modify "L" glass racks permanently or temporarily while at the business location to prohibit the use of pallet jacks. 3. Design and organize the work environment to eliminate hazard potential. 4. Institute and enforce strict material-handling policies that prohibit unsafe and improper materials handling. 5. Require written hazard assessments and plans specific to material-handling tasks. 6. Provide safety training materials in an appropriate language for employees. 1. Develop a safer method of glass transportation and handling that eliminates the use of "L" racks. The FACE program recommends that companies that use similar "L" racks investigate options for safer transportation and handling of large plate glass, mirrors, granite or concrete slabs, and similar size and weight materials. Alternate options include stacking materials horizontally or vertically in a crate, with spacers between each lite for protection and access for handling. A vacuum handling system with crane and forklift-mounted vacuum units for the handling and movement of individual lites would complement the crate transportation method. Handlers of glass mirrors and similar flat, heavy materials should refrain from using wheeled carts and dollies, which also pose significant hazards. 2. Modify "L" racks permanently or temporarily while at the business location to prohibit the use of pallet jacks. The design of the "L" rack involved in this incident made it possible for employees to use pallet jacks to move the rack and mirror crates. The bottoms of the racks should be modified to prevent the use of pallet jacks. Companies should develop a method to permanently block the spaces on any racks that will remain at the facility and a method that will not allow access by pallet jack while racks are at the facility. One method to permanently prevent the use of pallet jacks involves welding plates over the lower front and back spaces. To temporarily prohibit access, install plates that cover the bottom open spaces, connected front-to-back and tightened using a threaded rod and nut or clamping system. Other options include fabricating a solid object to fill the spaces under the rack with clamps that connect it to the base of the rack, or fastening a temporary belt around the bottom perimeter of the rack. 3. Design and organize the work environment to eliminate hazards. Companies should design and organize work areas so there is sufficient space to move and manipulate large racks of materials. In this case, the plant manager suggested that the employees were moving this tall "L" rack with pallet jacks because a scrap bin blocked the path of the forklift. During the investigation, FACE personnel, along with the company safety manager, noted there was insufficient space between the safety fence and the scrap bin to use a forklift to maneuver the "L" rack. As a hazard prevention practice, companies should organize the workspace to provide sufficient space for handling and movement of large and heavy materials. In this case, the scrap bin could have been easily moved and placed out of the way by a forklift. Another option is to segregate the workspace into zones where specific activities or equipment are either allowed or prohibited. In this case, it might have been beneficial to prohibit the use of pallet jacks in the area where large "L" racks were regularly moved. 4. Institute and enforce strict material-handling policies that prohibit unsafe and improper materials handling. In this case, a strict written and verbal policy against moving "L" racks with pallet jacks might have resulted in one of the three employees objecting or cautioning the other two against the maneuver. 5. Require written hazard assessments and plans specific to material-handling tasks. A specific, written hazard assessment and step-by-step plan should be required for material-handling tasks. This is especially important for non-routine tasks. The hazard assessment should include a specific listing of the individual tasks, who will be involved and how the hazards associated with each component will be managed. The assessment and plan should be developed jointly by all involved in the task, reviewed by the company safety manager and strictly followed. 6. Provide safety training and materials in appropriate language for employees. Companies should provide safety training and materials in languages appropriate to all employees to ensure they understand potential hazards and their consequences. This extends to verbal training and written materials. A possible option is to designate an employee as safety representative and liaison for each language. El trabajador empezó a cortar las ramas y algunas se enredaron y quedaron colgando encima de su cabeza y hombros. Él no podía respirar y murió asfixiado. Department of Labor and Industries AB - On February 21, 2008, a 39-year-old orchard laborer died when he was caught in the power take-off (PTO) shaft of a tractor-mounted post hole digger. The victim worked at an apple orchard where he had been an employee for six years. The victim and a coworker were assigned to auger postholes to support young trees. They were using a tractor-mounted post hole digger to dig two to three foot deep holes. The tractor's PTO shaft rotated at 540 revolutions per minute. As his coworker operated the tractor and PTO shaft, the victim stood behind the tractor and to the side of the post hole digger manipulating the guide lever with his hands to make sure it was boring a straight hole. The right sleeve of the victim's jacket got caught in the unguarded, rotating PTO shaft of the post hole digger, causing his arm to become entangled and severed. He died at the scene. An investigation determined that the manufacturer requires a guard to be used and that individuals stay back 10 feet while the auger is being operated.
