Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

He worked for his employer, a tree care service company, for 24 years as a trimmer and bucket operator. He was on a three-member crew pruning birch trees at a private home. They were using a bucket truck to remove damaged branches. He went up in the bucket alone while his co-workers worked on the ground. He was wearing all required personal protective equipment (PPE), including a fall arrest harness. After cutting a few branches, he had to move the truck to change the bucket angle near the tree. He lowered the bucket and unclipped his harness to get out. A co-worker helped him move the truck and set the outriggers. No one saw him re-enter the bucket before he went up again. As the bucket went up, the ground crew saw the truck tilting toward the passenger side with its wheels off the ground. The bucket appeared to be caught and pressing on a branch with enough force to tilt the truck. When the bucket broke free of the branch, the truck's wheels slammed down and shook the bucket so violently it ejected the trimmer 25 feet to the ground. The ground crew ran to him, called 911, and began CPR as directed by emergency dispatchers. First responders soon arrived but pronounced him dead at the scene from severe fall injuries. Following the incident, investigators found: The worker's fall arrest harness lanyard was not tied-off in the bucket. The employer had a fall protection work plan and the crew had a pre-job safety briefing. The bucket truck manufacturer inspected and found the vehicle to be in safe operating order. REQUIREMENTS: Employers must: Before elevating the platform (aerial bucket), ensure all persons on the platform wear a full body harness with a lanyard attached to either: (a) The manufacturer's recommended attachment point; or (b) The boom or platform if the manufacturer does not specify an attachment point. The employee must never attach a lanyard to an adjacent pole, structure, or equipment. RECOMMENDATIONS: FACE investigators concluded, that to help prevent similar occurrences, employers should: Develop and enforce policies and standard operating procedures (SOP) in their accident prevention program (APP) requiring: A co-worker be present to observe when the bucket operator enters the bucket. The bucket operator to give the co-worker visual confirmation they are wearing a personal fall arrest harness with the lanyard tied-off in the bucket before going up. The co-worker to yell "all stop" or use a small air horn and press the boom's emergency stop switch when an unexpected hazardous situation needs to end quickly to prevent injury. Update policies to reflect changes in site conditions and fall protection equipment. Review policies with workers at crew meetings, monthly safety meetings, and annual fall prevention stand-downs. Provide recurring hands-on fall protection training and ensure workers maintain skill proficiency. Shoring was not in place and no bracing or underpinning was used to ensure the stability of the existing wall. The trench undermined the neighboring wall, which suddenly collapsed. The wall fell on the ironworker in the trench and crushed him to death. The teen was using a walk-behind mini skid steer with a hydraulic rotary trencher attachment. The day before the incident, the teen received a brief demonstration of the trencher by a supervisor. He then used the trencher for 2 to 3 hours. The next day, a foreman assigned him to use the trencher unsupervised to dig a channel for fence posts. After a half hour, he stopped to check the trench depth. He walked in front of the trencher without shutting it off. As he stood on the 8-inch trench, the sidewall collapsed and his legs were pulled into and severed by the trencher's roller chain. While lying on the ground badly injured, the teen first tried to phone his mother and then his supervisor, who was off-site, but could not reach them. The teen sent photos of his injured legs to his supervisor, who saw the photos and called the workers at the jobsite. The workers rushed to aid the teen, called 911, and applied tourniquets to each leg. First responders arrived and had a critical care helicopter airlift the teen to the hospital where his life was saved. Following the incident, investigators found: The employer had a student learner exemption permitting minors to do some work that was otherwise prohibited for workers under 18. The skid steer and trencher attachment were not part of the exemption. The teen had no proper training or experience that qualified him to operate the trencher and was using it alone and out of site from co-workers and with no supervisors onsite. Requirements and restrictions for minors were not communicated from management to supervisors. REQUIREMENTS: Employers must: Unless granted a variance by the director of the Dept. of Labor & Industries, not allow minors participating in a bona fide work-based learning program certified and monitored by the office of the superintendent of public instruction or the minor employee's school district to perform the prohibited and hazardous employments outlined, except those exempted in subsections (5), (8), (9), (11), (13), (15), (16), and (23). Permit only those employees qualified by training or experience to operate equipment and machinery. RECOMMENDATIONS: FACE investigators concluded, that to help prevent similar occurrences, employers should: Develop an accident prevention program (APP) policy that covers relevant employment restrictions for minors and exemptions listed in the employer's approved variance application for minors in work-based learning programs. Communicate the policy at new worker orientation trainings, monthly safety meetings, and daily supervisor-led crew meetings before work begins. Ensure supervisors prohibit minors in work-based learning programs from hazardous work without direct supervision and/or alone, from restricted job tasks, and from restricted tools, equipment, or machines. Provide minors in work-based learning programs ample training, including refresher training, for each type of tool, equipment, and machine. Evaluate their knowledge and skills before allowing them to work. While standing on the patio roof, the installer started prying between the upper roof shingles and the fascia in order to make a space to attach the gutter flashing. As he continued prying toward the corner of the upper roof, he reached the edge of the patio roof on which he was standing. He either did not see the patio roof edge and stepped off or was extending his reach and lost his balance. He fell 13 feet to the concrete patio. The employer, who had just arrived, and homeowner found him at approximately 7 a.m. on the patio with his pry bar nearby. He was wearing a tool belt but not the personal fall protection equipment that was available in the work truck. Emergency responders declared him dead at the scene. Following the incident, investigators found the employer did not: Have a written accident prevention program (APP) or a fall protection work plan. Provide fall protection training or ensure workers used fall protection equipment. Document safety meetings. REQUIREMENTS: Employers must: Develop a formal, written APP. Develop and implement a written fall protection work plan, including each area of the workplace where the employees are assigned and where fall hazards of ten feet or more exist. Ensure that each employee on a walking/working surface with an unprotected side or edge four feet or more above the ground or lower level is protected by a fall protection system. Provide training for each employee exposed to fall hazards. Document attendance and subjects discussed at crew-leader safety meetings. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences, employers should: Arrive on-site before work begins or appoint a supervisor to ensure that workers use fall protection and follow fall prevention safe work practices. During the pre-job safety meeting, go over the fall protection work plan and discuss workers' responsibilities and the details of how they will implement the plan. Make fall prevention a company priority and emphasize to workers to always use fall protection when required. Aún se estaban montando las estructuras del piso y el supervisor retiró los barandales de prevención de caídas en un espacio en donde se iba a instalar una estructura de pared. El supervisor le ordenó al instalador de estructuras que cortara una abertura para una ventana en un trozo de revestimiento de madera que se estaba preparando para la estructura de la pared. Después de hacer la abertura, el instalador de estructuras fue a dejar los residuos en una pila de escombro, en el suelo. Caminó sobre la estructura de pared, que estaba recostada en el piso, y luego sobre un pequeño trozo de revestimiento pegado que sobresalía del borde del piso. El revestimiento no estaba sujeto con clavos de manera apropiada y cedió bajo su peso, haciendo que cayera 22 pies hasta el suelo. The framer was working with a supervisor on the unfinished, open-sided floor. The floor was still being framed, and the supervisor removed fall prevention guardrails in an area where a wall assembly was going to be installed. The supervisor instructed the framer to cut out a window opening in a piece of wood sheathing being prepared for the wall assembly. After making the opening, the framer went to drop the cuttings down to a scrap pile on the ground. He walked on the wall assembly, which was laying on the floor, and then onto a small piece of attached sheathing that extended past the edge of the floor. The sheathing was not properly nailed down and gave way under his weight, causing him to fall 22 feet to the ground. The supervisor witnessed the fall, ran downstairs, and called 911. The emergency dispatcher guided him in giving first aid. When first responders arrived, they could not save the framer, who died of severe head injuries. Following the incident, investigators found: The employer had no fall protection system and work plan and did not enforce any requirements. The employer had been previously cited for fall protection rule violations. The framer was wearing a body harness but had no other personal fall arrest system (PFAS) components. Supervisors routinely used chalk lines in place of PFAS when guardrails were removed, but no lines were observed at the incident site. The owners were aware of but never corrected the unsafe practice. Workers were rushing to raise the walls so trusses could be placed on top of them upon delivery and to avoid the delay of having the trusses delivered on the floor and lifted by a crane later. REQUIREMENTS: Employers must: Employers with employees exposed to fall hazards must have and enforce policies in their accident prevention program (APP) that meet the Unified Safety Standards for Fall Protection. The employer must develop and implement a written fall protection work plan including each area of the work place where the employees are assigned and where fall hazards of ten feet or more exist. Ensure a competent person trains each affected employee to know at least the following: (a) The nature of fall hazards in the work area; (b) When fall protection is required; (c) What fall protection is required; (d) The correct procedures for erecting, maintaining, assembling, disassembling, and inspecting the fall protection systems to be used; (e) The use and operation of fall protection systems used; (f) Limitations of fall protection systems used; (g) Proper care, maintenance, useful life, removal from service; and (h) The requirements of this chapter. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences employers should: Develop APP policies to use PFAS when fall prevention guardrails are not in place on open-sided floors. Discuss fall protection policies at crew meetings, monthly safety meetings, and annual stand-downs. Conduct recurring hands-on fall protection training and ensure workers show proficiency in practice. Require all levels of management to spot check workers and enforce fall protection policies and practices. Use a project timeline to communicate task schedules with workers and suppliers to avoid rushing. His job duties included maintaining construction vehicles, trucks, and trailers. He was married with young children. The mechanic was installing a swing drive into the de-energized excavator. A supervisor showed up at 9 am to check the work and arranged to have new hydraulic hoses delivered to the mechanic. He then left the jobsite. After the hoses were dropped off, an assistant came to help the mechanic. They completed installing the swing drive just before noon. The assistant then left the site as the mechanic planned to install the hoses by himself. Another supervisor who was getting ready to leave asked the mechanic if he needed anything. The mechanic said no, so the supervisor left him alone at the site. At 1:45 pm, the assistant texted and called the mechanic but got no response. The first supervisor texted the mechanic almost an hour later but also got no response and thought he went home. At about 5:30 pm, the mechanic's wife left a voice message on the employer's call service asking about her husband. A worker heard the message and texted the supervisor, but the text failed to reach him. The mechanic's wife also called a family friend. The friend did not know where the mechanic was but was able to get directions to the site from a worker. The worker also contacted the supervisor, whose GPS showed the mechanic's service truck was still at the site. Now around 40 miles away, he began driving back to the site. The friend arrived there first, found the mechanic unresponsive and upside down in the swing drive compartment, and called 911. First responders pronounced the mechanic dead at the scene. The medical examiner reported the mechanic died from suffocation caused by being trapped in a head-down position. Following the incident, investigators found: The mechanic had prior experience installing excavator swing drives. Ratchet straps, a come-along, and tools for the job posed trip hazards near the swing drive compartment. The boom side operator's cab window was shattered presumably when the incident happened. The employer had no written safety policy for lone workers. REQUIREMENTS: You must provide your employees a workplace free from recognized hazards that are causing, or are likely to cause, serious injury or death. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences, employers should: Develop and enforce policies and standard operating procedures (SOP) in their written accident prevention program (APP) that: Prohibit lone work and instead require using a buddy system where workers team up so they can observe each other and assist quickly when someone becomes endangered. Require a formal job hazard analysis (JHA) be conducted jointly by supervisors and workers to identify hazards for lone work. Prevent hazards by implementing less hazardous work practices. If lone work is unavoidable, provide and require use of lone worker monitoring technology or communication procedures, such as two-way radios, in-person check-ins, wearable panic buttons or worker down apps for cell phones. Require supervisors to routinely observe workers to ensure they are following policies and procedures. CONTRIBUTING FACTORS - Improper work practices (removing fronds from below) AB - Incorrect equipment to do the job safely AB - Homeowner hiring tree trimmers and supervisors that are not certified or licensed. RECOMMENDATIONS - The California FACE (CA/FACE) investigator determined that, in order to prevent similar incidents, landscaping companies and self-employed tree trimmers should ensure that: Palm tree trimming or removal is performed or supervised by workers trained and certified by organizations such as the International Society of Arboriculture (ISA). Proper work procedures and equipment are used, such as using an aerial lift and cutting fronds from above. Homeowners should: Only hire tree trimmers and/or supervisors who are certified by organizations such as ISA, and have a tree service contractor's license (in states where this is applicable). He worked for his aunt and uncle, who owned the gas station for three years. The clerk moved to the United States the previous year to learn how to run his own business. His aunt and uncle provided him room, board, and transportation. The gas station was in a rural area by an interstate highway. It was open daily from 8 am to 8 pm. On the day of the incident, the clerk was working with his uncle. At 4 pm, his uncle left to run some errands. Soon after returning, he was called to leave again, so he planned to come back later to drive the clerk home. The clerk was now alone to serve customers, close up, and prepare for the next day. He also had access to the store's safe where the cash till was placed at closing time. He worked alone several times before when his uncle left to get supplies. The clerk was restocking shelves when, around 8:45 pm, two men wearing masks and gloves entered the front door. One had a long gun with an extended magazine, and the other had a bag and flex cuffs. The man with the gun aimed it at the clerk, who was walking toward the cash register. The clerk turned his back to the gunman and both began to struggle over the gun. The other man joined the struggle before the assailants fled in a pick-up truck. A passerby saw the men fleeing and called 911. First responders found the clerk dead from a single shotgun wound to the chest. Interior and exterior surveillance cameras recorded the incident. Police caught both assailants and a third accomplice two hours away the next morning. Following the incident, investigators found: The employer did not have an accident prevention program (APP) or separate workplace violence prevention program policies or training. The employer discussed robbery scenarios with the clerk and told him not to resist robbers. The store had never been previously robbed. REQUIREMENTS: You must provide your employees a workplace free from recognized hazards that are causing, or are likely to cause, serious injury or death. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences, employers should: Develop a written APP that has a workplace violence prevention program with store-specific standard operating procedure (SOP) policies for emergencies and robberies, initial and refresher worker training, and program assessment and enhancement requirements. Conduct a store security analysis to identify hazards, conditions, operations, and situations that could lead to violence. If possible, hire a certified retail security consultant to assess risks. Follow OSHA's recommendations for workplace violence prevention programs in late-night retail establishments (see link below). Avoid working alone and improve store layout, access, lighting, video surveillance, cash limit signage, cash control methods, and incident tracking. The driver was following his co-worker, who was driving a much larger truck. The larger truck became inoperable and had to stop in the right lane of the highway. He worked for his employer, a sheet metal roofing manufacturer, for 23 years. His duties included delivering roofing materials to construction sites. The driver was at a residential construction site where he was delivering roofing materials with a flatbed trailer. The materials consisted of metal sheets bundled in three 56-foot long wood crates and a few smaller crates. The three long, heavier crates were on the driver's side of the trailer. Normally, the driver used his own forklift to unload freight, but the longer load left no room on the trailer to bring it. Instead, he asked construction workers at the site if they could use their company's telehandler forklift to unload the trailer for him. A lead carpenter operated the forklift assisted by spotters at each end of the trailer. They unloaded the smaller crates first from the passenger side of the trailer. The driver then directed the forklift operator to lift two of the longer crates at one time from the same side so he could pull the truck forward from under them. He left the workers and went to the driver's side of the trailer to put cargo straps into the underbody toolbox. As the operator began to lift the crates, the third crate was inadvertently pushed off the driver's side of the trailer. When the construction workers heard the crash, they went around the trailer and found the driver crushed under the fallen crate, which weighed around 2,500 pounds. They used a skid steer to lift the crate off him, started CPR, and called 911. Medics arrived shortly after, but he had already died at the scene. Following the incident, investigators found: The crate base lacked enough clearance for the telehandler's forks to slide under it unobstructed. The telehandler operator and spotters had obstructed views of the load and workers on the ground. They also thought the driver was in his truck cab when they were unloading the trailer. The driver was not wearing a hi-visibility vest or hardhat. REQUIREMENTS: You must provide your employees a workplace free from recognized hazards that are causing, or are likely to cause, serious injury or death. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences, employers should: Provide initial and refresher cargo securement training for flatbed drivers that emphasizes pre-task planning; maintaining situational awareness and clear visibility of workers, vehicles, equipment, and loads; wearing high-visibility clothing and PPE; and identifying blind zones around trailers. Prohibit forklift operators from unloading freight on flatbed trailers unless they are trained and certified. Create and enforce a standard operating procedure (SOP) policy for unloading freight in their accident prevention program (APP) that requires truck drivers to perform a site hazard analysis of the unloading area and develop a method for preventing workers from being harmed by falling or shifting freight and movement of material handling equipment. Place high visibility warning signs at each end of the blind zone on the opposite side of the flatbed trailer from where the forklift or crane operator is unloading. Mark the blind zone even when a spotter is present, and make sure no one enters it. Verify signage and other safeguards before starting work. Wait in cab or designated safe zone when other workers operate forklifts or cranes.
