Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The father was working from a walk-board attached to two extension ladders with the use of ladder jacks approximately six feet above a concrete patio. The son was working from one of the extension ladders a few feet above his father. The father was positioning a sheet of insulation while the son was cutting it around a windowsill. Suddenly and without warning the father lost his balance and fell, striking his head on the concrete patio surface. He was taken by helicopter ambulance to the closest trauma center but did not survive the head injury. He died at the trauma center. The purpose of the FACE Program is to identify risk factors that can and do contribute to worker injury and death, and to make recommendations to employers and individuals on how similar events can be avoided. From the information collected about this incident the MO FACE investigator concluded that: 1. Alternative methods of fall protection should be incorporated when the risk of falling to the next lower level or ground is 10 feet or less, and where there is an unusual risk of injury AB - 2. Contemporary fall protection, including the use of a full body harness, lifelines and rope grabs should be incorporated when working from ladder-jack scaffolding at heights greater than 10 feet. Ladder-jack scaffolding is not approved for use at heights of more than 20 feet above the ground AB - 3. The victim attempted to retrieve a piece of plywood left nearby. He was wearing a full body harness but did not immediately hook his safety line to the cable that had been installed as part of a fall protection system. The victim took one step forward, reached down, and retrieved the plywood. When he stood up and stepped back, he fell between the damper blades to the bottom of the shaft 41 feet below. The victim struck his head on a cross member that was approximately two feet up from the floor. Emergency medical service personnel entered the shaft through an access door located at the bottom of the shaft and then transported the victim to a local hospital. The TX FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Design a fall arrest system and include it as part of the required personal protective equipment (PPE) for employees who perform work in this area. 2. Provide a training program for each employee who might be exposed to fall hazards. 3. Perform a job safety analysis (JSA) to determine what hazards employees may encounter while performing their work. 4. Develop an emergency action plan for employees that describes the rescue and medical duties to follow and ensures that all employees are knowledgeable of those procedures. The auger had stopped moving corn indicating there was a blockage at the auger intake inside the bin. Both men entered the bin from the access door at the top of the 50-foot tall bin. They had probes and shovels with them for the clearing work. They had left the auger running and probed the corn with metal bars around the auger opening in the middle of the bin floor. The corn was approximately 10 feet deep at the sides and 6-8 feet deep in the middle of the 36-foot diameter bin. The blockage suddenly cleared and the flowing corn immediately began to suck one of the workers down. The other worker was looking the other way and was alerted by his co-worker yelling for help. He tried to assist his co-worker to get out but struggled to save himself from being pulled down as well. He scrambled out of the bin, turned off the auger and summoned for help. The controls for the auger were outside the bin and during the time it took to get out of the flowing corn, climb to the top access door, and down to the ground controls, the victim was engulfed in corn. No fall protection devices or lifelines were used and there was no emergency stop system for the auger. The rescue crews arrived and had some difficulty accessing the 50-foot tall bin. There was a side door at the bottom of the bin, but it was still under corn and not used during rescue. The victim was taken to a regional health center but was pronounced dead on arrival. Recommendations based on our investigation are as follows: 1. Employers should provide rescue equipment, training and adequate supervision to ensure that safe practices are followed while workers are entering grain bins. 2. Grain handling facility owners should install "chairs" over the center unloading auger openings. 3. Farmers and grain storage operators should ensure that adequate measures are taken to avoid spoilage of grain during storage. The boys were riding in the truck's cargo area as it slowly moved in reverse, when the victim either jumped or fell off and then slipped while trying to jump back onto the truck bumper. He fell face down on the field road, and the truck ran over him. The father felt the truck bump into something, and getting out of the truck to check, he found that he had run over the victim. The father called for help to workers in the field, and a worker used a cell phone to call emergency medical services (EMS). The EMS arrived within minutes of the call, provided emergency care, and transported the victim to an area hospital where he was pronounced dead. The baler rollers suddenly started moving and trapped the worker's leg inside the baler. The rollers and belts spinning around the hay started a fire. A neighbor passed the field, noticed the fire, and notified the worker's father of the fire. The worker's father notified the fire department and went to the field. The father uncoupled the baler and moved the tractor away from the burning hay baler. The father then found his son (the worker) suspended in the baler. The worker died at the scene from smoke inhalation and burns. FACE investigators concluded that, to prevent similar occurrences, employers (including persons who are self-employed) should: 1. ensure that safe work practices are followed, including disengaging the PTO and shutting off the engine prior to working on agricultural machinery AB - 2. ensure that machinery is maintained and operated according to manufacturer's specifications, including adjusting the baler pick-up height for correct ground clearance and adjusting the ground speed with the crop conditions and windrow size AB - and 3. provide workers who work alone with a means of remote communication for use in cases of emergency. Department of Labor AB - A 37-year-old carpenter was killed when the wooden portable scaffold he was working on collapsed. He was crushed under the scaffold and the load that was on it. The victim and a coworker were both on the scaffold which was on the forks of a hydraulic lift. The lift had the scaffold raised approximately 25 feet. Both individuals were installing sheathing and plywood to the side of an apartment complex when the incident occurred. The load on the scaffold was too heavy for the way it was attached to the hydraulic lift. The wood around the forks of the lift snapped, causing the scaffold and the two individuals to fall. Personnel on site called 911 and the victim was life-flighted to the hospital where he was pronounced dead. The other worker fell clear of the scaffold and was not severely injured. He was transported by ambulance to the hospital. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences: 1. Employers should not use "homemade" scaffolds on lifting devices. 2. Employers should ensure scaffold platforms are securely fastened to the lifting device they are attached to. 3. Employers should develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard recognition and abatement. The farmer apparently turned the tractor and hay baler out of the hay row, dismounted the running tractor, and tried to pat the fire out with his hands. The power take-off device (PTO) was not disengaged. The farmer crawled under the hay baler to reach the fire. His fingers became entrapped in a roller and the fire spread, burning the farmer and five acres of the hayfield. The farmer's left hand was traumatically amputated, and he was able to free himself from the hay baler. He walked approximately one mile to his pickup truck and then drove to five residences before he was able to find someone at home to call for help. The farmer was taken to the local hospital and was subsequently flown by air ambulance to a burn center. He died two days later. FACE investigators concluded that to prevent similar occurrences, employers (including persons who are self-employed) should: ensure that safe work practices are followed, including disengaging the PTO and shutting off the engine, prior to working on agricultural machinery AB - 1. ensure that multipurpose fire extinguishers having an A-B-C rating are mounted on agricultural machinery AB - 2. provide workers who work alone with a means of remote communication for use in cases of emergency AB - and 3. ensure that machinery is maintained according to manufacturer's specifications, including removal of excess hay, twine, grease, etc. on a regular basis. The tractor, having been purchased six weeks earlier, had undergone maintenance work but was not equipped with a Roll Over Protective Structure (ROPS) or a seatbelt. After making one pass around the hillside bordering the road, he continued a second pass in a counterclockwise direction traveling northward, parallel to the gravel road. The tractor's right front wheel went over the edge of the embankment, causing the tractor to overturn and come to rest on top of the victim's head in the center of the road. A neighbor discovered him about a half hour later and called emergency medical services (EMS). In order to prevent similar incidents, the Kentucky FACE investigator recommends that: 1. older model tractors should be retrofitted with ROPS and seatbelts AB - 2. operators should wear seatbelts when on ROPS-equipped tractors AB - 3. tractor wheels should be weighted to afford maximum stability AB - 4. tractors should be equipped with front-end counterweights AB - 5. novice operators should familiarize themselves with both the equipment and the land prior to beginning activities AB - 6. courses for novice tractor operators should be offered AB - and, 7. rural counties should periodically update road maps/signs, instruct citizens to write out directions to their farms, and implement enhanced 911 systems. At the same time a dump truck, leaving the work zone, was traveling behind the victim in the adjacent lane. As the truck approached, the victim walked in front of the truck's right front bumper. He was not seen by the truck driver and was struck and run over by the right front wheel of the truck. They completed most of the work on the guardrails in the morning and early afternoon, so the victim was assigned to finish up the silt fence plowing. He was using a 1974 tractor with wide-set front wheels to plow a trench when a front tire struck a hidden concrete culvert. The victim was jolted from the tractor seat, while the tractor continued to roll until it came to rest on his back. The tractor was not equipped with a roll over protective structure (ROPS) or operator restraint system. To prevent future fatalities of this type, the FACE investigator recommends employers should: 1. Provide tractors that are fully equipped with an operator restraint system and rollover protective structures (ROPS). 2. Conduct a thorough evaluation of the terrain prior to beginning an operation with a tractor, and inform workers of the hazards and safe work practices to avoid them. Additionally, agencies responsible for setting standards for road construction should: 3. Develop contract requirements for marking structures in ditches that may be hazardous to individuals working in those areas. Another worker, who had arrived after the victim had fallen, climbed to the top of the standpipe and saw the deceased at the bottom of the standpipe. Emergency Medical Services (EMS) were notified. When they arrived on the scene they removed the victim and the coroner pronounced him dead at the scene. The TX FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Install fixed ladders with safety climb devices on the inside and outside of standpipes and a platform equipped with rails for workers to stand on when they cannot reach the control valve from the ground. 2. Periodically inspect all fixed ladders and safety climb devices. No one saw the victim get caught at the nip point, but his supervisor heard him call out and immediately pushed the emergency stop button. Because there was no way to reverse the conveyor, a welder was brought from another area of the plant to cut it in order to extricate the victim. In the meantime, emergency medical services (EMS) had been called AB - they arrived in less than two minutes. The victim was transported to a local hospital, and later transferred to a regional trauma center, where he was pronounced dead at 9:30 p.m. In order to prevent similar incidents, the KY FACE investigator recommends that: 1. employers should ensure that platforms allowing workers to reach overhead conveyors (i.e., work zones) are a safe distance away from ingoing nip points AB - 2. guarding should be placed around ingoing nip points AB - 3. trainers should never leave new employees unobserved in hazardous areas AB - 4. employers should develop, implement and enforce comprehensive written safety programs AB - and, 5. all workers should be trained to recognize and avoid hazards in the workplace. The farmer, who was sharecropping, was moving rolled hay from a field to a wagon in preparation for transportation to his farm. Hydraulics were used to lift the approximately 1500-pound spiked bale, and the farmer began driving toward the wagon. He proceeded along a tobacco field road, which was bordered on the left by a tobacco field and on the right by a three-acre grass covered sinkhole. (See figure 1.) The field road was approximately 12 feet wide and went along the upper rim of the sinkhole at a slope of nearly 25 degrees downward into the sinkhole. The tractor overturned 180 degrees, resting on the left wheel and pinning the victim against the ground. A farmer in a neighboring field witnessed the event and immediately came to assist the victim. Unable to assist, he went for help. The victim was pronounced dead by the coroner at the scene. In order to prevent similar incidents, it is recommended that: 1. tractors be retrofitted with rollover protective structures (ROPS) and seatbelts AB - 2. tractor operators evaluate field roads and determine a safe travel route prior to use of such roads AB - and, 3. tractor operators consider using rear-mounted attachments to transport round bales. He and another worker then checked the victim for a pulse, but found none. EMS received the call at 12:02 a.m. and reached the scene at 12:10, but could detect no vital signs. EMS workers called the coroner and extricated the victim from the machine. The coroner pronounced the victim dead at the scene. As they were being lifted up the derrick board was pulled off the forklift's forks by a crane's whip line and the two workers fell to the ground. The victim struck his head on a rock and died from a blunt force trauma to the head and multiple skull fractures. The TX FACE Investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Instruct employees that standing on a load being moved, lifted, etc., by a crane or a forklift (in this case a front-end loader equipped with forks) is an unacceptable work practice and should never be done. 2. Perform a job safety analysis (JSA) and instruct supervisors to use the JSA as guidelines for completing a task. 3. Establish procedures that ensure personnel and equipment are properly employed during the "rigging up" process. 4. Ensure that safety is built into and practiced in all phases of all operations and holding those in the line of authority accountable for deviations from accepted work practices. 5. Develop a system of successively heavier penalties for violation of safe work practices. He was directing a forklift operator where to place the forms on the trailer while also straightening the stack of forms. The victim told the forklift operator that the load was complete and the forklift operator put the forklift away in a nearby building. Upon returning from the building where the forklift was parked, the operator observed the two highest forms rocking on top of the trailer. The forklift operator ran to the driver's side of the trailer and observed the top form lying on the ground and the victim lying next to it. The operator then ran to the office where he told a secretary to place a 911 call. Emergency personnel arrived shortly after being called and transported the victim to a local hospital where he died later that day. Occupational Health Branch AB - A 19-year-old male laborer (decedent) died after being run over by the rear wheels of the front trailer of a tractor/trailer rig at a construction site. The victim was attempting to clean the dirt from the right rear fender of the double trailer rig that was being loaded by an excavator. The decedent walked from the left side and around the back of the tractor trailer rig to perform the cleaning. The decedent was standing in front of the rear wheels when the tractor/trailer driver begin to drive off. The excavator operator normally sounds one horn to move and two to stop to inform the truck driver of what movements he should make. The truck driver indicated he heard a horn and begin to drive away. The excavator operator stated that he did not sound the horn. The excavator operator saw the decedent being run over and sounded his horn twice to attempt to get the truck driver to stop. The decedent was not wearing a high visibility garment. The CA/FACE investigator concluded that, in order to prevent future occurrences, employers should: 1. ensure employees assigned tractor/trailer cleaning duties do so away from the loading site and with the truck drivers' knowledge. 2. ensure employees do not stand in front of the direction of travel of any machine capable of movement. 3. have traffic move only at the direction of a traffic controller at sites where traffic movement may be dangerous to employees. In addition, heavy machinery manufacturers should: 4. design a machine's warning horn and backup alarm to be extraordinarily distinct. When he leaned out during a wall washing operation, the platform tilted, and the decedent fell over the platform's guardrail dropping 12 feet to the concrete floor. The personnel platform had not been lifted by placing the forklift prongs into the channels in the bottom of the platform. The platform was just resting on the top of the forklift prongs since the decedent had lifted it from the side rather than the front. The decedent was not authorized to drive the forklift and, according to company management and two employees, had never driven the forklift prior to this incident. The top guard rail was 35 inches above the platform's base. There was no means to secure the platform to the forks or mast. The decedent was not wearing fall protection. The forklift operator who had lifted the decedent to the 12-foot height, was not at the controls of the forklift when the decedent fell. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should: 1. ensure employees do not operate machinery until they are trained and authorized. 2. ensure personnel platforms conform to industry regulations. 3. ensure forklift operators remain at the forklift controls when employees are in raised personnel platforms. 4. ensure employees working at heights wear personal fall protection. 5. develop and implement a comprehensive employee safety program. On the day of the incident, the victim and ten other workers were working along a section of railroad tracks that was being dismantled. The victim and two coworkers used a Caterpillar equipped with a bucket to pick up steel rail connector plates that were scattered along the railroad bed. Shortly before the incident, the Caterpillar was driven toward a wooden trestle that had a catwalk along one side. The catwalk was designed to support railroad workers walking across the trestle when railroad equipment was stopped on the trestle. As the Caterpillar neared the trestle, the victim and another coworker climbed onto it, one on each side of the cab. When the Caterpillar reached the trestle, it wasn't centered on the load bearing portion of the trestle but instead its right track traveled onto the catwalk. When the caterpillar reached the middle of the trestle, the catwalk began to break. The operator accelerated the caterpillar but before he reached the end of the trestle, a portion of the catwalk collapsed. The passenger riding on the right side of the caterpillar was thrown into a shallow creek at the base of the trestle. The Caterpillar tipped on its side and landed on the victim. The operator and the other passenger climbed from the scene and ran to a nearby road. They stopped a passing motorist and requested that a call be placed to emergency personnel. Emergency personnel arrived shortly after they were contacted, removed the victim from beneath the Caterpillar and pronounced him dead at the scene. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. minors should not be employed in any occupation identified by state or federal agencies as hazardous or detrimental to their well-being AB - 2. equipment operators should never allow passengers to ride along on equipment AB - and 3. employers should design, develop, and implement a comprehensive safety program. The victim had been driving the tractor on a paved surface and it departed the left side of the roadway and rolled over in a ditch. A coworker went looking for the victim and found him under the tractor. He called out to the victim but did not receive a response. The coworker then went back to his office (less than a half mile from the accident scene) and called 911. The victim was pronounced dead at the scene. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences: 1. Employers should ensure tractors are equipped with rollover protective structures (ROPS) and operator restraint systems. 2. Employers should ensure all personnel are thoroughly trained on equipment before allowing them to operate the equipment unsupervised. 3. Employers should develop, implement and enforce a comprehensive safety program that includes all employees (both permanent, part-time and temporary).
