Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

According to witnesses, the victim stepped onto a panel that had not been secured and it gave way causing him to fall. The victim fell to the gravel floor sustaining head, back and internal injuries. The fall occurred at approximately 8:27 a.m. The victim was pronounced dead shortly following the incident. Recommendations: 1. Employers should evaluate each worksite and each job for potential safety hazards and plan accordingly. 2. Fall protection must be provided and utilized whenever the potential of a serious fall exists. 3. A training program needs to be provided for each employee who might be exposed to fall hazards. 4. Employers need to obtain information about workplace hazards to which their employees may be exposed and take appropriate action to protect affected employees from these hazards. 5. The employer should set up a companywide safety training program and designate one person to be in charge of it. Company policy was to walk alongside the railcars and not ride them. Poles had been placed in-between sets of tracks which were used for fall protection by workers when they were working on top of the railcars. Since it was dark and there was no lighting, the victim did not see one of the poles. There was a 7 1/2 inch clearance between the pole and the railcar. The victim was caught between the pole and the railcar. The TX FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Install adequate lighting in the area designated as the no ride area. 2. Install concrete walkways between the sets of railroad tracks in the area designated as the no ride area. Employers should consider putting everything on a pallet which requires a forklift for handling. 3. Employers should consider implementing a spot inspection program to ensure all employees are complying with safety requirements and develop and enforce consequences for noncompliance. 4. Develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard recognition to include safe operation of forklifts. Department of Health & Human Resources AB - On December 1, 1998, a 25-year-old male tree feller (the victim) died of injuries sustained when he was struck on the head by a limb which had snapped off of the tree he was felling as the tree impacted the ground. The victim was felling trees on terrain with an average 35% slope. He was in the process of felling a yellow poplar. The poplar was approximately 24 inches in diameter at breast height and 90 feet tall. The tree was non-typical in that it had a large, 65-foot-long limb which was 18 feet above the ground. The limb was approximately 6 inches in diameter at the point of attachment and was oriented 90 degrees to the tree's trunk. The limb was pointing directly downhill, which was also the direction of the felling. After making the final cut, the victim retreated approximately 21 feet from the stump at an angle close to 45 degrees uphill from the cutting side. The victim was hit on the back of the head with the tapered end of the limb as it flipped back uphill towards the logger. A dozer operator discovered the victim shortly after the incident and notified the owner. The owner administered first aid and CPR. Another worker ran to a local residence and contacted EMS. The victim was life-flighted to the local trauma center and died the next day. The WV FACE Investigator concluded that, to reduce the likelihood of similar occurrences, employers should: 1. Ensure that tree fellers properly evaluate the timber and the area around the timber to be felled so that potential hazards can be identified and appropriate control measures implemented. 2. Develop, implement, and enforce a written safety program which includes, but is not limited to, training in hazard identification, avoidance, and abatement. 3. Designate a competent person to conduct frequent and regular site safety inspections. On the day of the incident the victim had been installing metal air ducts. Prior to falling, the victim and two co-workers were working from a scaffold that was 45 feet above the ground. The two coworkers had been working on one end of the scaffold which was near a large vertical air duct known as a riser. For reasons that remain unclear, the victim climbed over the scaffold's guardrail and onto a horizontal section of the air duct . The victim was not wearing or using any type of fall protection equipment. The victim fell from the air duct he had climbed onto, to the floor of the building. Emergency medical personnel were immediately called and the victim was transported to a local hospital where he died one and one-half hours later. The tractor lurched forward and knocked the victim face down to the ground, then pushed and pinned him as it moved forward. It stalled after running into a skidsteer loader in the machine shed, with the left rear wheel resting on his left shoulder and chest. A feed company deliveryman went into the shed to look for the victim after the victim failed to meet him in the farmyard as expected. He found the farmer on the ground, pinned by the wheel. Emergency medical services were summoned and arrived within fifteen minutes. The victim was pronounced dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, farm tractor operators should: 1. Be seated in the operator's seat before starting a tractor. 2. Place the transmission in neutral or park, set the brakes, and follow the manufacturer's starting procedure. In addition, farmers and farm workers with functional limitations caused by illness or injury should: 3. Seek and use the services of organizations and agencies that provide technical assistance and/or adaptive equipment to agricultural workers with disabling conditions. The victim was working alone installing wiring in an emergency egress hallway of a commercial establishment and apparently was unaware that the cable was energized with 277 volts. No personal protective equipment was in use while the electrician was working with the live wires. The victim was unobserved during the event, but was believed to be standing on the ground with the cable in one hand and a wire stripper in the other hand. He was found in the poorly illuminated hall by the employee of another subcontractor who came to the victim's work area to borrow a tool. Upon finding the victim on the floor the worker kicked the foot of the victim to see if he was awake and then noticed the arcing at the victim's chest where he was clutching the cable and the wire stripper. The worker hollered to others to deenergize the power and call 911 to activate the emergency medical services. Another worker at the scene shut off the power to the building, came to the aid of the victim, and found him pulseless. No one on the scene knew CPR. Within five minutes the police responded and the officer initiated CPR. The fire department medic unit arrived several minutes later and continued CPR until transporting the victim to the hospital where he was pronounced dead 70 minutes later. The Maryland FACE Investigator concluded that to prevent similar occurrences employers should: 1. Develop, implement, and enforce a lockout/tagout program for electrical circuits or equipment in accordance with the requirements of 29 CFR 1926.417. 2. Ensure that employees do not work alone in situations where a "buddy system" could prevent injury through greater attention to the safety requirements of a job. 3. develop a specific job site safety plan and ensure that care is taken to update the plan when job specifications are changed. According to multiple sources familiar with the case the victim exited the cab of the truck on the passenger side and was walking behind and to the side of the truck after signalling the driver to begin backing. The collector, walking behind the truck, stumbled, fell, and was unable to get out of the path of the truck before it ran over him. The truck rolled over the right side of the victim and stopped when his body was below the truck cab. The co-workers were unaware that the victim had been run over until a witness told them what had happened. Paramedics treated the victim at the scene before transporting him to the hospital where he died eighty minutes later. The FACE field investigators recommend that to prevent similar incidents in the future: 1. employers should develop, implement, and enforce a safety and health program that includes training on specific policies and safe work procedures for hazardous activities AB - 2. landfill operators should develop safe work procedures that minimize risk for all workers who may be exposed to maneuvering vehicles and other hazards associated with the operation of a landfill AB - 3. municipalities and employers should consider equipping vehicles with devices which will eliminate the blind spot behind trash collection vehicles by either providing the driver with a view of the rear of the vehicle or by sensing the presence of objects behind the vehicle. The victim removed the cover of the oven to access the circuitry. The oven was plugged in and turned on when the victim began to handle the circuitry. The witnesses heard a "pop", saw the victim jerk his hand out of the oven, and fall to the ground. One witness unplugged the equipment while the other witness checked the condition of the victim and then telephoned 911 to summon the emergency medical services (EMS). Paramedics responded within several minutes. The victim was transported to the hospital where he died from his injuries one hour and 15 minutes after the incident. The Maryland FACE investigator suggests that to prevent similar occurrences employers should: 1. ensure that diagnostic procedures performed on electrical equipment are done with the correct testing instruments and in conformance with the directions for their safe use. 2. ensure that repairs to electrical equipment are performed when the equipment is deenergized and/or removed from the live energy sources. Department of Labor AB - A 23-year-old mill operator and a 20-year-old maintenance technician were killed when they were engulfed in corn in a 6,000 bushel cone bottom gravity feed bin at a feedlot. The two individuals had been "knocking down" soy bean meal in a gravity feed bin next to the corn bin prior to the incident. For some reason, after their work in the soy bean meal bin they went to the corn bin and both individuals became engulfed in cracked corn. Both of these bins were active, flowing soy bean meal and cracked corn into a "clam" approximately once every five to ten minutes. The "clam" was then emptied into feed trucks. When the corn flowed from the bin, this pulled the victims under the corn. When others at the feedlot realized the two were trapped in the corn, they cut openings in the bottom of the bin to release the corn and rescue the victims. Both individuals died from suffocation. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences employers should: 1. ensure a permit is issued prior to entering bins. 2. ensure all equipment which presents a danger to employees is locked-out, blocked-off, or otherwise prevented from operating. 3. ensure employees are wearing appropriate fall protection anytime they are exposed to a fall into a bin or silo. 4. provide and have in place equipment for rescue operations which is specifically suited for the task being conducted. 5. develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard recognition and abatement. 6. consider installing locks on all accesses to bins and silos and controlling the keys to the locks. 7. consider installing phones in close proximity to bins and silos which are normally entered, or providing teams entering bins and silos with cell phones or two-way radios to provide communications to the mill operator. After riding the first conveyor approximately 50 yards, marks indicate that he fell down a chute approximately 6 feet to the lower conveyor, was carried another 50 yards before falling through a second chute, then traveled 150 yards to a point where the conveyor travels upward. The victim's body weight tipped the belt and caused him to fall off the conveyor prior to reaching the packer (final chute and conveyor). Recommendations based on our investigation are as follows: 1. Employers should with the participation of all workers conduct a job hazard analysis of all work activities. 2. Conveyor lines should be guarded from all belts and conveyor hopper and chute openings. 3. Belt conveyors that are fixed in place should have safety guards. 4. Employees should be instructed on the rules regarding safety around conveyors. 5. All workers should be thoroughly trained in safety aspects of dangerous equipment and made aware of all shut-off switches and safety devices. The victim and a co-worker were felling trees in separate work areas and hauling them to the landing with skidders. The victim used a log skidder he had purchased from another logger about 4 years before the incident. The skidder in this incident was over 20 years old, and had originally been equipped with a mechanical brake system. At some unknown time, the mechanical brake was replaced by a micro lock brake system. Throughout the morning, the victim and co-worker were in voice contact and shared a lunch break near the landing site. Around 3:30, the victim was felling trees on a slope with a 20% grade. He parked the skidder about 20 feet uphill in line with the tree he was felling. He engaged the micro lock system, kept the motor running, and positioned the blade near the ground. The skidder wheels were not blocked by any object, nor was the blade hooked over a stump to prevent the machine from rolling backwards. He apparently stood near the tree, facing downhill with his back to the skidder, and began a wedge cut with his chain saw. The incident was unwitnessed, but apparently the brakes failed and the skidder rolled downhill, striking the victim with the left rear wheel and crushing him against the tree. The co-worker heard the victim's cries for help and ran to the scene. After seeing the victim injured on the ground, the co-worker ran to his own truck. He tried to use his CB radio to call for help, but it didn't work. The co-worker drove his own skidder to where the victim was lying, and placed him in the skidder. He drove the skidder to his truck, carried the victim to the truck's passenger seat, then drove almost 15 miles to the hospital. The victim was taken to surgery, then transferred to intensive care where he died about eight hours after the incident. Sheriff's and coroner authorities were notified. The skidder motor was still running when the sheriff's deputies arrived the next morning. They turned it off and secured the area, then called OSHA. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Obtain and follow log skidder manufacturers' recommendations before using modified equipment. 2. Develop and enforce a safety policy that requires loggers to block a log skidder parked on a grade. 3. Provide personal communication devices to workers in isolated worksites. The conveyor started and stopped automatically as fuel was needed by the steam plant. The victim had apparently entered the enclosed area where the conveyor pulley was located to tighten the conveyor belt. This was a task he performed routinely while the conveyor was running. It appears the victim either stepped or fell onto an unguarded section of the moving conveyor. A co-worker found the victim after searching for him when he did not take his lunch break. When the co-worker entered the conveyor area, he saw the victim's body, pinned between a wall, the belt and the metal chute frame that extended over the belt. The belt was not moving, so the co-worker tried to free him. This caused the belt to move, so he stopped and used his radio phone to contact the control room to stop the belt. He also called for emergency medical services. The EMS first responder arrived in about one minute. Co-workers and EMS responders used a chain to lift the conveyor belt off the pinned portion of the victim's body, so he could be moved from the area. The coroner pronounced the victim dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Consider fitting belt conveyors with an mechanical, switch-activated conveyor management system that automatically adjusts belt tension and tracking. 2. Install guards on belt conveyors wherever personnel may have contact with the moving parts. 3. Ensure that lockout/tagout (LOTO) programs cover all workers while they perform adjustments that place them in contact with moving parts of operating equipment. The company manager was the victim's father, and he hired the youth to provide lawn care services for the summer. The victim had been using the tractor, which was equipped with a mower deck, to mow grass in the vicinity of the pond. When the rear wheel of the tractor became stuck in mud on the edge of the pond, he apparently got off the tractor to push it from the rear, and it rolled backward, pinning him under the tire. A delivery person arrived at the company after the incident occurred, and saw the victim pinned under the tire. EMS responders arrived within minutes of the delivery person's call for help. Company workers had initiated CPR, which was continued while the victim was airlifted to a hospital. He died the next day. To prevent future fatalities of this type, the FACE investigator recommends employers should: 1. Train tractor operators to shift the tractor into park, set the brakes and turn off the engine before dismounting from the tractor. For tractors without a park position, operators should shift into neutral, set the brakes and turn off the engine before dismounting. 2. Know and comply with child labor laws which exclude employment of youths less than 16 years of age in occupations which are deemed detrimental to their health or well-being by the Secretary of Labor. 3. Provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments. The victims worked as electricians for an electrical company that was subcontracted to complete the electrical work on an office tower complex that was under construction. The two victims were working on the sixth floor of a planned 30 story tower when they were struck by an ironworkers' bolt bin that fell from the eighth floor of the building. The bin was resting directly on steel decking, rather than having its weight distributed over two structural beams which was the usual procedure. Shortly before the incident occurred, a construction crane was used to assist ironworkers who were setting a structural column for the ninth and tenth floors. The column had been previously raised from ground level and placed in a horizontal position on the eighth floor decking. The lower end of the column was near a seam in the decking rather than in a central portion of the decking that provided a safer location for heavy materials. During the lifting process, the lower end of the column slipped off pieces of wood that had been placed on the steel decking to prevent it from slipping. The column punctured the eighth floor decking and dropped approximately seven feet. The column swung from the crane's cables and bent a structural beam that supported the ends of the decking on which the bolt bin was sitting. The ends of the decking sheets fell from the beam as a result of the beam being bent. The bolt bin and its contents fell through the eighth floor decking and continued to slam through each of the lower floors until it reached the first floor. The two victims who were working directly beneath the falling bin were apparently struck by it and also fell to the first floor. Emergency medical personnel were immediately called and pronounced the victims dead at the scene. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. employers should ensure that heavy items placed on preliminary flooring are placed over structural beams that can adequately support the weight of the items AB - 2. employers should ensure that heavy construction materials, whenever possible, are only raised to elevations as they are needed AB - and 3. employers should ensure that construction materials "such as columns" are placed in the safest locations available if they cannot be stored on the ground. His foot somehow caught on the lever which controlled the mast causing the mast to tilt toward the cage crushing the victim. The victim was found by a co-worker who summoned for help. The MA FACE Program concluded that to prevent similar future occurrences: Employers should examine plant layout to eliminate hazardous conditions. Manufacturers of forklift trucks should consider the following safety improvements: 1. The addition of a guard on the front of the operator's area AB - and, 2. Connecting the seat safety switch to the hydraulic load controls in such a way that the levers cannot be operated when the operator is not in the seat. 3. Employers should develop, implement, and enforce a comprehensive safety program that includes, but is not limited to, a thorough hazard analysis and utilization of controls particular to the job.
