  During 1975 the Health Interview Survey (HIS) questionnaire included a supplement on health insurance coverage, with special emphasis on whether persons covered by private plans were members of prepaid group practice plans (hereafter “prepaid group”). Members of pre- paid group plans were defined as including both those who belonged to plans classified as health maintenance organizations (HMO) and those who belonged to other prepaid group practice plans.
