Modifiers of Exposure-Response Estimates for Lung Cancer among Miners Exposed to Radon Progeny

  A review of epidemiologic studies of radon (10043922) progeny exposure was presented, with emphasis on factors that modify exposure to response models for lung cancer. The exposure to response model considered in this review was the linear relative risk model. The modifiers of relative risk estimates under consideration were attained age, time since last exposure, exposure rate and radon/smoking interaction. Many recent studies reported a decrease in relative risk per working level month with increasing age. It was suggested that only age specific risk coefficients would be meaningful. Several studies reported a decrease in relative risk with increasing time since last exposure, independent of increasing age. Animal and in-vitro studies reported that exposure rate was an important factor in addition to cumulative exposure, specifically that long term low level exposures were more carcinogenic than short term high levels of exposure. The interaction between radon exposure and smoking was reported to be intermediate between additive and multiplicative for risk of lung cancer. The authors conclude that there is no doubt of a causative link between radon exposure and lung cancer, although the estimation of excess risk per unit of exposure and identification of factors which affect the exposure to response relationship still need to be determined in order to better assess the risk of indoor radon exposure.
