Description:Hypertension, a common condition affecting nearly one in two of American adults, increases the risk of heart disease, stroke, and death. Compared to non-Hispanic White and Hispanic persons, African American persons are more likely to develop high blood pressure, at an earlier age, and with greater impact on health outcomes. The Hypertension Management Program (HMP) aims to improve the quality and management of patient care, and decrease the number of patients with uncontrolled hypertension.
