  The Hypertension Control Challenge recognizes clinicians and health systems that have excelled at controlling blood pressure. The Hypertension Control Champions use evidence-based strategies to achieve exemplary blood pressure control rates in their practices. As of 2017, CDC has recognized 83 clinicians, health care practices, and health systems as Million Hearts® Hypertension Control Champions for achieving blood pressure control rates of at least 70%.
  
  This handout highlights the work of three large health care organizations, Presbyterian Healthcare Services in Albuquerque, New Mexico; Kaiser Permanente Southern California; and Essentia Health in Duluth, Minnesota to help their patients manage chronic conditions like hypertension. These three large health systems, which server more than 4 million patients, have helped at least 70% of their hypertensive patients achieve a safe blood pressure label of 120/80 or below.
