Description:The purpose of this guide is to support public health practitioners and health care professionals who are focused on improving health equity by providing a detailed description of an intervention intended to address health disparities among heart failure (HF) patients. The Grady Heart Failure Program (GHFP) is located at Grady Memorial Hospital (GMH) in Atlanta, Georgia, and was designed to address socioeconomic barriers to care at the individual level. This guide is based on insights gained from an evaluation of the GHFP. The goal of the evaluation was to assess how the intervention reduced barriers to health equity, defined here as “every person has the opportunity to ‘attain his or her full health potential’” in cardiovascular disease (CVD).
