Description:The goal of this implementation guide is to support public health practitioners’ engagement of pharmacists in hypertension management through the Pharmacists’ Patient Care Process (PPCP). In this guide, we share lessons from an evaluation of a PPCP implementation through the Michigan Medicine Hypertension Pharmacists' Program (hereafter referred to as the Michigan Medicine Program).
This document is intended for public health practitioners and health care professionals who are interested in implementing a hypertension pharmacists' program rooted in the PPCP.
