Description:The Hypertension Management Program (HMP) is a team-based, patient-centered, integrated care model. The goal of the HMP is to improve the quality of patient care and decrease the number of patients with uncontrolled hypertension to improve each patient’s overall health and wellbeing. It can be implemented in many organizations, including in resource-constrained contexts, to promote hypertension control among patients, including those without health insurance. This toolkit was developed by the Centers for Disease Control and Prevention (CDC) Division for Heart Disease and Stroke Prevention (DHDSP) to provide healthcare organizations, including high-burden organizations, with the information and resources to implement the HMP and improve hypertension control among their patients. The purposes of this toolkit are to provide healthcare organizations: An overview of the HMP, its ten core components, and suggestions for implementing the HMP in clinical settings; and, Guidance to staff, administrators, and other healthcare professionals on how to implement and adapt the HMP for their organization. To complement this PDF toolkit document, CDC DHDSP developed an online toolkit that consists of interactive e-learning modules that are designed to guide learners through the key features of the ten HMP components and prepare them to implement the components at their organization. Both versions of the toolkit include links to additional resources that may support HMP implementation, as well as tips for adapting each component to suit an organization’s needs.
