Description:The Hypertension Management Program (HMP) is a team-based, patient-centered, integrated care model. The goal of the HMP is to improve the quality of patient care and decrease the number of patients with uncontrolled hypertension to improve each patient’s overall health and wellbeing. It can be implemented in many organizations, including in resource-constrained contexts, to promote hypertension control among patients, including those without health insurance. This toolkit was developed by the Centers for Disease Control and Prevention (CDC) Division for Heart Disease and Stroke Prevention (DHDSP) to provide healthcare organizations, including high-burden organizations, with the information and resources to implement the HMP and improve hypertension control among their patients.
