Description:A document was provided which contained several appendices for other reports. Appendix-A contained samples of the spirometry protocol, spirometry testing, and a trouble shooting guide. Appendix-B dealt with pulmonary function testing equipment and included a cotton study spirometry sheet. Appendix-C contained questionnaires. Appendix-D contained instructions on the questionnaires including a procedures manual for the questionnaire dealing with the characterization of byssinosis in segments of the cotton industry. Appendix-E contained the questionnaire to definition protocol for the classification of disease entities. Appendix-F contained medical notification forms. Appendix-G contained industrial hygiene protocol sheets. Appendix-H contained descriptions of comparison facilities. Appendix-I contained gin description form for statistical sampling. Appendix-J contained code numbers and geographic regions for all of the facilities in the NIOSH secondary cotton study.
