Description:The big obstacle to reducing accident rates today is considered to be lack of knowledge about the real causes. Reports made after the incident may be incomplete or focus on secondary causes instead of primary causes for special reasons. The role of physicians and nurses and other members of the occupational health and safety team who give first aid in removing the information gap is stressed. Recording of information gathered in casual conversation is recommended since such recording can reveal key pieces of information while events are fresh, and lead to procedural changes that can help prevent reoccurrence.
