  A survey questionnaire designed to gather from hospital administrators information on the physical characteristics of their hospitals and their current anesthesia practices is presented. The questionnaire asks for: 1) information on the physical characteristics of anesthetizing locations including number and size of rooms where inhalation anesthesia is administered and air changes, air supply, air conditioning, and air recirculation data on these rooms; and 2) inhalation anesthesia equipment and practices including anesthetic agents, breathing circuits, methods of anesthesia administration, and types of gas machines, nonrebreathing values, and ventilators. The information is intended for use in a criteria document on known health effects of chronic exposure to anesthetic gases and recommended control procedures and exposure levels.
