Workplace Violence Prevention for Nurses

    On average, over the last eight years healthcare and social assistance (HSCA) workers have accounted for 62% of the nonfatal workplace violence injuries involving days away from work. With a rate of 9.0 per 10,000 full-time equivalents (fte), the risk of HCSA workers experiencing an assault that requires time off from work is 4.5 times greater than the overall workforce rate of 2.0 per 10,000 fte. These numbers and rates do not represent the assaults that resulted in less severe physical injuries or the psychological trauma that healthcare workers experience from workplace violence. Vida Health Communications, Inc. (Vida) produced and evaluated Workplace Violence Prevention for Nurses, an on-line course for nurses working in hospital settings. The course addresses theory, practice, and policy implications of the growing problem of violence and abuse experienced by nurses in the workplace. Emphasis is placed on violence prevention, and the immediate and long-term health and safety of front-line nurses. The self-paced curriculum consists of 14 lesson units covering important foundational and background information, prevention, assessment and intervention strategies. Five case study scenarios reinforce the teachings of the course. All units and case studies make extensive use of graphics, video, and interactive activities to engage learners and reinforce learning objectives. In particular, video is used to deliver engaging personal testimonials from actual nurses about real-life experiences with workplace violence. At the time of submission of this report NIOSH is pilot testing the course to meet application requirements for awarding of continuing education credits. The course was evaluated using a randomized control trial of 204 registered nurses working in hospital settings. Nurses who took the course: Made substantial gains in knowledge about violence in the workplace, and how to prevent, assess, and respond to violent incidents. Expressed more positive attitudes toward intervening to prevent violence and reporting it after the fact. They expressed greater belief that the target behaviors - such as reporting incidents, seeking counseling, etc. - would be effective to reduce violent incidents, greater confidence in their ability to implement the behaviors, and greater intent to do so. Who experienced or witnessed violent incidents in the past month were more likely to seek or recommend counseling for those involved and were more likely to advocate for policy change. The project demonstrated that a well-designed and relatively brief learning intervention can shift attitudes and behaviors of nurses so that they become more engaged in a process of workplace violence prevention. The investigators believe that this course can serve as a core building block to improve the outcomes for all nursing workplace violence prevention programs through improved workforce engagement and empowerment.
