Comparison of vibrotactile thresholds with physical examination and electrophysiological assessment

Description:The use of vibrotactile thresholds was compared with electrophysiological testing and physical examination in determination of the functional integrity of patients' somatosensory systems. The 79 participating patients, evaluated at the Mount Sinai Hospital Neurophysiological Laboratory, were subjected to nerve conduction studies and standard sensory function examinations. These testing results were compared with vibrotactile threshold values obtained using an electromechanical device. Significant associations were found between vibrotactile thresholds and physical examinations (vibration perception, proprioception, and pain perception) for all digits tested in the upper and lower extremities. Significant associations were also seen between vibrotactile thresholds and electrophysiological evaluations of the median, ulnar, tibial, peroneal, and sural nerves. The most significant association occurred between great toe vibrotactile thresholds and late response latencies in lower extremity nerves. The authors conclude that vibrotactile threshold determinations may be useful for measurements of large fiber nerve function and when electrophysiological study is impossible.
