  A new approach for collecting farm accident data was described. The approach was a personalized mail questionnaire survey that avoided signs of being a mass mailing. Each mailing consisted of an attractively laid out questionnaire coded to insure confidentiality, an individually typed and signed cover letter that conveyed to the respondent in direct simple language the importance of his response, and a prepaid addressed return envelope. The envelopes addressed to the subjects were individually typed. The cover letter include the telephone number of the researchers and an invitation to call collect if there were any questions. The approach was tested by sending the questionnaire package to 1200 farm operators representing a random sample of 2.12 percent of the farm operators in the state of Pennsylvania. Three followup contacts were made after the questionnaire package was mailed. Seventy six percent of the forms were returned after the four mailings. Ten percent of the forms were not useable. A total of 117 accidents were reported by 101 responses yielding an accident rate of 0.143 accidents per farm. The total printing and mailing costs of the survey amounted to 1,391 dollars. The authors conclude that a personalized mail survey technique is a useful approach for collecting agricultural accident data and related information. [Description provided by NIOSH]
