    A potential relationship between histocompatibility (HLA) antigens and the expression of coal workers' pneumoconiosis was investigated in a population of active and retired coal miners in Pennsylvania and northern West Virginia. A total of 277 coal miners were included, with a mean age of 56 years and a mean mining experience of 34 years. The subjects were divided into three experimental groups according to the clinical expression of pneumoconiosis. Group- I included 35 percent of the subjects that showed no radiologic evidence of the disease. Group-II included 31 percent of the subjects who were diagnosed as having simple pneumoconiosis, and Group-III included 34 percent of the miners who had progressive massive fibrosis. HLA typing utilized a two stage microcytotoxicity method after separation of the lymphocytes from other blood components. Standard immunohematological methods were used to determine the ABO, Rho(D), and MN blood groups. Serum immunoglobulin-G, immunoglobulin-A, and immunoglobulin-M levels were determined with a microfluoronephelometric analyzer. No significant differences in the frequencies of the blood group antigens were found among the three clinical categories. HLA1 antigen and antigen W18 occurred significantly more frequently among miners in Group-I than those in Groups-II or -III. Risk factor analysis indicated negative associations between both antigen W18 and the HLA1 antigen and the development of complicated pneumoconiosis. The authors suggest that these two antigens are associated, respectively, with resistance to development of progressive massive fibrosis and of simple and complicated coalworkers' pneumoconiosis.
