A pre-triage system for mass casualty care.

  The disaster program at O'Hare International Airport in Chicago was used to illustrate the problems encountered in mass casualty care. The program was based upon a two phase pretriage program aimed at getting basic life saving services to the casualties in the shortest possible time. Specific problems encountered in the disaster scenario included the type and location of the accident, difficulty in the extrication of victims, traffic congestion, inclement weather conditions, and the administration of immediate life saving care for the casualties. The two phases of pretriage care at the airport included the on-site care of casualties during extrication, and stabilization of the casualties at the main airport fire department. Transportation to the hospital after the second phase was considered the third phase of pretriage care. Essential components of the pretriage program included preplanning for traffic control, highly mobile equipment for sustaining life, trained personnel, back up equipment for stabilization of casualties, and preplanned methods of transportation to area hospitals. Equipment, back up supplies, staff training, and emergency notification were further described.
