Description:An outbreak of skin problems among employees at the Kroger Company's (SIC-5411) Western Hills store in Cincinnati, Ohio was evaluated. Local 1099 of the Retail Store Employees requested the evaluation. Medical questionnaires were administered between August 14 and 19, 1980. In August and September 1981, further surveys were performed at Kroger stores in other locations. In the initial survey, 18 cases of dermatitis were identified and diagnosed as phototoxic contact dermatitis. Thirteen of the 18 cases involved sackers. Forty two percent of sackers interviewed had the dermatitis. In the second survey, 21 of 41 sackers interviewed had the dermatitis. Affected sackers spent 2 daylight hours working outdoors compared to unaffected sackers. Significantly more affected sackers came into frequent contact with unpackaged celery. The prepackaging of celery failed to prevent a recurrence of the dermatitis. Of 47 sackers interviewed in other cities, only two had very mild cases of dermatitis. The authors conclude that there was a high prevalence of phototoxic contact dermatitis among sackers at several Kroger stores in Cincinnati. A search for the phototoxin responsible has been unsuccessful. Protective clothing and barrier creams are recommended as well as a medical surveillance program.
