Description:The case of a fatal injury to a construction foreman while supervising construction in a subway tunnel was examined. The 59-year-old male foreman was tagging a load being moved by a crane on May 18, 1982, when he was struck on the head and killed. During the lowering of a load into the subway work area by a crane which did not have its outriggers extended or set on the ground the load became caught on a ledge, causing the crane to tip over when the load suddenly became loose from the ledge. The foreman was struck on the head and killed by the crane boom as it fell toward the ground. The main causative factor in this incident was the nonusage of the outriggers to stabilize the crane, probably in an effort to minimize the street area being blocked during the operation. It was recommended that the importance of the use of outriggers be stressed and that personnel tagging crane loads be educated to observe both the load and the crane during construction operations.
