Description:On January 26, 1984, a supply motorman at an underground coal mine was fatally injured by a crib block that fell off of a forklift while being loaded onto a mine flatcar. The incident occurred when two crib blocks which had been loaded onto a mine flat car were partially lifted off the car to scrape dirt and ice off of the bottoms in an attempt to get the blocks to settle properly on the car. During the second attempt to clear the bottom of the blocks one of the cribs fell on the supply motorman who was trying to scrape them. When the load was lowered after the fall, the forklift operator saw that three of the four half inch steel bands that run around each crib block were broken and the cribbing was scattered on the car and ground. The cause of death was determined to be multiple traumatic injuries. It was concluded that this incident occurred due to the lack of experience and training of both the forklift operator and the victim, although both had many years of experience in the field. It was recommended that the Safe Work Instructions for aboveground workers be upgraded, and a formal training program be developed.
