Description:

A 34-year-old male mill operator died after his upper torso was caught between two rollers on the rubber lamination machine he was operating. The employer was a manufacturing company that produced rubber compounds which were ultimately used in the manufacture of automobile parts. At the time of the accident, personnel at the site were engaged in producing fabricated rubber compounds. Raw rubber was loaded through a series of rollers while fibrous material was simultaneously loaded through a separate series of rollers. The materials ran through a graduated series of machine rough trim rollers, ply rollers, and a set of final trim rollers. At the time of the incident, although unwitnessed, the victim apparently was under the final trim roller removing excess material when his right hand became caught between the final trim roller and the idler roller which pulled his arm and upper torso between the rollers, crushing his chest. There was a machine shut off safety tripwire cable present in the area where the victim was working, but the device was inoperative. Recommendations include ensuring that emergency shut off devices are checked for proper operation before each shift, providing machine guarding, and considering the use of special hand tools for use in proximity to moving machine parts.
