Shift Supervisor Dies From Injuries Received After His Arms Were Caught Between Two Paper Machine Rollers–Tennessee

  This report concerned the death of a 30-year-old male shift supervisor whose arms were caught between two rollers on the paper machine as he was inspecting it. The employer was a paper manufacturer that produced high quality coated and noncoated paper. On the day of the accident a paper break occurred at the area where the continuous sheet located on the woolen felt screen traveled over the bottom roller. The supervisor entered the unguarded access roller area without first raising the top roller. Apparently, the victim was standing directly in front of the operating rollers nip point, feeling the woolen felt screen, when his hand or hands became caught between the top and bottom rollers. His arms were then pulled up into the rollers at the nip point. A coworker saw the victim caught in the rollers and raised the top roller, allowing the victim to fall to the floor. His arm had been partially amputated at the shoulder area. He died due to extensive blood loss. Recommendation stemming from this accident included that access to hazardous areas be sufficiently guarded, that permanent interlocking gates be provided to protect workers from hazardous areas, that reviews and revisions of current safety programs be carried out, that danger and warning signs and placards be placed at conspicuous places regarding hazards, that unscheduled worksite inspections be performed, and that evaluations be performed at other sites in the facility for similar type hazards.
