Janitorial Worker Dies As Result of Injuries Sustained In Fall With Personnel Lift–Virginia

  This report concerned the death of a 51-year-old male janitorial worker who fell 14.5 feet, landing on the floor of a parking garage. He was employed by a janitorial service company which had no safety program or formal safety training. He and a coworker were dusting the drainpipes which were hanging from the ceiling. To reach the pipes they had used a manually propelled personnel lift which extended 14.5 feet. Once they had finished dusting an area, they partially lowered the lift and a worker on the ground pushed the platform to the next section. The outriggers were not extended. In the area of the incident, the floor sloped about 1 degree. At the time of the accident the lift tipped over and the victim struck his head on the pavement. He was transported to the hospital and died 12 hours later from blunt craniocerebral trauma. Recommendations included ensuring that such lifts are properly erected and that all safety features have been made operational, including outriggers, that workers are properly trained in the use of all equipment, that suitable equipment is used for every task, and that a competent person be designated and trained to perform regular safety inspections.
