Description:This report concerned a 47-year-old woman who fell 60 feet from a continuous motion belt manlift. She worked as an area operator for a food additive manufacturer
there were 156 workers on two 12-hour shifts. The operations at the facility covered five floors and access between floors was by means of a continuous motion belt manlift operating through 36-inch diameter openings. The victim had picked up a clipboard on the second floor and gone to take final gauge readings on the fifth floor. She was found unconscious on the second floor at the base of the manlift about 20 minutes later. She was pronounced dead at the scene. The cause of death was severe trauma to the head and chest. Although no one witnessed the fall, she apparently fell from the manlift between the fourth and fifth floors. The manlift was still running, although the emergency stop bar at the top of the manlift had been displaced
the on/off switch was found to be faulty. It was recommended that employees be instructed to exercise extreme caution when using equipment that by design exposes them to potential fall hazards and that they adhere to established safe work procedures.
