Description:The collapse of a cement filled tank at a concrete facility resulted in the deaths of a 31 old truck driver and a 29-year-old truck driver. The manager of the facility was loading trucks when the system which transported the aggregate to the weigh hopper under one tank began to malfunction. The manager began a trouble shooting procedure to determine the cause, and the two truck drivers decided to help while waiting for their trucks to be loaded. The manager noted that the drive belt from the motor to the head roller on the conveyor was slipping. One of the men assisting had ascended about one third of the way up the belt which was locked out and tagged. At this point the tank started to topple toward the belt. The three workers were trapped beneath the tank. The manager was injured, and both truck drivers died. The first victim died of multiple internal injuries and the second due to massive head injuries. Subsequent investigations revealed severe corrosion to the legs supporting the tank. Recommendations noted that regular structural inspections and evaluations be made to ensure that facility structures are maintained in good repair.
