Description:This report concerns the death of a 43-year-old male who was president/co-owner of a company that designed, fabricated, and installed advertising signs. The subject died after being pinned to a concrete driveway by the rollover/falling object protection structure (ROPS/FOPS) on a forklift he was operating when it overturned. The company had received a shipment of steel tubing via flatbed tractor trailer, which had to be unloaded. The victim began unloading the flatbed trailer using a forklift. After loading about two thirds of the steel tubing to the forklift, he drove the forklift with the forks about 6 inches above the concrete driveway, down the left side of the trailer. As he approached the rear of the trailer, traveling about 5 miles per hour down a 3% grade, he turned left to go behind the trailer. The forklift began to tip over and the victim tried to jump free but was pinned. Recommendations were made that only trained and authorized personnel be permitted to maintain, repair, adjust, and inspect industrial trucks and only in accordance with manufacturer's specifications; that employers retrofit forklifts with seat belts; and that employers inspect ROPS/FOPS for damage and repair where needed.
