Teen Roofer Electrocuted when Ladder Contacts High Voltage Power Line

Description:On Monday, September 10, 2018, a 16-year-old male roofer (the victim) attempted to maneuver a fully extended 25-foot ladder when he lost control, causing the ladder to fall backwards and contact a 7200-volt electric power line, electrocuting him. Contributing Factors: Key contributing factors identified in this investigation include: 1. Work performed outside youth employment regulations. 2. Lack of hazard recognition and safety training. 3. Use of a conductive ladder around high voltage lines 4. Transporting an extension ladder in the vertical position. Recommendations: FACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Perform a job hazard analysis of the worksite. 2. Become familiar with and comply with all federal, state, and local regulations associated with youth employment. 3. Use non-conductive ladders around power lines. 4. Lower the extension ladder and transport it horizontally.
