Construction Foreman Falls to his Death from a Roof

  The case of a 53 year old male construction foreman who died when he fell from the roof of a building under construction to a dirt floor 30 feet below was examined. The man was employed by a general construction company where he had worked for 8 years. No written task specific safety rules or procedures existed, but general safety rules and procedures were available. The roofing/sheet metal work on a building was being performed by the company. The victim and a coworker went to the roof to begin applying fiberglass insulation over the bar joists. The victim stepped from the edge of the roofing panels out onto the 5 inch bar joist, lost his balance, and fell to the ground. Cause of death was listed as multiple traumatic injuries. It is recommended that fall protection equipment is provided and used by the workers whenever work is performed at an elevation. The hazards presented by this particular job should have been covered in open safety meetings prior to beginning the job. The employer should have reviewed the current safety program and incorporated written safety rules and procedures for specific tasks.
