Description:The case of a 29 year old president of a roofing company who fell approximately 52 feet to his death from the edge of a roof to a concrete entryway at ground level was examined. The company employed four workers. No written safety policy of program existed. The company had only been in existence for about 6 months. The contractor was responsible for completing roofing operations on a local high school after the original roofer went out of business. Workers gained access to the roof location using a hydraulic lift with guardrails. Workers complained about the jerking motion of the lift. At the time of the accident the victim and two coworkers rode the lift to the edge of the roof. One opened the guardrail gate and stepped onto the roof. As he began to follow, the victim instructed the remaining coworker who was operating the lift to lower the platform. As the coworker activated the lift controls, the platform jerked and the victim fell from the roof. It is recommended that equipment should not be used if mechanical problems are reported. Only qualified personnel should be allowed to operate mechanical materials handling equipment. It is not clear in this case if the contractor assessed the qualifications of these individuals who operated the lift.
