Description:Under the 1705 cooperative agreement with CDC, the National Association of Chronic Disease Directors (NACDD) and its state and local partners focus on reaching people with disabilities (PWD) as a priority population for recruiting, enrolling and retaining participants in the National Diabetes Prevention Program (National DPP) lifestyle change program. Additionally, NACDD seeks to recruit, enroll, retain, and obtain reimbursement for Medicare and Medicaid beneficiaries, where applicable. In response to local and state partner feedback, NACDD worked with the Lakeshore Foundation and National Recreation and Park Association (NRPA) to undertake a ‘Partnership Mapping Project’ to identify potential partners at the state and local level to engage with to:
1. Increase awareness about the National DPP; and
2. Increase referrals into local Nat DPP lifestyle change programs.
