This guide was adapted from material written by Heidi Behforouz, M.D., and staff of Boston’s Prevention and Access to Care and Treatment (PACT) project and distributed by the New York City Department of Health and Mental Hygiene through their Care Coordination Program.
Dr. Behforouz is the Executive Director of PACT, a community-based project in inner city Boston, whose mission is to improve health outcomes for underserved individuals with HIV/AIDS. PACT was a project of Brigham and Women’s Hospital and Partners in Health, a non-profit health care organization dedicated to providing a preferential option for the poor.
We acknowledge the support provided by the Centers for Disease Control and Prevention (CDC) through Cooperative Agreement # U62PS004242 for the revisions of this product under the STEPS to Care (StC) project. The StC Project supported the development and pilot implementation of the StC online toolkit that is designed to support diverse agencies in the uptake and implementation of three evidence-informed strategies to improve retention in HIV care and reduce viral loads: Patient Navigation, Care Team Coordination and HIV Self-Management.
