Description:The Racial and Ethnic Approaches to Community Health Implementation Guide contains guidance and resources to help implement the required strategies under the REACH Notice of Funding Opportunity (NOFO). The REACH program funds communities to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease (i.e., hypertension, heart disease, Type 2 diabetes, and obesity). Recipients will work with one or two of the following five priority populations including African Americans/Blacks, Hispanic Americans, Asian Americans, Native Hawaiian/Other Pacific Islanders, and American Indians/Alaska Natives.
Recipients must work in three of the four strategies (tobacco, nutrition, physical activity, and community-clinical linkages) and their accompanying activities. This guide provides background information for each of the strategies and potential recipient activities to support implementation.
